Introduction: It is important to find a way to delay the onset of disability to lessen the burden of long-term care in aging society. Fracture is one of the risk factors that affect physical functions and make the older people dependent. This study aimed to test 1) investigate the risk of fragility fractures by urinary incontinence and fall in the elderly 2) how much more the older adults who experienced fractures entry into long-term care status compared with those who did not. Materials and Methods: 1) Between 2007 and 2012, 39,854 Korean adults(age, 66-80 years) who participated in health examination and were followed up until 2015 were analyzed. The patient group and comparison group were classified with or without the elderly population with urinary incontinence, respectively. Hazard ratios (HRs) of fragility fractures were calculated for the risk of urinary incontinence with fall using the Cox proportional hazard model. 2) Analyses included insurants aged 65 years old and over from Korean National Health Insurance Service-Senior cohort study (2002~2013). Cox proportional hazard models were used to calculate the hazard ratios of the first certification of initiation of long-term care after fracture, by fracture sites and multiple recurrent fractures. Results: 1) The risk of fractures in men with a history of falls is 1.25 times (p=0.097) higher than those without, and in women with a history of falls, 1.30 times (p<.001) higher than those without. However, the results were not significant for both men and women in the case of urinary incontinence. 2) The incidence rate of the first getting long-term care beneficiaries was approximately 2.5 times higher when older people had experienced fracture and they entered long-term care beneficiaries status three years earlier compared with those who had not fracture event. Discussion: First, the urinary incontinence of the elderly was not significantly related to the occurrence of fractures when the interaction of falls was excluded, but the fall itself was a risk factor for fractures. second, fracture has been a factor that has increased the risk of getting into a long-term care than other chronic diseases, and has pushed the entry time by 3 to 10 years. additional attention to fracture sites in prevention and rehabilitation settings at reducing disability and related long-term care burden may be warranted.
