HSV1-tk 보고 유전자가 이입되어 있는 세포주인 MCA-tk와 이입되지 않은 MCA 세포주를 이용하여 in vitro 상에서의 [18F]FHBG의 섭취 및 방출 실험을 실시하였으며 섭취량과 발현량의 상관성 평가를 위해 세포수 백분율에 따른 섭취실험을 실시하였고 세포내로의 유입기전을 알아보기 위해 transporter 경쟁 및 억제 실험을 실시하였다. In vivo 상에서의 평가를 위하여 피하 종양 형성 동물모델을 이용하여 [18F]FHBG의 생체분포 시험을 실시하였으며 micro-PET 이용하여 생체영상을 분석하였다. HSV1-tk 유전자가 이입된 MCA-tk 세포에서는 특이적인 [18F]FHBG의 집적이 발생하였으며 대조군인 MCA에서는 거의 집적이 이루어지지 않았다. 섭취 후 120분에서 두 세포주의 섭취비는 107배로 나타났다. 또한 섭취된 방사능의 방출 실험에서 방출 후 1시간 경과 까지 86% 이상의 [18F]FHBG가 세포내에 잔류하였다. MCA-tk 세포주의 백분율이 증가함에 따라 [18F]FHBG의 섭취량도 직선적 상관관계(R2=0.9954)에 따라 증가하여 기질의 섭취량이 유전자 발현량을 잘 반영하고 있음이 확인되었다. 또한 [18F]FHBG는 nucleoside transporter와 nucleobase transporter 모두를 통하여 세포내로 유입되는 것으로 확인됐으며 nucleoside transporter의 경우 equilibrative와 concentrative transporter 모두를 이용하는 것으로 생각된다. 피하 종양 동물모델의 생체분포 결과 [18F]FHBG 신장을 통해 빨리 배설 되었으며 간담관을 통해 소장으로도 빠르게 배출되어 투여 후 30분 이내에 80% 이상이 혈중에서 제거되어 빠른 혈중 제거율을 보였다. MCA-tk 종양 대 혈액 비는 투여 후 1~2시간 사이에 17에서 98로 최대 변화를 보여 [18F]FHBG의 특이적 섭취와 집적이 주로 투여 1시간 이후에 이루어짐을 확인하였다. Micro-PET을 이용한 생체영상에서 일반적인 종양에 특이적으로 집적되는 [18F]FDG를 사용한 경우 대조군인 MCA과 보고 유전자가 이입된 MCA-tk 종양에의 집적이 차이를 보이지 않은 것에 비해 [18F]FHBG를 기질로 사용한 경우 확연한 차이를 보여 주었다. 이상의 결과로서 유전자 치료법 적용 시 그 치료 효과의 평가와 예측을 위해 꼭 필요한 과정으로서 생체에 전달한 치료 유전자의 발현 정도와 지속성 그리고 위치를 확인하기 위해 HSV1-tk 보고 유전자 시스템을 이용하는 경우 비침습적 PET 영상을 위한 방사성동위원소 표지 기질로서 [18F]FHBG가 매우 유용할 것으로 생각된다.

Gene therapy would be the most prospective method for cancer therapy in the near future. And the procedure which determine the location and magnitude of expression of transduced gene is essential for evaluation of gene therapy. For this reason, various reporter gene and probe systems have been developed. Among these systems, HSV1-tk reporter gene system is the most widely using system because of its advantage that direct monitoring would be possible without the introduction of a separate reporter gene in case of HSV1-tk suicide gene therapy. In the present study, we investigated the usefulness of 9-(4-[18F]fl-uoro-3-hydroxymethylbutyl)-guanine([18F]FHBG) as reporter probe for a non-invasive reporter gene imaging using PET. With the MCA and MCA-tk cell line which was transduced with HSV1-tk reporter gene, In vitro uptake and release-out study of [18F]FHBG on the cells was performed. And to estimate correlation between [18F]FHBG uptake ratio and degree of gene expression, comparison study was performed according to increasing MCA-tk cells. Competitor and inhibitor assay was performed to elucidate the transport mechanism of [18F]FHBG across the cell membrane. To evaluate in vivo behavior of [18F]FHBG, biodistribution data and micro-PET scan images were obtained with MCA and MCA-tk tumor bearing BALB/c-nude mice model. Specific accumulation of [18F]FHBG was observed in HSV1-tk gene transduced MCA-tk cells but not in MCA cells. The accumulation in MCA-tk cells was 107 times higher than in MCA cells after 2 hr incubation with [18F]FHBG, and the above 86% of uptaked [18F]FHBG in cells was retained at consecutive 1 hr release-out experiments. The uptake of [18F]FHBG was linearly correlated (R2=0.995) with increasing percentage of MCA-tk numeric cell count. According to transporter assay results, [18F]FHBG was transported through both nucleoside and nucleobase transporter, and in case of nucleoside transporter, it was confirmed to transport [18F]FHBG via equilibrative and concentrative transporter. Biodistribution results showed that [18F]FHBG was mainly metabolized in liver and excreted to intestine, and 80% of injected radioactivity of [18F]FHBG was cleared from blood stream within 30 minute. The maximum uptake ratio of MCA-tk tumor to blood was observed during 1 to 2 hour after injection of [18F]FHBG, and it means that [18F]FHBG was specifically localized to HSV1-tk gene expressing tumor after 1 hour injection. In micro-PET scan with [18F]FDG, known to general tumor specific metabolic probe, the significant difference between MCA and MCA-tk tumor was not observed, but in case of [18F]FHBG, remarkable difference of accumulation was observed. In conclusion, [18F]FHBG was expected as a very useful non-invasive PET imaging substrate for HSV1-tk reporter gene system and there shoud be another consideration for inhibition of [18F]FHBG uptake by GCV, in the case of GCV treatment.
