Patients with PD are likely to have uncertainty regarding illness management due to the fact that not only do dopaminergic medications cause unexpected side effects such as, inconsistent efficacy period, dystonia, and dyskinesia, but there is also no complete cure for PD. Like the cases of other refractory chronic diseases, depression, which is the most prevalent non motor symptom in PD, can function as a predictor of low medication adherence. Purposes: This descriptive survey study was performed to identify the degree of uncertainty, as well as factors influencing depression in PD patients. Methods: The subjects in this study were patients visiting the neurology outpatient department of a tertiary hospital in Seoul and events managed by the Korean Parkinson Disease Association from March 2015 to May 2015. A total of 120 subjects agreed to participate in this study. Data includes general characteristics, disease related characteristics, uncertainty, and depression. Disease related characteristics were measured using Modified Morisky Scale(MMS) for medication adherences, Parkinson’s Disease Sleep Scale(PDSS) for quality of sleep, Parkinson Fatigue Scale(PFS-16) for fatigue, Schwab and England Activities of Daily Living Score(SEADL) for activities of daily living, and Instrumental Activities of Daily Living Scale(IADL) for instrumental activities of daily living. Uncertainty and depression were measured with Mishel Uncertainty in Illness Scale(MUIS) and Short Form Geriatric Depression Scale(SFGDS), respectively. Data were analyzed using descriptive statistics, t-test, ANOVA, Scheffe test, Pearson’s correlation and multiple linear regression analysis by SPSS 20.0 program. Results: The mean age of the subjects was 65.01(±9.08) and 50.8% of them were male. The mean scores of uncertainty and depression were 99.03(±13.04) and 6.73(±3.91), respectively. Among total subjects, 67.5% of them represented point higher than five, the cut-off score in depression. The differences of depression score by medication adherence(motivation), quality of sleep, fatigue, activities of daily living, instrumental activities of daily living, and perceived health status were statistically significant. Also, uncertainty and depression were positively correlated. Multiple linear regression showed that uncertainty(β=.326, p<.001), perceived health status(β=-.233, p=.004), and fatigue(β=.218, p=.009) were predictive factors for depression in patients with PD. Conclusions: Depression in patients with PD was associated with uncertainty. The results of this study suggest that interventions dealing with uncertainty, perceived health status, and fatigue are needed to manage depression well in patients with PD. Health care providers including nurses may enhance medication adherence, the cardinal element for PD managements, by lowering depression of patients with PD through proper interventions stated above.
