Background: Percutaneous radiofrequency ablation (RFA) has known a useful and safe procedure for treatment of hepatic neoplasm. However, RFA of liver causes severe pain, which increased the demand of monitored anesthetic care (MAC). We designed to compare the efficacy and safety of propofol and dexmedetomidine, which were commonly used in sedation, during the RFA of hepatic neoplasm under MAC. Methods: In this randomized controlled trial, forty patients were randomly allocated into two groups for elective RFA. Patients were received with either dexmedetomidine (group D) or propofol (group P). For pain control, both groups received with continuous infusion of remifentanil. The primary outcomes were opioid consumption and a difference of arterial carbon dioxide (PaCO2) level between pre- and immediate end of RFA. In addition, hemodynamic parameters, patients’ satisfaction scale, and interventional radiologists’ satisfaction were investigated. Results: There were significant differences in opioid consumption [50.1 ± 16.8 (D) and 71.2 ± 18.7 (P) ng/kg/min, respectively; p= 0.001] and PaCO2 level [10.4 ± 6.4 (D) and 17.2 ± 9.2 (P) mmHg, respectively; p= 0.016] between two groups. Moreover, respiratory rates were significant different between both groups during intra-procedure of RFA (p<0.001). However, blood pressure and heart rates were not changed significantly during the RFA procedure. Both patients’ and interventionists’ satisfaction scale were not different in both groups. Conclusions: Dexmedetomidine provided respiratory stability and reduced opioid consumption than propofol during RFA of hepatic neoplasm under MAC.
