혈액 속에 포도당이 다량 축척 된 것을 고혈당(Hyperglycemia)이라 한다. 많은 당료병 환자들에게서 혈관질환(Vascular disease)과 당료병성 신증(Diabetic nephropathy) 증상이 나타나며, 이는 축척 된 혈당과 밀접한 관련이 있음이 알려져 있다. 여러 연구결과에 따르면 고혈당으로 인한 NADH/NAD+가 증가하게 되면, 당 대사 효소인 GAPDH 활성이 저해되어 비정상적인 신호가 전달되게 된다. 그 중에서도 특히 glyceraldehydes-3-phosphate가 증가된 NADH의 도움을 받아 DAG로 전환되어 protein kinase C(PKC)의 활성을 증가시킨다. 활성이 증가된 PKC는 여러 가지 이상 신호전달을 하여 활성산소(reactive oxidative species)를 유발시키고, mitogen-activated protein kinase(MAPK)신호전달 단백질에 비정상적인 인산화를 유도하는 것으로 보고되어 있다. 또한 세포 외 기질(ECM: extracellular matrix)의 증가와 cytokine인 transforming growth factor(TGF-1)등을 유발하여 당요병성 신증(Diabetic nephropathy)을 일으키는 주요 원인이 되고 있다. 이러한 원인에서 최근 PKC inhibitor의 개발이 이루어지고 있으나, 고지혈증, 고혈당 등의 부작용 때문에 아직은 제제로서의 가치가 낮다고 할 수 있다. 이에 본 연구는, 한국에서 자생하는 참 당귀 뿌리로부터 coumarin ring구조를 가진 Decursin을 추출하여 유도체화시킨 DD-004를 가지고 당료병성 신증 경감효과를 확인하고 그에 따라 발현되는 단백질의 양상을 조사하였다. 정상적인 신장 세포(RMC)에 인위적인 당뇨를 유도하여 DD-004 처리 후 세포 내에서 변화가 생기는 단백질을 확인하기 위해 이차원적 전기영동과 은-염색과정을 거쳐 차이가 생기는 단백질들을 MALDI-TOF와 TOF/TOF로 분석하여 동정하였다. 그 결과 정상인 신장세포에 비해 당뇨성 세포에서 증가하거나 감소하고 DD-004에 의해 정상으로 회복되는 단백질 45개를 찾아 조사하였다. 이번 실험 결과로 이미 보고 된 PKC와 ROS관련 단백질들을 확인할 수 있었으며, 이 밖에 새롭게 조사된 단백질들 또한 직-간접 적으로 관련이 있을 것으로 추정된다. 신장세포 수준에서 변화된 단백질들의 연구를 통해 당뇨병성 신증(Diabetic nephropathy)의 복잡한 기작을 이해하는데 많은 도움이 될 것으로 예상되며 새롭게 조사된 단백질의 기능 연구가 앞으로 과제이다.

Hyperglycemia seems to be an important causative factor in the development of atherosclerotic vascular disease and diabetic nephropathy in patients with diabetes. Many studies have demonstrated that high glucose concentration increase de novo synthesis of diacylglycerol(DAG) with subsequent activation of protein kinase C(PKC) and oxidative stress. Moreover, the activated PKC phosphorylates mitogen-activated protein kinases(MAPK), such as extracellular regulated kinase(ERK). Recent studies have revealed that high glucose induces intracellular reactive oxygen species(ROS) in mesangial and tubular epithelial cell. High glucose-induced ROS in mesangial cells can be effectively blocked by inhibition of protein kinase C(PKC), NADPH oxidase and mitochondrial electron transfer chain. And actin cytoskeletal disassemblywas a prominent feature of human diabetic nephropathy. Decursin(Dec) was extracted from the root of Angelica gigas.Which known to protect renal injury due to the diabetic nephropathy. In this study, used decursin derivative(3-methylbut-2-enoic acid-7-hydroxy-2, 2dimethylchroman-3-yl ester: DD-004), The DD-004 shown that down-regulation of PKC and decrease of MAPK activation in renal mesangial cell. Here weinvestigated the DD-004 effect under high glucose stress by proteomic analysis. Proteins were separated by 2-DE and analyzed by image analysis software. And selected spots were digested with trypsin. Protein identification was done by peptide mass fingerprinting with matrix-assisted laser adsorption ionization-time of flight(MALDI-TOF) and(MALDI-TOF/TOF). These data shows that the High glucose stress resulted in the up-regulation of at least 21 proteins and in the down-regulation of at least 24 proteins, and differential protein expression was recovered by DD-004(20M) treatment. These results suggest that differential proteins involved in the PKC signaling pathwayand actin cytoskeleton regulatory proteins synthesis. The differential expression of renal proteinsis necessary to understand better the complex pathogenic mechanisms of diabetic nephropathy.
