본 연구는 항암화학요법을 받는 암환자의 감염발생에 영향을 주는 요인을 파악하고 모형의 예측력을 비교하고자 로지스틱회귀 분석과 의사결정 나무 분석을 이용한 서술적 조사연구이다. 자료 수집은 2011년 3월부터 2013년 2월까지이었으며, 연구대상은 서울시 소재의 일개 대학부속병원에 내원한 항암화학요법을 받는 암환자 732명이었다. 자료 분석은 SPSS 18.0 프로그램을 이용하여 인구 사회학적 특성, 환경적 특성, 임상적 특성으로 구분하여 빈도, 백분율, 평균과 표준 편차 및 -test, 로지스틱 회귀분석, 의사결정나무로 분석하였다. 본 연구 결과는 로지스틱 회귀분석에서 항암화학요법 시 투여장소, 항암화학요법 시 재원병동, 항암제의 종류 중 항암제1, 항암제7, 항암제8, 항암제9, 항암제10, 기저질환이 유의미한 변수임을 확인하였고 의사결정나무 분석에서 항암제 종류 중 항암제1, 항암제3, 항암제7, 기저질환이 공통적으로 감염발생 예측 요인으로 제시되었다. 로지스틱 회귀 분석에서 특이도 88.9%, 민감도 66.7%로 나타났으며 감염발생 예측에 대한 의사결정나무 분석에서 특이도 89.0%, 민감도 55%로 나타났다. 전체 분류 정확도는 로지스틱 회귀모형 87.9% 의사결정 나무 모형 87.2% 이었다. 항암화학요법을 받은 암환자의 감염발생에 영향을 주는 요인 분석에서 민감도와 분류정확도가 높은 로지스틱 회귀분석 방법이 예측모형을 설명하기에 더 적절하다고 생각되며 감염예방을 위한 간호중재 개발에 근거자료로 사용될 수 있으리라 사료된다.

The purpose of this study is to identify risk factors of developing infections with patients in chemotherapy and compare the predictors of each models through a descriptive research using logistic regression & decision tree. The survey data had been collected from 732 patients in chemotherapy who visited general hospitals in Seoul from March 2011 to February 2013. The data were classified into demographic characteristics, environmental characteristics, clinical characteristics by SPSS 18.0 program and analyzed by frequency, percentage, mean and standard deviation, χ²-test. logistic regression and decision-making tree. These results represent that injection place, administration ward, chemotherapy 1, 7, 8, 9, 10, and underlying diseases are meaningful variables in logistic regression analysis while chemotherapy 1, 3, 7 and underlying diseases are shown as common predictors of infection according to decision making tree. 88.9% specificity and 66.7% sensitivity are shown in logistic regression analysis and 89% specificity and 55% sensitivity are presented in decision making tree. The overall classification accuracy represents 87.9% in logistic regression model and 87.2% in decision making tree model. In conclusion, logistic regression analysis is considered to be more effective to identify risk factors of developing infections with patients in chemotherapy by showing high specificity and classification accuracy. Moreover it is expected to be basic materials for developing nursing interventions of the prevention of infection.
