We performed a follow-up study whether high sensitivity C-reactive protein (hs-CRP) level within the normal range and the interval changes of hs-CRP can be used as a predictor of the development of metabolic abnormalities in metabolically healthy subjects. A cohort study was performed on 3,748 Korean male workers, aged 30-59 years, with normal range of hs-CRP level and no evidence of the any component of metabolic syndrome. At each visit, hs-CRP level and metabolic abnormalities were measured. The interval change of hs-CRP for each subject was calculated from the difference between baseline and subsequent measurements. COX proportional hazard model and logistic regression analysis were used to evaluate the relationships of hs-CRP with incident metabolic abnormalities. Hazard ratios of metabolic syndrome and HDL-C abnormality incidence were increased by hs-CRP categories within normal range in non-adjusted model. After adjusting for age, smoking, alcohol intake and exercise, these incidences were increased by hs-CRP categories but statistically significance was not shown. Higher hs-CRP within normal range predicted incidences of BMI and triglyceride abnormalities in dose response manner. Incidence of BMI and triglyceride abnormalities of the highest hs-CRP group was significantly higher than the lowest one. Metabolic syndrome, BMI abnormality and hypertriglyceridemia incidences were significantly higher in the increased group, as to subjects that hs-CRP level was increased at the second visit compare to first visit than the decreased or unchanged group. Higher hs-CRP group in the healthy cohort, even if hs-CRP level is within the normal range, predict increased metabolic abnormalities incidence compare to the lower hs-CRP group. And the group that hs-CRP level was increased during the follow-up period had more metabolic risk than the other group. Therefore, appropriate follow-up and management for individuals even in the normal range of hs-CRP will be needed.
