The therapeutic art activity is one of milieu therapies that promotes social adaptation, prevents social withdrawal, and improves interpersonal relationships. It also gives an understanding of patient's inner world and becomes a way of facilitating self-growth through self expression and sublimation. Such an important therapeutic art activity has been carried out these days, yet in much of cases, activities are neither well organized nor have clear objectives. In this study, the effects of therapeutic art activity to patients was investigated through analyzing patients who have been in practice of therapeutic art activity of psychiatric ward. To plan an effective art activity scheme for nursing intervention, qualitative research method has been employed. Upon agreement, 29 psychiatric inpatients at E university hospital in D city participated. The medical diagnosis of subjects were schizophrenia 52%, mood disorder 21%, substance-related disorder 17%, anxiety disorder 7%, and 3% of conduct disorder. The data were collected from May to August in 2003 by participating observations, field notes, interview and medical records. During the art activity, patients were told to draw whatever they wanted. For data analysis, field note description carried by open-coded method was categorized by content analysis. The major subjects for drawings were trees, mountains, flowers, and sea. They were mostly dependent on memories of nature and family. Married women were apt to express the longing for their children and unmarried men for guilt feeling towards their mothers. Such family backgrounds revealed from the drawings were useful source in understanding the patient. The motivations of patients for participating in art activity varied. In most cases, patients experienced achievements from their creations and positive aptitudes within themselves. Also, another patients were motivated by their nurses with trustful relationship. From the perspective of emotional expression, participating in therapeutic art activity give patients feeling of safety like piece in mind, comfort, that made patients feel better, relaxed and got rid of conflicts. On the other hand, there were patients who experienced depression and hopelessness through drawings. For patients who showed strong denial in the therapeutic art activity, because they had limited self-expression, it has been good chance for them to express their thoughts through the activity. However, there is a need for providing patients with more suitable art activity. Furthermore, through an art activity, patients can get along with other peers so they can appreciate each other's drawings and learn understanding and sympathy as long as they can have closer relationship with nurses. The therapeutic art activity also promotes patient's communication skills and interpersonal ability. This shows that when planning therapeutic art activity for patients, a time for social interaction and feedback should be considered as well as actual drawing time. In conculsion, in order to adapt therapeutic art activity for patients effectively, the art activity should be considered as a field to understand patient in-depth, not just as a leisure time. So, to give attention to patient's small expressions and let the art activity be a way of self expression systematic nursing planning and management is needed. Of course, nursing practice guidelines and training are required.
