This study was objectively performed to identify dietitians' job in the elderly health-care facilities, to assess facilities and dietitian's demographic characteristics, and to determine performance and importance of dietitian's job analysis including the demand of therapeutic diet development. Survey was conducted by mail and samples were drawn among dietitians working in 376 facilities which capacity is over 50 members from nationwide 583 the elderly health-care facilities, appeared inStates of 2006 seniors citizens welfare institutions. Returned questionnaires were 102 and used for statistical analysis (response rate: 27%). The demographic characteristics of the elderly health-care facilities were classified as the 39 elderly nursing facilities(38.2%), the 33 specialized nursing facilities(32.4%), 13 geriatirc hospitals(12.7%), the 9 elderly economic nursing facilities(8.8%), the 3 paid specialized nursing facilities(2.9%), the 1 paid nursing facility(1.0%), and the other 4 facilities(3.9%). Capacity size of the facilities shown the highest frequencies was 50~100 numbers, corresponding 52.9% of total samples. 52.5% of the samples showed its menu price as 1000-1,500 won. A cycle-menu program was in-use at the 87.3% facilities, but only 6.9% facilities have been introduced a selected menu system. 93.1% facilities employed only one dietitian and average numbers of employee working in foodservice department was under 2 or below. In the demographic characteristics of dietitians, the major age range of dietitian was 25~29 years old, 40.9% dietitians had 1 to 3 years of work experiences in health-care facilities, and only 14.7% dietitians had a medical dietitian license. At a question on the periods of requiring practices or internship, 51% of respondents answered at least 1 to 3 months. In addition 36.3% of dietitians responded to be familiar with their job from 6 months to 1 year required. Job activities of dietitians in the elderly health-care foodservices was identified as 55 activities with 9 dimensions. Job performance score evaluated dietitian oneself was 4.71 of 7 points. Average score for 9 dimensions showed as menu management 5.77, purchasing management 5.32, production management 5.98, nutrition management 3.10, sanitation and safety management 5.71, work management 5.65, financial management 3.33, marketing 3.18, and human resources management 4.80, respectively. The average importance score that dietitians evaluated their own job was 5.66 points of 7. The importance scores by 9 items as the followings: menu management 5.92, purchasing management 5.63, production management 6.28, nutrition management 5.20, sanitation and safety management 6.33, work management 5.73, financial management 4.30, marketing 4.79, and human resources management. Results from the dietitian's job activities of 45 items showed that assembly and service management (6.81), stored meal management (6.71), meal tasting (6.67), sanitation management (6.64), purchasing and receiving of food material (6.57) were performed highly and dietitians perceptive job importantly were checking use-by date of the food products (6.68), regular-diet menu planing (6.56), sanitation management (6.54), stored meal management (6.53), meal tasting (4.54). On the other hand, foodservice store public activity (3.70), profit and loss income statement drawing and financial management (4.30), external program's meal support (4.31), volunteer enlistment·arrangement·education·management (4.54), nutrition education of employee (4.87) were perceived relatively unimportant job. The job activities shown higher importance but lower performance was nutrition analysis of menu, therapeutic diet development for in-patients, and menu development suitable for taste of the elderly. Most common diseases of the aged was proved as dementia (36.6%), hypertension (27.7%), and diabetes mellitus (17.8%). The therapeutic diets frequently provided were diabetes mellitus diet, dysphagia diet, low sodium diet, high fiber diet, and high protein diet, in order. For those reasons, dietitians in the elderly health care emphasized that the information about therapeutic diet development such as diabetes mellitus diet, dysphagia diet, low sodium diet and hypertension diet must be continuously developed and provided. In nutrition counseling, essential points dietitians must understand was revealed as special knowledge about elderly diseases(42.0%), the elderly psychologic characteristics (21.0%), and so on.Based on the findings, we recommend as follows:1. From the result that dietitian's job such as menu nutritive analysis, therapeutic diet development of in-patients, and menu development of the aged showed a higher importance but lower performance, the program support for analysis of menu nutrition should be demanded and a supported system for the development of therapeutic diet and elderly diet appropriate to resident's needs should be constructed.2. Second, for improving the performance of nutrition management, the nutrition evaluation-education program should be supported3. When dietitian performs nutritional education, the elderly health care facilities' resident should be performed education to dementia, hypertension and diabetes, disease's attacking is highly expressed.4. When dietitian performs individual nutrition consult, general special knowledge to the elderly is demanded. Therefore, the elderly special nutrition consulting program should be demanded.
