This paper is to analyze impacts and problems of the new KDRG-based payment system during its first year and suggest possible solutions. The new KDRG-based payment system, so called 7-diseases DRG, has implemented and which prospectively reimburse medical fee for seven diseases since July 2012. It was expanded to all types of hospitals on July 2013, with objectives of controlling problems of Fee-for-service payment system as rapid growth medical care budget and patient cost. The Korean government, the Ministry of Health and Welfare, suggested the intended effects of DRG – to reduce documentation requirements for hospital reimbursement and control public insurance budget and help patients’ burden.
However, it has burdened hospitals to adjust new payment system and the development of DRG classification is likely to increase documentation costs of hospitals as much as Fee-for-service system. By raising the price of DRG-relate cares, it was expected to enhance quality of care at the planning stage. However, management of hospitals restrain medical services for DRG-related diseases and encouraged medical staffs to cut costs with incentives whereas government expected increased medical care as much as raised price. It remains vulnerable to excessive cost reduction of hospitals to increase profit and it may negatively affect patient outcome.
In order to solve problems of current KDRG-based payment system, it is required to control the number of DRG classifications to decrease administration procedure, expand the prospective payment system for the most diseases, implement extra payment systems which may control the growth in total medical service expenditures, reinforce the monitoring system to ensure quality of medical care and inaugurate arbitrators independent to the Ministry of Health and Welfare.
Since this study was designed to only review advanced researches and interviews due to lack of measurable data, it is recommend to start quantitative study as soon as publication of government official reports about the new KDRG.
