배경 : 알츠하이머병 환자에서 베타 아밀로이드가 뇌척수액으로 배출이 안되고 뇌에 축적되어 신경독성 효과를 지니며, 타우 단백은 과인산화되어 신경섬유농축제를 형성하고, 이는 알츠하이머병에서 핵심 병리 과정에 해당한다. 아세틸콜린 에스테라제는 베타 아밀로이드 단량체를 소중합체로 변형시키는 작용을 하여, 뇌에서 베타 아밀로이드의 응집을 증가시켜, 독성 작용을 증가시킨다. 또한, 베타 아밀로이드가 증가할수록 뇌에 정상적으로 존재하는 미세관 관련 단백질인 타우 단백을 과인산화시켜 뇌의 세포골격을 망가뜨린다. 따라서 아세틸콜린 에스테라제 저해제가 이런 베타 아밀로이드 1-42분획과 타우 단백의 독성을 억제하는 효과가 있음이 알려져 왔다. 이 연구는 알츠하이머병 환자와 대조군의 뇌척수액 베타 아밀로이드 1-42분획 및 총 타우 단백을 측정함으로써 알츠하이머병 환자의 조기 진단 생물학적 표지자의 가능성을 평가하고, 알츠하이머병 환자 중 아세틸콜린 에스테라제 저해제 복용 군과 복용하지 않은 군간의 뇌척수액 베타 아밀로이드 1-42분획 및 총 타우 단백을 측정함으로써 아세틸콜린 에스테라제 저해제가 이들 생물학적 표지자에 미치는 영향을 알아보고자 한다. 방법 : 알츠하이머병 환자 16명, 정상 대조군 14명을 대상으로 하였으며, 알츠 하이머병 환자 중 아세틸콜린 에스테라제 저해제 복용 군 9명과, 복용하지 않은 군 7명으로 나누어 뇌척수액 검사를 시행하였다. 뇌척수액에서 베타 아밀로이드와 타우 단백을 효소면역측정법을 이용하여 측정하였다. 모든 알츠하이머병 환자들은 간이신경상태검사와 임상치매평가척도를 포함한 신경심리검사를 시행하였다. 결과 : 본 실험에서는 뇌척수액에서 측정한 베타 아밀로이드 1-42분획 농도가 각각 알츠하이머병 환자군에서391.68±241.96pg/ml, 정상 대조군에서 827.55±263.66pg/ml으로 알츠하이머병 환자에서 의미있게 감소되어 있었다. (p<0.01) 뇌척수액 총 타우 단백의 농도는 알츠하이머병 환자군에서283.72±6.96pg/ml, 정상 대조군에서 277.82±6.90pg/ml로 별다른 차이를보이지 않았다. 또한, 알츠하이머병 환자에서 아세틸콜린 에스테라제 저해제 복용 군이 그렇지 않은 군에 비해 뇌척수 액 베타 아밀로이드 1-42분획 농도가 증가되어 있었다. 결론 : 본 실험은 알츠하이머병 환자에서 뇌척수액 베타 아밀로이드 1-42분획 농도가 감소 되어 있음을 발견하였으며, 아세틸콜린 에스테라제 저해제 복용군에서 그렇지 않은 군에 비해 뇌척수액 베타 아밀로이드 농도가 증가되어 있는 것으로 보아 향후 질환경과를 변경할 수 있는 약물개발에 있어서 그 약물에 대한 효과를 임상 증상이 아닌 생물학적 대리표지자를 통해 예견할 수 있을 것이다. 그러나, 본 연구는 대상군의 수가 적은 한계점이 있으며, 이를 극복하기 위해서 추가적인 연구가 필요할 것이다.

Background: Alzheimer’s disease (AD) is characterized by pathological results of amyloid plaques and tau-associated neurofibrillary tangles. Acetylcholinesterase(AChE) transforms β-amyloid monomer into β-amyloid oligomer, and increases β-amyloid aggregation in brain. Therefore, it increases a toxic effect in brain. Increased β-amyloid breaks cytoskeleton of brain by hyperphosphorylation of tau protein, microtubule-associated protein. Previous studies supports that AChE inhibitor has inhibitory effect on toxicity of β-amyloid and phophorylated tau protein. The purpose of this study is to investigate the effect of acetylcholine esterase inhibitor on CSF-Aβ42 and tau protein in Alzheimer’s disease. Methods: Subjects were 16 AD patients, and 14 normal control groups. The nine patients of AD had taken AChE inhibitor, the rests of AD hadn’t. The CSF-Aβ42 and CSF-tau were measured by ELISA. All patients with AD carried out the neuropsychologic test(SNSB), including the MMSE and CDR. Results: We found decreased CSF-Aβ42 levels in probable and possible AD. The levels of CSF-Aβ42 were 391.68±241.96pg/ml in AD patients, and 827.55±263.66pg/ml in control groups. The CSF-Aβ42 levels were significantly lower in AD patients.(P<0.01) The levels of CSF-tau were 283.72±6.96pg/ml in AD patients, and 277.82±6.90pg/ml in control groups. We also found increased CSF-Aβ42 levels in AChE inhibitor group. Conclusions: The level of CSF-Aβ42 may have a role in the clinical workup of patients with cognitive impairment. Also, we may expect the effect of AChE inhibitor by measuring CSF-Aβ42 levels. Therefore, we may use the level of CSF- Aβ42 to exploit disease modifying drugs, and it may be possibility as biological surrogate marker of AD treatment. But, the limitation of our study is a small number of patients. So that, further studies are necessary in overcome of the limitation.
