본 연구의 목적은 서울 소재 체력측정기관에 내원하여 검진을 받은 소방공무원을 대상으로 동일한 운동량을 부과하여 각 단계별 혈압(BP), 심박수(HR), 심근산소요구량(MVO2 : myocardial oxygen consumption)을 의미하는 심부담도(RPP : rate pressure product)와 최대산소섭취량(O2max)을 측정하여 소방 공무원의 직무 스트레스가 심혈관계 질환(cardiovascular disease)의 잠재적인 위험인자로 작용하는지 알아보는데 목적이 있다. 본 연구의 대상자는 2015년 12월 1일부터 2015년 12월 31일까지 서울 소재 체력측정기관에 내원하여 검진을 받은 소방공무원 184명 중 선천성 심장질환 및 심근경색, 심근허혈 및 고혈압 등의 심혈관계 질환이 없는 사람을 대상으로 실시하였다. 또한, 당일 안정시 혈압이 140/90mmHg 이하이며 연구목적을 이해하고 참여할 것을 동의한 대상자를 실험에 참가 하도록 하였다. 최대산소섭취량(O2max), 심박수(HR), 수축기혈압(SBP), 이완기혈압(DBP)을 측정하기 위하여 점증운동부하 검사를 실시하였다. 점증운동부하검사는 85m/min의 속도에서 매 1분마다 경사도를 2.5% 씩 증가시키는 Modified Balke protocol을 사용하였다. 수축기혈압(SBP)과 심박수(HR)의 곱을 이용하여 심부담도(RPP)를 산출하였고, 호흡가스는 검사 중 지속적으로 채집하여 최대산소섭취량(O2max)을 측정하였다. 직무 스트레스(Job Stress)는 표준화 된 연구를 통하여 신뢰도와 타당도가 입증된 한국인 직무스트레스 측정도구인(KOSS : Korean Occupational Stress Scale) 7영역 24문항의 단축형을 사용 하였다(장세진 등, 2005). 본 연구에서 얻어진 모든 자료들은 SPSS/PC window 18.0 version을 이용하여 기술통계치(Mean, SD)를 산출하였고, 직무 스트레스를 상위군과 하위군으로 장세진 등, (2005)이 제시한 참고치를 기준으로 나누어 심폐지구력에 대하여 Independent - T test를 실시하였다. 또한, 직무 스트레스에 따른 운동 중 혈압(BP)과 심박수(HR), 심부담도(RPP)에 대하여 나이, 흡연, 음주, 운동사항을 공변량(covariance)을 주어 one way ANOVA repeated measure of analysis를 실시하였다. 모든 검증의 유의 수준은 p<.05로 설정하였다. 1) 직무스트레스에 따른 상위군·하위군의 심박수(HR) 변화 수치를 분석한 결과 두 집단 모두 단계별 증가가 나타났으나 집단 간 유의한 차이는 나타나지 않았다.

The purpose of this research is to find out whether the job stress of fire fighter exerts bad effects on the potential dangerous factor of cardiovascular disease. Subjects who had medical check-ups by visiting a physical fitness center in Seoul were imposed same volume of exercises in order to measure Rate Pressure Product(RPP), and maximal oxygen uptake(O2max) which accordingly indicates Blood Pressure(BP), Heart Rate(HR), and myocardial oxygen consumption(MVO2). Among 184 participants of firemen who visited the clinic from 1st December, 2015 to 31st December, 2015, on this study, subjects were selected as ones who have no cardiovascular diseases including congenital heart disease, myocardial infarction, and high blood pressure. And their resting blood pressures of the testing day were below 140/90mmHg. Operations were proceeded after they understood and agreed the purpose of this research. In order to measure O2max, HR, Systolic Blood Pressure(SBP), and Diastolic Blood Pressure(DBP), exercise tolerance test was used. It was Modified Balke protocol that used in the test which increase inclination every 1minute at the speed of 85m/min. Multiplying SBP by HR, RPP is calculated and O2max is computed by consistently collecting respiratory gas during the test. Job stress was measured by using Korean Occupational Stress Scale Shortened Form(KOSS-SF) which has been surely approved the reliability and validity through standardized researches(Sejin Jang et al, 2005). All the data from this test were computed to make descriptive statistics(Mean, SD) by applying SPSS/PC window 18.0 version. Independent-T test was processed toward cardiovascular endurance dividing groups into two of high rank group and low rank group for job stress according to the reference of KOSS-SF(Sejin Jang et al, 2005). Also, one way ANOVA repeated measure of analysis was used putting covariance which includes age, smoking, drinking, and exercising; depending BP, HR, RPP during the test according to the job stress. Significance level of all verifications is set to p<.05.
1) In the result of changes in HR of high rank group and low rank group by job stress, there were increases by stages in both groups but were no significant distinctions. 2) In the result of changes in SBP of high rank group and low rank group by job stress, there were increases by stages in both groups and was notable gap at 7th stage. But there were little gap between the groups. 3) In the result of changes in DBP of high rank group and low rank group by job stress, there were rare increases by stages in both groups and were little gap between the groups. 4) In the result of changes in RPP of high rank group and low rank group by job stress, there were increases by stages in both groups. At the stage of 2, 3, 6, and 7, there were remarkable differences and were notable intergroup distinctions.
