End stage renal disease(ESRD) is not a curable disease, and requires renal replacement treatment options, such as hemodialysis or peritoneal dialysis. Exercise capacity in hemodialysis patients is low compared with age-matched healthy persons. Hemodialysis patients suffer from a constellation of symptoms characterized by the uremic syndrome. Common ramifications of the uremic syndrome include reduced physical work capacity, decreased health-related quality of life, and cardiovascular disease. Exercise training in most populations results in increased functional capacity, reduced cardiovascular risk, and improved psychological functioning. Similar benefits are reported in patients on dialysis. The purpose of this study is to investigate the effects of an intradialytic exercise on the physiological and psychological responses in hemodialysis patients. The study included 21 patients (control group 11, experimental group 10) on maintenance hemodialysis at 2 university hospitals between July, 2007 and November, 2007. There was no difference in age, sex, duration of hemodialysis, Kt/V, blood pressure, intradialytic weight gain, and psychological well being status between control and experimental groups. Experimental group performed intradialytic exercise 20-30 minutes per session, 2-3 times a week for 10weeks. The collected data were analyzed using pc-SAS program; Fisher's exact-test, Wilcoxon 2-sample test, t-test, paired t-test, Wilcoxon rank sum test, and repeated measures ANOVA were utilized. The results of this research are as follows: 1. “ The Kt/V of the patients who are provided with intradialytic exercise may be higher than that of the patients who are not provided with intradialytic exercise", did not exhibit a statistically significant difference between two groups(t=-0.55, p=0.0804). 2. “ The MAP variation of the patients who are provided with intradialytic exercise may be lower than that of the patients who are not provided with intradialytic exercise", did not exhibit a statistically significant difference between two groups(F=2.66, p=0.1193). 3. “ The rate of increase in intradialytic weight gain(IDWG) of the patients who are provided with intradialytic exercise may be lower than that of the patients who are not provided with intradialytic exercise", did not exhibit a statistically significant difference between two groups(F=0.06, p=0.8164). 4. “ The psychological well being scores of the patients who are provided with intradialytic exercise may be higher than that of the patients who are not provided with intradialytic exercise", exhibited a statistically significant difference between two groups(t= 1.39, p=0.0483). In conclusion, the intradialytic exercise used in this research was confirmed to be effective nursing intervention for hemodialysis patients. Although physiological responses did not exhibit statistically significant differences between two groups, intradialytic exercise may provide a more convenient and time-efficient model for hemodialysis patients.
