Objectives : The purpose of this study was to find out the status of extracorporeal shock wave lithotripsy (ESWL) of urinary stones and especially to analyze expected factors that influence ESWL outcome of ureteral stones in the clinic. In addition, the study was designed to provide information for an appropriate choice to patients for medical treatment.
Materials and Methods : In this study, health insurance data for patients with urinary stones who were treated with ESWL from January to June 2007 was evaluated. To find out ESWL frequency differences based on the characteristics of patients and treatment providers, the chi-squared test was performed. To confirm factors that influenced ESWL outcome for ureteral stones, logistic regression was performed to identify independent variables that influenced ESWL frequency.
Results : (1) The overall frequency of ESWL procedures for the treatment of urinary stones was increased in older patients or patients with comorbidity conditions, including kidney stones, more than two urinary stones or hydronephrosis. For the characteristics of the treatment providers, a large number of ESWL procedures for urinary stones were performed at comprehensive specialist hospitals with more 800 beds, and medical care providers located at Gwangju, Daejeon. (2) The success rate for ESWL for ureteral stones tended to decrease when the age of the patients was higher, when ESWL was recently introduced in the clinic and for a recently established clinic. The age of the physician and the location of the medical care provider also showed statistically significant differences. Patient age and location of the medical care provider are the main factors that affected the outcome of ESWL procedures.
Conclusion : To choose a method of ureteral stones treatment, patient age and location of the medical care provider should be considered. However, there is a need for an additional study to determine a more precise correlation.
