본 연구의 목적은 서울에 위치한 일개 상급 종합병원에 입원하여 말초 정맥관을 통하여 치료를 받고 있는 성인 환자를 대상으로 침윤과 일혈의 발생율과 위험요인을 분석한 전향적인 조사연구이다.

4.정맥주사관련 특성과 침윤 발생을 분석한 결과 24시간 이상 지속적 투약, 카테터 유지시간이 통계적으로 유의하며, 그리고 일혈 발생유무를 분석한 결과 카테터 크기, bolus 주입, 24시간이상 지속 투약, 삽입자 연차, 카테터 유지시간이 통계적으로 유의하게 (p<.05) 연관이 있다.
5.수액이나 약물의 특성과 관련하여 침윤과 일혈 발생유무를 분석한 결과 등장액, 비경구영양제, 항응고제, 3세대 세팔로스폴린계 항생제투약이 침윤 발생과 연관이 있는 것으로 나타났다. 그리고 일혈은 등장액, 비경구 영양제, 제산제, 방사선 조영제 투약이 통계적으로 유의하게 (p<.05) 연관이 있다.
6.Logistic regression analysis을 통하여 침윤과 발생위험요인 분석한 결과 발병장기, 입원목적, 등장성 수액, 비경구영양제, 3세대 세팔로스폴린계 항생제, 카테터 유지시간, BMI가 통계적으로 유의하게 (p<.05) 연관이 있다. 그리고 일혈은 비경구 영양제, 24시간 이상 지속적 투약, 카테터 유지시간이 통계적으로 유의하게(p<.05) 연관이 있다.
이상의 연구결과로 볼 때 성인 말초 정맥관과 합병증인 침윤과 일혈의 발생율을 줄이고, 이차 합병증 예방을 위한 연구가 필요하다. 그리고 간호사 교육에 있어 역량에 따른 체계적인 말초 정맥관 교육 프로그램을 개발하여, 환자들에게 정맥주사 요법을 받는 다양한 특성에 따른 적절하고, 지속적인 간호를 제공할 수 있다.

The purpose of this study was to investigate the incidence of infiltration and extravasation, and the risk factors associated with the use of peripheral intravenous catheters in adult inpatients treated in a general hospital in Seoul.
A total of 214 adult patients admitted to the department of gastrointernal or the surgical ward who received fluid infusion and drug treatment participated in this study. Data were collected from April 9 to May 31, 2016. Eleven patients dropped out before the completion of the study, and thus, data from 203 patients were analyzed. A structured case record form was used to collect the data. Additionally, a scale proposed by the Infusion Nurses Society (INS, 2006) was used to measure infiltration. Extravasation was defined as leakage of the drug with the potential to cause blisters into tissues around the skin other than the blood vessel path, as suggested by Sauerland et al(2006). To analyze the data, descriptive analysis, independent t-test, Chi-square test, and logistic regression analysis were performed using SPSS WIN 21.0.
Major study findings are as follows.
1.The occurrence rate of infiltration was 18.7%. In regards to the extent of infiltration, 16.7% of the patients scored 1 grade and 2% of the patients scored 2 grades, whereas cases of no infiltration received 3 or 4 grades. The occurrence rate of extravasation was 43.3%.
2.The analysis of the presence or absence of infiltration according to general patient characteristics showed that infiltration was significantly associated with the affected organ, the department in charge of treating the patient, reasons for admission, length of hospital stay, and BMI (p <0.05 for each).
3.The analysis of the presence or absence of extravasation according to general patient general characteristics showed that extravasation was significantly associated with the affected organ, the department in charge of treating the patient, underlying disease involving the digestive system, reason for admission, length of hospital stay, and BMI (p <0.05 for each).
4.The results showed that infiltration was significantly associated with continuous administration over 24 hours and duration of catheterization (p <0.05), while extravasation was significantly associated with catheter size, bolus injection, continuous administration over 24 hours, clinical experience of the nurse administering the infusion, and the duration of catheterization (p <0.05 for each).
5.The analysis conducted according to the drugs and fluid characteristics showed that infiltration was significantly associated with isotonic fluid, parenteral nutrition, anticoagulant, and third-generation cephalosporin antibiotic (p <0.05 for each), while extravasation was significantly associated with isotonic saline solutions, parenteral nutrition, antacid, and radiologic contrast agent (p <0.05 for each).
6.The logistic regression analysis examined risk factors of infiltration and extravasation, showed that affected organ, reason for admission, isotonic fluid, third-generation cephlalosporin antibiotics, parenteral nutrition, duration of catheterization, and BMI were significant risk factors for infiltration (p <0.05 for each), whereas, parenteral nutrition, continuous administration over 24 hours, and duration of catheterization were significant risk factors for extravasation (p <0.05 for each).
Based on the study findings, further research is necessary to reduce the incidence of infiltration and extravasation, i.e., complications of peripheral intravenous catheterization in adults, and to prevent secondary complications. Moreover, an educational program should be developed to provide systematic education on peripheral intravenous catheterization for nurses. Finally, patients should be provided with appropriate and continuous care based on the type of intravenous infusion, they receive.
