나타났다(P<0.001). 구강 건강상태에서 현존 자연치아 수는 남녀 모두 연령이 증가할수록 20개 이상 치아수의 비율은 감소하였다(P<0.001). 저작 불편 및 말하기 어려운 상태는 남자에서 는 연령별로 유의하지 않았지만 여자에서는 저작 불편이 65-74세와 75세 이상에서 연령별로 유의한 차이가 있었다(P=0.004). 식이보충제 및 식이요 법은 남자보다 여자가 더 많이 실천하고 있었으며 연령별로는 65세-74세 가 많았다. 만성질환의 보유수가 많을수록 현재흡연은 줄었으며 비흡연자도 많았다 (P=0.004). 주관적 건강상태는 만성질환 보유수가 많을수록 나쁜 인 식을 가지고 있는 것으로 조사되었고(P<0.001), 근력 운동도 매주 1회 이 하가 많았다(P=0.046). 만성질환 보유수가 많을수록 삶 의 질 지수가 낮았 으며, 활동제한이 많았다(P<0.001). 구강 건강상태는 만성질환의 보유수에 따라 유의한 차이는 없었다. 만성질환의 보유수가 많을수록 저작불편 및 말 하기 어려움은 많았고, 주관적 구강건강상태도 나쁜 것으로 조사되었다. 식 이보충제는 만성질환의 보유수가 많을수록 많이 섭취 하는 것으로 나타났다 (P<0.001). 3. 신체계측치는 연령별, 성별로 유의한 차이가 있었으며, 남자에서 허리 둘레는 90 cm 이상, BMI 25 kg/m2 이상이 65-74세에 더 많이 분포하고 있었고, 수축기 혈압은 남녀 모두 140 mmHg 이상이 75세 이상에서 높 았다. 혈액 분석치는 성별 및 연령별로 평균적으로는 정상범위에 속해 있었 지만 만성질환별로 관련되는 질환에 따라 신체계측치 및 혈액 분석치는 정 상치를 벗어난 범위를 나타내는 비율이 높았다. 또한 만성질환의 보유수에 따라 혈액 요소질소 및 혈소판을 제외한 모든 혈액 분석치 항목에서 유의적 인 차이가 있었다. 4. 만성질환군 중 남녀 모두에서 고혈압군이 가장 많았고 연령이 증가할 수록 더 많은 분포를 보였으며 여자가 남자보다 많았다(P=0.009). 그 다음 으로 이상지질혈증(고콜레스테롤혈증, 고중성지방혈증)을 가진 고령자가 많 았고 여자가 남자보다 많아 유의하였다(P<0.001). 비만군은 남자 65세- 74세가 많은 분포를 보였고 연령별로 유의적이었다(P=0.004). 여자는 연령 별로 유의적이지는 않았지만 전체적으로 비만한 상태가 남자보다 더 많았다 (P<0.001). 만성질환의 보유수가 없는 정상군은 남자가 20%, 여자는 12% 로 나타났다. 만성질환의 수는 남자가 1개 보유군이 많았고, 여자는 3개 이 상군이 많아서 여자가 남자보다 더 많았으며 유의적인 차이가 있었다 (P<0.001). 5. 고령자의 식품군별 섭취상태에서 남자는 감자∙전분류(P=0.022), 과일류(P<0.001), 식물성 유지류(P<0.001), 난류, 어류, 음료류, 주류 (P=0.004)가 연령별로 유의적인 차이가 있었으며, 여자는 곡류군(P=0.029), 두류, 채소류, 과일류(P<0.001), 동물성 유지류, 음료류(P<0.001)에서 연령 별로 유의적 차이가 있었다. 질환별로는 비만군이 곡류, 기타식물성, 육류 및 우유군의 섭취량이 다른 군보다 높았다. 고혈압군은 해조류, 어류 및 알 콜류의 섭취량이 높았다. 당뇨군은 종실류, 채소류, 과일류 및 음료류를 높 게 섭취한 것으로 조사되었다. 이상지질혈증군은 콩류, 버섯류, 식물성 유지 류, 난류 및 동물성 유지류에서 다른 군보다 높은 섭취량을 보였다. 심뇌혈 관 질환군은 감자∙전분류, 양념류에서 섭취량이 다른 군보다 높았으나 다른 식품군은 전체적으로 낮은 섭취량을 보였다. 만성질환의 보유수에 따라 3개 이상군에서는 곡류, 채소류, 버섯류, 과일류, 해조류, 육류, 난류, 어류 및 알 콜류의 섭취가 높은 것으로 나타났다. 만성질환 보유군에 따라 섭취에 유의 한 차이가 있는 것은 기타식물류(P=0.037)와 난류(P=0.025)였다. 6. 고령자의 영양소 섭취상태는 남자에서는 에너지, 단백질, 지방, 포화지 방산, 단일불포화지방산, 다가불포화지방산, ω-3 및 ω-6계 지방산, 탄수화 물, 식이섬유, 칼슘, 인, 철, 나트륨, 칼륨, 비타민 A, 레티놀, 티아민, 리 보플라빈, 나이아신, 비타민 C의 섭취량이 75세 이상에서 낮은 섭취를 보였 으며, 카로틴을 제외하고 모든 영양소가 연령별로 유의적인 차이가 있었다 (P<0.001). 여자에서는 75세 이상이 낮은 섭취량을 보였고 비타민 A, 레티놀 및 리보플라빈을 제외한 모든 영양소에서 유의한 차이를 보였다. 고 령자의 KDRI 대비 주요 영양소 분포는 남녀 및 연령별로 유의적인 차이가 있었다(P<0.001). 영양소별 KDRI 대비 결과는 에너지, 칼슘, 칼륨, 리보플 라빈, 나이아신 및 비타민 C가 75세 이상 남녀에서 낮은 섭취량을 보였다. 식사 질 지수인 INQ가 1 미만인 영양소는 남녀 모두 칼슘이 가장 낮았다. 리보플라빈, 나이아신, 칼륨의 섭취량이 낮았고, 비타민 C는 75세 이상 남 녀에서 낮았다. 건강식생활실천율을 나타내는 지방, 나트륨, 과일 및 채소, 영양 표시의 4개 지표 중 2개 이상을 만족하는 대상자는 남자와 여자 모두 28%로 조사되었다. 또한 건강식생활실천율 중 지방적정섭취섭취비는 남자 가 여자보다 많았고, 나트륨적정비는 남자가 여자보다 낮았다(P<0.001). 7. 질환별로 볼 때 비만군은 에너지, 단백질, 지방, 포화지방산, 단일불포 화지방산, 탄수화물, 인, 나트륨, 리보플라빈, 나이아신에서 높은 섭취량을 보였다. 당뇨군은 ω-3계 지방산, 식이섬유, 칼슘, 철, 비타민 A, 카로틴, 레 티놀, 비타민 C의 섭취량이 높은 것으로 조사되었다. 이상지질혈증군은 불 포화지방산, ω-6계 지방산, 리보플라빈의 섭취량이 높았다. 심뇌혈관계질환 군은 칼륨과 티아민의 섭취량이 높았다. 만성질환의 보유수에 따라 살펴보 면 만성질환이 3개 이상인 군은 에너지, 단백질, 식이섬유, 인, 나트륨, 칼 륨, 티아민, 리보플라빈, 나이아신, 비타민 C의 섭취가 다른 군보다 낮은 섭 취량을 보였다. 3개 이상군에서 에너지 분포는 탄수화물, 단백질 및 지방이 각 73%, 13%, 13%로 나타나 KDRI 대비 적정섭취량 범위에서 탄수화물은 초과하고 단백질과 지방은 적정범위보다 낮았다. 건강식생활을 실천하는 군 은 만성질환의 보유수가 2개군에서 많았지만 유의하지는 않았다. 8. 혈액 분석치에서 총 콜레스테롤(P<0.001), 중성지방(P<0.001), LDL- 콜레스테롤(P<0.001), 공복혈당(P<0.001), 당화혈색소(P<0.001)의 농도가 높을수록, HDL-콜레스테롤(P<0.001)은 낮을수록 만성질환이 2개 이상인 복합만성질환의 위험이 높아지는 것으로 분석되었다. 영양소에서는 철 (P=0.002), 티아민(P=0.001)의 섭취가 평균필요량 미만으로 섭취할 경우 만성질환이 2개 이상일 가능성이 높아지는 것으로 분석되었다. 9. 고령 만성질환자의 경구영양 보충식 관련 논문 분석에서는 문헌의 질 적 평가를 실시하였고 논문의 질 평가 도구는 Jadad scale을 사용하였다. 평가점수을 5점척도법으로 점수화한 결과 논문 평가 점수가 0-2점인 문헌 자료는 제외되었고 5점을 받은 문헌은 70편(59.3%), 4점 12편(10.2%), 3점 36편(30.5%)을 추출하여 최종적으로 총 118개의 문헌자료가 추출되어 분석에 사용되었다. 문헌자료의 경구영양 보충식에서 열량은 100 mL 당 100-240 kcal, 열량 밀도는 1.0-2.4 kcal/mL로 조사되었다. 총 에너지에 대한 탄수화물의 섭취비는 33-60%, 단백질 섭취비 12-30%, 지방 섭취비 는 18-40%로 조사되었다. 10. 국내외 시판용 경구영양 보충식품의 영양조성에서 열량은 100 mL 당 93-150 kcal, 열량밀도는 1.0-1.5 kcal/mL이었고, 식이섬유는 0.3-1.5 g으로 조사되었다. 탄수화물의 섭취비는 49-62%, 단백질 섭취비 14-20%, 지방 섭취비는 19-35%로 조사되었다. 11. 복합만성질환자의 영양지원 가이드라인을 수집, 검토한 결과 여러 가 지 영양불량 검증도구를 사용하여 상세한 평가와 치료계획이 수립되어야 한 다고 하였다. 에너지 요구량은 일반적으로 27 kcal/kg, 중증의 저체중환자 는 30 kcal/kg으로 추정하고 있다. 단백질은 하루에 최소 1.0 g/kg을 섭취 해야 하는 것으로 권고하였다. 일일 미량영양소의 요구량을 적절하게 섭취 하여 충족해야 하고 특정 영양소인 아르기닌, 글루타민 및 β-HMB는 치유 를 위해 첨가할 수 있으며 식이섬유도 장 기능 향상을 위해 사용할 수 있다 고 하였다. 영양불량 환자의 영양지원을 조직적으로 실시하기 위해서는 강화된 식단 제공과 다학제적인 영양 프로토콜을 사용한 중재가 실시되어야 한다고 권고하였다. 12. 델파이기법을 사용하여 전문가의 의견을 수집하여 이를 영양조성의 제시 내용에 반영하였다. 델파이조사는 2회에 걸쳐 실시하였으며 고령자의 만성질환 현황은 평균 4-5점에 100%의 동의를 보였다. 고령자의 만성질 환 발생에 영향을 미치는 요인과 경구영양 보충식 관련 문항은 70% 이상의 동의를 보였다. 경구영양 보충식 선택 시 고려할 사항은 70% 이상의 동의 를 보였고, 영양조성 제안에 관련한 전문가의 의견은 대부분 타당하다는 견해를 나타냈다. 그러나 비타민 및 무기질의 경우 상한섭취량은 안전한 것 이 아니므로 최대 80%를 넘지 않도록 하면서 영양소의 종류에 따라 적절 하게 조정하여 설정하면 좋겠다는 의견이 있었다. 13. 본 연구에서는 경구영양 보충식 개발 제품의 영양조성에서 열량은 100 mL 당 100-240 kcal, 열량밀도는 1.0-2.4 kcal/mL, 식이섬유는 100 mL 당 0-1.2 g을 제안하였다. 탄수화물의 섭취는 100 mL 당 총 에너지에 대해 탄수화물 섭취비 50-65%, 단백질 섭취비 12-20%, 지방 섭취비 18- 30%로 제안하였다. 비타민 및 무기질의 최소치는 중재연구에서 영양개선이 확인된 문헌자료의 결과를 적용하여 100 mL 당 최소치로 하고 최대치는 영양소 섭취기준의 상한치를 보정하여 제안하였다. 본 연구에서 국민건강영양조사 자료를 이용하여 고령자와 고령 환자의 건 강 및 영양상태를 분석한 결과를 종합하면 영양불량의 예방과 영양상태 개 선을 위해 경구영양 보충식을 통한 영양지원이 필요하다고 사료된다. 고령 만성질환자용 경구영양 보충식은 영양소가 과학적 근거에 기반하여 적정하 게 함유되어야 하고, 필요시에는 강화된 영양성분을 첨가할 수 있을 것이 다. 앞으로 우리나라에서 급격한 고령화로 인해 만성질환자가 증가하는 현 실을 감안할 때 고령 만성질환자용 경구영양 보충식의 영양조성은 개별 맞 춤형에 대한 다양한 요구를 반영함으로써 고령자 영양불량의 예방과 관리에 도움이 되도록 해야 할 것으로 사료된다.

탄수화 물, 식이섬유, 칼슘, 인, 철, 나트륨, 칼륨, 비타민 A, 레티놀, 티아민, 리 보플라빈, 나이아신, 비타민 C의 섭취량이 75세 이상에서 낮은 섭취를 보였 으며, 카로틴을 제외하고 모든 영양소가 연령별로 유의적인 차이가 있었다 (P<0.001). 여자에서는 75세 이상이 낮은 섭취량을 보였고 비타민 A, 레티놀 및 리보플라빈을 제외한 모든 영양소에서 유의한 차이를 보였다. 고 령자의 KDRI 대비 주요 영양소 분포는 남녀 및 연령별로 유의적인 차이가 있었다(P<0.001). 영양소별 KDRI 대비 결과는 에너지, 칼슘, 칼륨, 리보플 라빈, 나이아신 및 비타민 C가 75세 이상 남녀에서 낮은 섭취량을 보였다. 식사 질 지수인 INQ가 1 미만인 영양소는 남녀 모두 칼슘이 가장 낮았다. 지방산, 리보플라빈의 섭취량이 높았다. 심뇌혈관계질환 군은 칼륨과 티아민의 섭취량이 높았다. 만성질환의 보유수에 따라 살펴보 면 만성질환이 3개 이상인 군은 에너지, 단백질, 식이섬유, 인, 나트륨, 칼 륨, 티아민, 리보플라빈, 나이아신, 비타민 C의 섭취가 다른 군보다 낮은 섭 취량을 보였다. 3개 이상군에서 에너지 분포는 탄수화물, 단백질 및 지방이 각 73%, 13%, 13%로 나타나 KDRI 대비 적정섭취량 범위에서 탄수화물은 초과하고 단백질과 지방은 적정범위보다 낮았다. 건강식생활을 실천하는 군 은 만성질환의 보유수가 2개군에서 많았지만 유의하지는 않았다. 8. 혈액 분석치에서 총 콜레스테롤(P<0.001), 중성지방(P<0.001), LDL- 콜레스테롤(P<0.001), 공복혈당(P<0.001), 당화혈색소(P<0.001)의 농도가 높을수록, HDL-콜레스테롤(P<0.001)은 낮을수록 만성질환이 2개 이상인 복합만성질환의 위험이 높아지는 것으로 분석되었다. 영양소에서는 철 (P=0.002), 티아민(P=0.001)의 섭취가 평균필요량 미만으로 섭취할 경우 만성질환이 2개 이상일 가능성이 높아지는 것으로 분석되었다. 9. 고령 만성질환자의 경구영양 보충식 관련 논문 분석에서는 문헌의 질 적 평가를 실시하였고 논문의 질 평가 도구는 Jadad scale을 사용하였다. 평가점수을 5점척도법으로 점수화한 결과 논문 평가 점수가 0-2점인 문헌 자료는 제외되었고 5점을 받은 문헌은 70편(59.3%), 4점 12편(10.2%), 3점 36편(30.5%)을 추출하여 최종적으로 총 118개의 문헌자료가 추출되어 분석에 사용되었다. 문헌자료의 경구영양 보충식에서 열량은 100 mL 당 100-240 kcal, 열량 밀도는 1.0-2.4 kcal/mL로 조사되었다. 총 에너지에 대한 탄수화물의 섭취비는 33-60%, 단백질 섭취비 12-30%, 지방 섭취비 는 18-40%로 조사되었다. 10. 국내외 시판용 경구영양 보충식품의 영양조성에서 열량은 100 mL 당 93-150 kcal, 열량밀도는 1.0-1.5 kcal/mL이었고, 식이섬유는 0.3-1.5 g으로 조사되었다. 탄수화물의 섭취비는 49-62%, 단백질 섭취비 14-20%, 지방 섭취비는 19-35%로 조사되었다. 11. 복합만성질환자의 영양지원 가이드라인을 수집, 검토한 결과 여러 가 지 영양불량 검증도구를 사용하여 상세한 평가와 치료계획이 수립되어야 한 다고 하였다. 에너지 요구량은 일반적으로 27 kcal/kg, 중증의 저체중환자 는 30 kcal/kg으로 추정하고 있다. 단백질은 하루에 최소 1.0 g/kg을 섭취 해야 하는 것으로 권고하였다. 일일 미량영양소의 요구량을 적절하게 섭취 하여 충족해야 하고 특정 영양소인 아르기닌, 글루타민 및 β-HMB는 치유 를 위해 첨가할 수 있으며 식이섬유도 장 기능 향상을 위해 사용할 수 있다 고 하였다. 영양불량 환자의 영양지원을 조직적으로 실시하기 위해서는 강화된 식단 제공과 다학제적인 영양 프로토콜을 사용한 중재가 실시되어야 한다고 권고하였다. 12. 델파이기법을 사용하여 전문가의 의견을 수집하여 이를 영양조성의 제시 내용에 반영하였다. 델파이조사는 2회에 걸쳐 실시하였으며 고령자의 만성질환 현황은 평균 4-5점에 100%의 동의를 보였다. 고령자의 만성질 환 발생에 영향을 미치는 요인과 경구영양 보충식 관련 문항은 70% 이상의 동의를 보였다. 경구영양 보충식 선택 시 고려할 사항은 70% 이상의 동의 를 보였고, 영양조성 제안에 관련한 전문가의 의견은 대부분 타당하다는 견해를 나타냈다. 그러나 비타민 및 무기질의 경우 상한섭취량은 안전한 것 이 아니므로 최대 80%를 넘지 않도록 하면서 영양소의 종류에 따라 적절 하게 조정하여 설정하면 좋겠다는 의견이 있었다. 13. 본 연구에서는 경구영양 보충식 개발 제품의 영양조성에서 열량은 100 mL 당 100-240 kcal, 열량밀도는 1.0-2.4 kcal/mL, 식이섬유는 100 mL 당 0-1.2 g을 제안하였다. 탄수화물의 섭취는 100 mL 당 총 에너지에 대해 탄수화물 섭취비 50-65%, 단백질 섭취비 12-20%, 지방 섭취비 18- 30%로 제안하였다. 비타민 및 무기질의 최소치는 중재연구에서 영양개선이 확인된 문헌자료의 결과를 적용하여 100 mL 당 최소치로 하고 최대치는 영양소 섭취기준의 상한치를 보정하여 제안하였다. 본 연구에서 국민건강영양조사 자료를 이용하여 고령자와 고령 환자의 건 강 및 영양상태를 분석한 결과를 종합하면 영양불량의 예방과 영양상태 개 선을 위해 경구영양 보충식을 통한 영양지원이 필요하다고 사료된다. 고령 만성질환자용 경구영양 보충식은 영양소가 과학적 근거에 기반하여 적정하 게 함유되어야 하고, 필요시에는 강화된 영양성분을 첨가할 수 있을 것이 다. 앞으로 우리나라에서 급격한 고령화로 인해 만성질환자가 증가하는 현 실을 감안할 때 고령 만성질환자용 경구영양 보충식의 영양조성은 개별 맞 춤형에 대한 다양한 요구를 반영함으로써 고령자 영양불량의 예방과 관리에 도움이 되도록 해야 할 것으로 사료된다.

The purpose of this study was to investigate nutritional status of Korean elderly using data of the National Health and Nutrition Survey (2013–2015), and then develop the nutritional composition of oral nutritonal supplements for the elderly patients. Nutritional composition of oral nutritional supplements for the elderly patients with chronic diseases was developed by investigating the nutrition composition of oral nutritional supplements products in markets and articles from intervention studies related to oral nutritional supplements. Based on these data, the nutritional composition of oral nutritional supplements to improve nutrition status of elderly patients with chronic diseases was developed. The main results of this study are summarized as below. 1. In this study, there were 1,622 (71.7%) subjects between age 65-74 years and 688 (28.3%) over age 75 years. The general characteristics of subjects were as follows: a total 69.3% of married spouses lived together, 67.4% were not employed, 57.9% had household incomes of ‘over one million won’, 42.1% reported incomes of ‘less than one million won’, and 58.9% had education levels ‘below elementary school’, As for general characteristics, according to the number of chronic diseases, most of those living with their spouses were in the participant group without any diseases. Subjects lived alone owing to death of their spouse, separation, or divorce (P<0.001). The group of subjects with three or more chronic diseases were the largest. The greater the number of chronic diseases among subjects, the less they participated in economic activities (P=0.004), and the higher the subjects education level, the fewer the number of chronic diseases (P=0.004). 2. Among the health behaviors in this elderly population, there were significant differences in drinking, walking exercise, resistance exercise, and quality of life index among men and smoking, walking exercise, resistance exercise, and quality of life index among women. Overall, there was decreased with older age and all factors were significant by gender (P<0.001). Oral health status among all subjects showed fewer teeth with increasing age; there were few subjects with more than 20 teeth (P<0.001). Chewing and speaking difficulties showed no significant difference according to age in men but the difference was significant by age in women. Use of dietary supplements and dietary therapy was most frequent in women aged 65-74 years. With a higher number of chronic diseases, the number of smokers decreased and the number of nonsmokers increased (P=0.004). With more chronic diseases. subjective health status was more often perceived as poor (P<0.001) and more subjects practiced resistance exercise less than once a week (P=0.046). The higher the number of chronic diseases, the lower the quality of life index and the greater the activity limitation (P<0.001). Oral health status was not significant according to number of chronic diseases. The more chronic diseases, the more subjects had difficulty chewing and speaking and the subjective oral health condition was found to be worse. We found that dietary supplements were consumed more often with more chronic diseases (P<0.001). 3. Anthropometric measurements were significant difference according to age and sex. Waist circumference more than 90 cm for men and BMI more than 25 kg/m2 were more frequently distributed among subjects age 65-74 years old. Systolic blood pressure over 140 mmHg was more frequent in both men and women over 75 years old. The biochemical parameter distribution was within the normal ranges, on average, by age and gender; however, anthropometric measurements and biochemical parameter levels were abnormal among subjects with chronic diseases. In addition, there was a significant difference in the remaining parameters, except for blood urea nitrogen, according to the number of chronic diseases. 4. In the chronic disease group, hypertension was more common in both men and women than in the control group. With increased age, there was more frequent distribution of hypertension, and hypertension was more frequent among women than men (P=0.009). Dyslipidemia (hypercholesterolemia, hypertriglyceridemia) was the next most frequent condition among elderly subjects and was more frequent among women than men (P<0.001). The group with obesity showed a large distribution of men aged 65-74 years (P=0.004). Among women, there was no significant difference by age but overall, more women were obese than men (P<0.001). The normal group without chronic diseases showed that 20% of men and 12% of women were obese. The number of chronic diseases was higher in the one group of men than the three groups of women. There were more women than men, with significant differences (P<0.001). 5. The intake status of food groups among elderly subjects was significantly different according to age for potatoes and starches (P=0.022), fruits (P<0.001), vegetable oils (P<0.001), eggs, fish, beverages, and alcohol (P=0.004) in men and for grains (P=0.029), beans, vegetables, fruits (P<0.001), animal fat, and beverages (P<0.001) in women. The obesity group showed higher intakes of grains, other vegetables, meats, and milk than the other groups. The hypertensive group had high intakes of seaweed, fish, and alcohol. Subjects with diabetes were found to have high intakes of seeds, vegetables, fruits, and beverages. The dyslipidemia group showed higher intakes of beans, mushrooms, vegetable oils, eggs, and animal fat than the other groups. The group with cardiocerebrovascular diseases had higher intakes than the other groups of potatoes and starches and seasonings whereas the other food groups showed lower over all intake. Intakes of grains, vegetables, mushrooms, fruits, seaweed, meats, eggs, fish, and alcohol were higher in subjects with three or more chronic diseases. Significant differences according to the number of chronic diseases were found for other plant foods (P=0.037) and eggs (P=0.025). 6. Nutrient intake status among elderly subjects was lower in men over 75 years old for energy, protein, fat, saturated fatty acids, monounsaturated fatty acids, polyunsaturated fatty acids, ω-3 and ω-6 fatty acids, carbohydrates, dietary fiber, calcium, potassium, vitamin A, retinol, thiamin, riboflavin, niacin, and vitamin C. All nutrients except carotene were statistically significant by age (P<0.001). A significant difference was found for all nutrients except vitamin A, retinol, and riboflavin in women over 75 years old. The distribution of major nutrients compared with the dietary reference intake for Koreans (KDRI) in elderly subjects was significant in both men and women by age (P<0.001). Nutrient intakes of energy, calcium, potassium, riboflavin, niacin, and vitamin C were lower in both men and women over 75 years of age compared with KDRI, by nutrient. Nutrients with an index of nutritional quality (INQ) less than 1 had the lowest calcium. The intakes of riboflavin, niacin, potassium, and vitamin C were lower in both men and women over 75 years old. Among both men (P=0.021) and women (P=0.060), 28% of subjects satisfied two or more of the four indices of fat, sodium, fruits and vegetables, and nutrition labeling, which indicate healthy eating habits. In addition, the appropriate intake ratio of fat was higher in men than in women, and that of sodium was lower in men than women (P<0.001). More male than female subjects consumed sufficient vegetables and fruits. 7. The obesity group showed high intakes of energy, protein, fat, saturated fatty acids, monounsaturated fatty acids, carbohydrates, phosphorus, sodium, riboflavin, and niacin. Subjects with diabetes were iron, vitamin A, carotene, retinol, and vitamin C. In the dyslipidemia group, intakes were high of unsaturated fatty acids, ω-6 fatty acids, and riboflavin. Subjects with cardiocerebrovascular diseases had high intakes of potassium and thiamin. Subjects with three or more chronic diseases had lower intakes than other groups of energy, protein, dietary fiber, phosphorus, sodium, potassium, thiamin, riboflavin, niacin, and vitamin C. In the group with three or more chronic diseases, the energy distribution was 73%, 13%, and 13% for carbohydrates, protein, and fat, respectively. The optimal intake range for KDRI was exceeded for carbohydrates, and those for protein and fat were lower than the appropriate range. The subject group with two or more chronic diseases showed the most healthy eating habits but this was not significant (P=0.619). 8. The possibility of multimorbidity prevalence was higher concentrations of total cholesterol (P<0.001), triglycerides (P<0.001), LDL-cholesterol (P<0.001), fasting blood glucose (P<0.001), glycated hemoglobin (P<0.001), otherwise it was lower concentration of HDL-cholesterol (P<0.001). It was analyzed that the intake of iron (P=0.002) and thiamine (P=0.001) in the nutrient intake less than estimated average requirement increased the possibility of more than two chronic diseases. 9. The articles related to oral nutritional supplements were extracted and used for analysis. The Jadad scale was used to evaluate the quality based on clinical evidence. The evaluation score was obtained using the 5-point scale method, and 118 articles with 3–5 points were included in the analysis. The energy content of oral nutritional supplements in intervention studies ranged from 100 to 240 kcal/100 mL, 1.0 to 2.4 kcal/mL of energy density. The composition ratio of oral nutritional supplements from the articles was 33–60% for carbohydrate, 12–30% for protein, and 18–40% for fat. 10. The energy content of oral nutritional supplement for market products ranged from 93 to 150 kcal/100 mL, 1.0 to 1.5 kcal/mL of energy density, and 0.3 to 1.5 g/100 mL of dietary fiber. As for the composition ratio of products, the ratio of carbohydrates was 49–62%, 14–20% for protein, and 19–35% for fat. 11. Nutritional support guidelines for multimorbidity should include detailed assessment and treatment planning using a variety of malnutrition verification tools. Indirect calorie from energy requirements are estimated as 27 kcal/kg using actual body weight and 30 kcal/kg for severely underweight patients. Proteins should account for at least 1.0 g/kg per day to reduce re-entry rates and risk of complications as well as to prevent weight loss and improve functional outcomes. Specific nutrients such as arginine, glutamine, and β-HMB be added to speed healing, and dietary fiber can be used to enhance bowel function. To systematically implement nutritional support for malnourished patients, it has been recommended that provision of an enhanced menu and multidisciplinary nutrition protocols be used. 12. The opinions of experts using Delphi technique were collected twice. The status related to chronic diseases among elderly subjects was an average 4–5 points with 100% agreement. Factors affecting chronic diseases in elderly people and oral nutritional supplementation items showed more than 70% agreement. Things to consider when choosing oral opinions related to the recommendations were found to be reasonable. However, in the case of vitamins and minerals, the tolerable upper intake level was not considered safe; it was the opinion as that it should be set as low as possible according to the type of nutrients, while not exceeding a maximum of 80%. 13. The energy content of oral nutritional supplements suggested from this study ranged from 100 to 240 kcal/100 mL, 1.0 to 2.4 kcal/mL of energy density, and 0 to 1.2 g/100 mL of dietary fiber. As for the composition ratio of products, the proposed range for intake ratio of carbohydrates was 33–65%, 12–20% for protein, and 18–30% for fat. The minimum value of vitamins and minerals was proposed as the minimum value per 100 mL of documented data regarding nutritional improvement in intervention studies, and the maximum value was corrected by adjusting the tolerable upper intake level of KDRI. In this study, we examined the nutritional status of Korean elderly through analysis of National Health Nutrition Survey data. This population is thought to need nutritional support with oral nutritional supplements, to prevent malnutrition and improve nutritional status. Oral nutritional supplements for elderly patients with chronic diseases should be based on scientific evidence showing adequate nutrient content and enhanced nutrients can be added when necessary. Given the increasing number of chronic diseases due to aging, the nutritional composition of oral nutritional supplements for elderly patients with chronic diseases will need to be adapted for the prevention and management of malnutrition among elderly populations.
