목적: 한국의 알츠하이머병(Alzheimer disease, AD) 환자에서 병전 성격 및 특정 인지기능과 치매의 행동 심리 증상(Behavioral and psychological symptoms in dementia, BPSD)의 관련성에 대해서 단면 연구를 시행하였다.
방법: 알츠하이머병 환자 103명을 대상으로, 병의 경과 중 치매의 행동 심리 증상의 여부를 알아 보기 위해 돌보는 사람이 점수를 매기는 한국형 신경정신행동 검사(Neuropsychiatric Inventory, NPI)와 병전 성격을 알아 보기 위해 정보 제공자가 평가하는 후향적 성격 평가인 NEO-Five Factor Inventory(NEO-FFI) 질문지를 사용하였다. 인지기능의 문제는 한국판 간이 정신상태 검사 (Korean version of Mini-Mental State Examination, K-MMSE), 한국판 보스턴 이름대기 검사 (Korean-Boston Naming Test, K-BNT), 언어 기억 검사(Seoul Verbal Learning Test, SVLT), 통제 단어 연상 검사(Controlled Oral Word Association Test, COWAT)로 평가하였다.
결과: 병전 신경성(neuroticism)은 망상, 초조, 불안, 탈억제, 전체 NPI점수(p<0.05), 수면장해(p<0.01)와 유의한 상관관계가 있었고, 낮은 병전 성실성(conscientiousness)은 환청, 수면장해(p<0.05)와 유의한 상관관계가 있었다. 하지만 병전 신경성과 성실성은 BPSD의 요인인 “정신증, 과활동성”이나 “정서, 무감동, 전두엽”을 독립적으로 예측하지 못하였다. 환자의 기억과 집행 기능의 수행 정도(각각p<0.05, p<0.01)는 전체 NPI 점수(빈도, 심각도, 합성점수)와 유의한 상관관계가 있었지만 인구학적, 임상적 특성들을 통제하기 전, 후로 전체 NPI점수를 예측하지 못했다.
결론: 병전 성격과 기억, 집행 기능을 포함한 특정 인지 영역은 NPI 점수와 유의한 상관관계가 있었다. 그러나 병전 성격과 특정 인지 기능의 문제는 인구학적, 임상적 변수들과 독립적이든 독립적이지 않든 BPSD의 가능한 예측인자는 아니었다.

Objectives: We performed a cross-sectional study to examine the relationship between premorbid personality, specific cognitive domains, and behavioral and psychological symptoms in dementia (BPSD) in Korean patients with Alzheimer’s disease (AD).
Methods: We assessed 103 patients diagnosed with AD for the presence of BPSD over the disease course by using the caregiver-rated Neuropsychiatric Inventory (NPI) and for the presence of a premorbid personality by using a retrospective version of the NEO-Five Factor Inventory questionnaire completed by informants. Cognitive deficits were evaluated using the Korean version of the Mini-Mental State Examination, the Korean-Boston Naming Test, the Seoul Verbal Learning Test, and the Controlled Oral Word Association Test.
Results: Premorbid neuroticism was significantly correlated with delusion, agitation, anxiety, disinhibition, total NPI score (p < 0.05), and sleep disturbances (p < 0.01). Lower premorbid conscientiousness was significantly correlated with symptoms of hallucinations and sleep disturbances in AD patients (p < 0.05). However, premorbid neuroticism and low premorbid conscientiousness did not act as independent predictors for “psychosis, hyperactivity,” or “moods, apathy, frontal” factors of BPSD.
Performance of the patients in memory and executive function (p < 0.05 and p < 0.01, respectively) was significantly correlated with total NPI (frequency, severity, and composite) scores. However, their performance did not estimate total NPI scores, before and after controlling for demographic and clinical characteristics.
Conclusions: Premorbid personality and specific cognitive domains, including memory and executive function, were associated with the NPI score. However, premorbid personality and specific cognitive deficits were not observed to be potential predictors of BPSD, before and after accounting for demographic and clinical variables.
