To walk independently is one of the common rehabilitation goals for patients who suffered from stroke. Ability to walk is determined by several factors such as muscle strength, endurance, balance, sensory function, cognitive function and need for an assistive walking device. These factors should considered in gait assessment as well as rehabilitation programs planning. In clinical setting, observational gait assessment (OGA) is routinely used for assessment of gait. Observational measures have advantages and limitations, and each is capable of identifying a number of gait deviations. However, relationship studies between factors that influence gait, OGA and level of gait independence are lacking. Therefore, the main purpose of this study was to investigate which relationship between factors affecting gait and level of gait independence and the correlation between OGA tools and level of gait independence. It also determined the cut-off values of each factor and OGA tools according to the functional ambulation category (FAC). A total of 210 hemiparetic patients participated in the first study. The level of gait independence was assessed using the FAC. Each participant underwent clinical examination of muscle strength, range of motion (ROM), sensation, spasticity and coordination of the paretic leg and functional assessments including Modified Rivermead mobility index (MRMI) and Berg balance scale (BBS). Stepwise multiple regression analysis showed that the clinical variables related to FAC were the muscle strength (hip flexor, knee extensor, ankle plantar flexor), coordination, and proprioception of the paretic leg. Functional variables related to FAC were MRMI and BBS. In distinguishing independence group and supervision or less level group, the cutoff value for the Medical research council (MRC) of hip flexor was 3.5 point, knee extensor was 3.5 point, ankle plantar flexor was 2.5 point, proprioception was 1 point, Lower extremity motor coordination test (LEMOCOT) was 9 point, MRMI was 36.5 points, and BBS was 42.5 points. Also, some sub-items of the MRMI (item 4, item 7, and item 8) and BBS (item 1, item 7, item 9, item 11, and item 12) were closely related to the level of gait independence. The second study, 210 hemiparetic patients were included. The level of gait independence was assessed using the FAC. Gait ability of all participants were assessed using the OGA tools (Rivermead visual gait analysis; RVGA, Wisconsin gait scale; WGS, Tinetti gait scale; TGS, and functional gait assessment; FGA). Stepwise multiple regression analysis showed that the FGA tool was related to FAC. There was also a significant group differences on FGA total scores. In distinguishing independence level, the cutoff value for the FAC 1 from FAC 0 was 0.5 point, FAC 2 from under level was 5.5 point, FAC 3 from under level was 10.5 point, FAC 4 from under level was 14.5 point, and FAC 5 from under level was 18.5 point. Some sub-items of the FGA (item 1, item 2, item 3 and item 10) showed a more related to independence level of the gait. This study results showed that muscle strengths of the hip flexor, knee extensor and ankle plantarflexor, proprioception, coordination of the paretic leg are important factors in determining independence level of gait after stroke. There was a close relationship between MRMI and BBS scores and level of gait independence in stroke patients. Also, the FGA is a useful tool in determining independence level of gait after stroke. The cutoff values for each factor and the FGA score were valuable indicators to identify the level of gait independence. These findings can be beneficial in the assessment and developing appropriate goals for the rehabilitation of stroke patients with gait problems. Finally, future studies is needed to develop better assessment tools with more clinical factors to determine the level of gait independence in stroke patients.
