Effect of hand washing depends on method as well as frequency. However, there has not been objective data or analysis focused on this subject. So, this study intends to provide the effective intervention which are education and feedback for improving quality and frequency of hand washing. This study is pretest-posttest experimental design on frequency and method before and behind providing hand washing education and feedback. The subjects were comprised of 25 staff nurses who cares patients directly in Cardiac Surgery Intensive Care Unit at 1 general hospital in Seoul, Korea. The data was collected by hand washing tool and observation sheet designed by researcher. The data was collected from May 7, 2004 to May 28, 2004. Lecture using hand washing education material developed by researcher, discussion and one to one hand washing instruction were used. The feedback was performed by camera mobile image and notice board including the result of hand washing notice. Result variations were hand washing frequency and method. The collected data was analyzed by SPSS 10.0/PC. The results of this study were as follows, 1) Hand washing frequency was improved 48.6%(1,308/2,692) behind intervention as compared to 28.9%(1,014/3,510) before intervention(p<0.001). 2) Hand washing frequency by work shift was 28.7%(514/1,788) before intervention, 47.3%(673/1,424) behind intervention in day shift, 29.0%(500/1,722) before intervention, 50.1%(635/1,268) behind intervention in evening shift(p<0.001). 3) Hand washing frequency compared by ratio of total admitted patient number versus working staff nurse number during the observation time was improved after intervention regardless nurse to patient ratio(p<0.001). 4) Hand washing classified by nursing activities was all improved in casual contact with patient, invasive equipment insertion, invasive equipment management, wound contact, invasive treatment with equipment, applying aseptic technique except blood, body fluid, mucosa and contaminated substance contact(p=0.204). 5) Hand washing methods were improved from mean 2.08, 2.14 in before and after activity each before intervention to 3.02, 3.49 in before and after activity each behind intervention(p<0.001). 6) Hand washing agent, hand washing time, rubbing area, drying method after hand washing were all improved behind intervention(p<0.001). Hand washing education by lecture, discussion and one to one demonstration and providing feedback of posting the result of hand washing by camera mobile image and notice board including the result of hand washing notice has been proved to enhance both of the effect of hand washing frequency and method. Otherwise, there was a limitation on just frequency of hand washing intervention programs in previous studies, this study investigates significant improvement both of frequency and method of hand washing.
