본 연구는 Newman(1994)의 의식확장으로서의 건강 이론에서 제시하는 실무연구방법론을 근거로 우울 청소년들의 건강경험을 탐색하는 질적연구이다. 연구 참여자들은 14~19세의 청소년 6명으로 학교적응 및 대인관계 갈등 때문에 상담을 받고자 희망하여 본인이 직접 N정신보건센터로 찾아왔거나 혹은 학교 내 상담교사나 담임교사를 통하여 의뢰되었다. 이들은 BDI검사결과 1명은 경증우울을 5명은 중증우울을 경험하고 있는 것으로 나타났다. 본 연구에서는 실무연구방법론을 위해 Newman(1994)이 제시한 프로토콜에 따라 면담-옮겨 쓰기-이야기의 전개-그림그리기-후속면담-이론의 적용을 사용하였다. 본 연구의 자료수집 및 분석을 위하여 Newman(1994, 2000)의 실무연구방법론에서 제시하는 해석학적-변증법적 과정으로 패턴분석방법을 활용하였다. 자료수집 기간은 2007년 3월부터 10월까지였으며, 각 참여자별로 6~8회의 심층면담을 진행 하였고, 면담은 40~90분이었으며 평균 60분간 소요되었다. 모든 면담내용은 녹음한 후 필사하여 요약한 후 순환적 이해를 위해 참여자와 함께 확인하였다. 본 연구 탐색결과를 의식확장으로서의 건강 이론에 따라 제시한 결과 참여자들의 건강경험은 다음의 순차적 세 가지 패턴으로 나타났다. 이들 세 가지 건강경험을 구체적으로 제시하면 다음과 같다. 첫째, 참여자들이 우울로 인해 의식확장의 구속단계에서 겪었던 초기 건강경험은 1) 따듯한 둥지에 대한 그리움 2) 자신의 쓸모없음을 한탄함 3) 죽음을 탈출구로 생각함 4) 사람들과 벽 쌓기 5) 감정의 소용돌이를 경험함 6) 분노감에 의해 어긋나게 행동함으로 나타났다. 둘째, 참여자들이 초기 건강경험 패턴에 대한 인식과정과 참여자-연구자 상호작용 속에서 자신과 자신의 상황에 대한 통찰력을 얻고 새로운 행동의 가능성을 얻은 전환점 경험은 1) 현실을 이해하고 변화가능성을 발견함 2) 자기 개방의 필요성을 인식함 3) ...

Guided by Newman’s theory of health as expanding consciousness, this study explored the health experience of depressive adolescents. The researcher engaged in six and eight in-depth interviews with six adolescents. To begin the dialogue, the researcher asked each participant to tell the first important memory he/she had. All the narratives and diagrams sharing between the researcher and participants were summarized according to recognized patterns and later elaborated in the following interviews based on Newman's praxis methodology. The results are summarized as follows: 1. The significant individual patterns of early health experience during the binding stage were as follows: 1) yearning toward comfortable nest 2) grieving for self-valuelessness 3) thinking suicide as a way of escape 4) walling from significant others 5) swirling of sentiment 6) expression of anger using inappropriate behaviors. 2. At the turning point, individual patterns for participants revealed a personal journey of self-discovery and then emergence of reflecting behaviors. The participants experienced 1) understanding reality and finding possibility of change, 2) recognizing the need for self-disclosure, and 3) rediscovering the meaning of one's life. 3. After the turning point, the participants changed as they evolved from the initial period of disruption and disorganization to organization at the higher level. Their transformed experience were as follows: 1) expressing the feeling of self-worth and one's emotion 2) increasing interpersonal relationships 3) managing one's anger 4) participating in one's life with passion 5) changing behaviors in the hope of the future. 4. During the participant-researcher relationship process, the researcher's transformed experiences were as follows: 1) realizing the importance of the experience of caring others 2) self-confidence in helping the depressive adolescents 3) experiencing the essence of health by exploring the health pattern, and 5) experiencing and discovering the essence of nursing. The results suggest that depressive adolescents found new ways of relating to friends, family, healthcare providers, and the community by expanding their consciousness. Newman's praxis methodology is a good way of helping and studying depressive adolescents because it emphasizes participant-nurse/researcher partnership and pattern recognition as nursing practice.
