골격성숙도는 Potable X-ray(Corus: Y. C, M Growth Well Co., Korea)를 이용하였으며, TW3방법을 이용하여 골격성숙도를 산출하였다.

The purpose of this study was to examine the bone mineral density, blood lipids, arterial compliance, and physical fitness in obese male adolescents. Twenty male adolescents volunteered to participate in the study. They were divided into 10 obese adolescents (over 30% of percent body fat measured by DXA) and 10 normal adolescents (under 25% of percent body fat measured by DXA). Body height and weight were measured. TW3 methods were applied to analyse the skeletal maturity. Body composition and four site of bone mineral density (whole body, femur, lumbar, forearm) were estimated by using DXA (Hologic, QDR-4500, USA). All subjects were instructed to complete 12 hours fasting and were limited intense physical activity for the 24 hours before the blood sample collected. Blood lipids including triglycerids (TG), total cholesterol (TC), high density lipoprotein-cholesterol (HDL-C), low density lipoprotein-cholesterol (LDL-C) and free fatty acids (FFA) were analysed. TG, TC, HDL-C, and LDL-C were analyzed using colorimetric assay (AU680, Beckman Coulter, USA), and FFA was determined using enzymatic assay (Hitachi, Roche, Japan). Arterial compliance was measured with non-invasive Arterial screening method (VP-1000, Colin Co. Ltd., Komaki, Japan). Subjects were examined after a 30-minute rest, double cuffs were wrapped on each brachial and ankle arteries in the supine position. Systolic blood pressure (SBP) and diastolic blood pressure (DBP) on ankles and brachial arteries of both sites were included. Brachial-ankle pulse wave velocity (baPWV) and ankle-brachial pressure index (ABI) were estimated. baPWV was calculated by the distance between two different arteries divided by the pulse wave transmit time (PTT) according to body height and ABI was automatically calculated by ankle SBP divided by brachial SBP. Health-related fitness including flexibility (Sit & reach), muscle strength (Grip strength and Standing long jump) muscular endurance (Sit-up), and cardiorespiratory fitness (maximal oxygen uptake: VO2max) were measured. All subjects were performed after 10 minutes warm up before the test began. All data were analyzed utilizing SAS 9.2 (SAS Institute, NC, USA). Independent t-test was used to evaluate the differences between obese adolescents and normal adolescents. Pearson correlation analysis was applied to figure out the relationship between bone mineral density and health-related fitness. Multiple regression analysis were applied to figure out the factors of physical fitness which could influence on bone mineral density. A level of significance was set at .05. The results of the study indicated that fat tissue (p<.001), and percent body fat (p<.001) were significantly higher in obese adolescents than normal adolescents. However lean tissue was not significantly difference between obese and normal adolescents. Whole body BMD was significantly lower in obese adolescents than normal adolescents (p<.05). However femur, lumbar, and forearm BMD were not significantly difference between obese and normal adolescents. In relation to blood lipids, obese adolescents were significantly higher on TG (p<.05) and TC (p<.05) than normal adolescents. However HDL-C (p<.05) was significantly lower in obese adolescents than normal adolescents. Light brachial systolic blood pressure (LbSBP) was significantly higher in obese adolescents than normal adolescents (p<.05). However brPWV and ABI were not significantly difference between obese and normal adolescents. Health-related fitness including standing long jump (p<.001) and sit up (p<.001), VO2max (p<.001), duration (p<.001), and grip strength (p<.05) were significantly higher in normal adolescents than obese adolescents. Grip strength was significantly positively related with whole body BMD (p<.001), femur BMD (p<.001), lumbar BMD (p<.01), forearm BMD (p<.01). Standing long jump was positively related with femur BMD (p<.05). Sit up and VO2max were significantly posively related with whole body BMD (p<.05). Sit up and grip strength were predictors for whole body BMD in explaining 65% and 75% in obese adolescents. Sit up was a predictor for lumbar BMD in explaining 67%. Grip strength and standing long jump were predictors for femur BMD in explaining 73%, 82% in obese adolescents. Sit up and grip strength were predictors for forearm BMD in explaining 61%, 72% in obese adolescents. In conclusion, obese male adolescents have higher fat tissue and percent body fat than normal adolescents. Blood lipids including TG and LDL-C were higher in obese adolescents than normal adolescents. HDL-C was lower in obese adolescents than normal adolescents. Sit and reach, sit up, standing long jump, VO2max and duration were higher in normal adolescents than normal adolescents. Grip strength was positively related with whole body, femur, lumbar, and forearm BMD in both obese and normal adolescents. Muscluar related fitness including sit up, grip strength were predictors for explaining BMD in obese adolescents. Future study in obese adolescents should be applied by exercise or nutritional prescription. And the changes of arterial compliance in obese person should be examined during life span.
