Scarce literature exists describing the patterns of readmission after heart transplantation. This retrospective study sought to examine rates of readmission after heart transplantation and determine primary etiologies and risk factors for unplanned readmission within 1 year. This study cohort included patients age 18 years or older who received a heart transplantation between November 1992 and December 2016 at a tertiary hospital, located in Seoul, Korea. Patients who died within 1 year and who underwent re-transplantation were excluded. Final cohort for analysis consisted of 484 heart transplantation patients with readmission (n=204) and non-readmission (n=280). All analyses were performed using Window Statistical Package for Social Science program version 23. Data were analyzed by mean, standard deviation, percentage, Independent t-test, Fisher’s exact test, and χ²-test according to the characteristics of variables and a logistic regression analysis was performed to identify risk factors associated with 1 year readmission. The results of the study are as follow: 1. The readmission rate of heart transplantations was 42.1%. 2. As an individual factor, there was a significant increased risk of being readmitted within 1 year among patients who received heart transplantation before the year 2000. 3. In disease-related and therapeutic factors, the readmission were significant differences according to postoperative creatinine levels, cholesterol synthesis inhibitor medications, and immuno-suppressive therapies. 4. The characteristics of readmission after heart transplantation were infection (49.2%), rejection (16.8%), cancer (2.8%), CAV (cardiac allograft vasculopathy) (1.2%), nephrotic disease (4.4%) and others (25.6%). 5. In logistic regression analysis, operation date: after the year 2000 (odds ratio [OR]=0.419, 95% CI=[0.246, 0.713], p=.001), postoperative creatinine levels: High (OR=1.539, 95% CI=[1.062, 2.232], p=.023), postoperative cholesterol synthesis inhibitor medication use (OR=0.663, 95% CI=[0.441, 0.996], p=.048), and immuno-suppressive therapy were associated with increased odds of readmission. In conclusion, healthcare professionals should carefully assess risk factors during the postoperative period to reduce preventable readmissions after heart transplantation. Also, this study will inform quality improvement intervention to develop educational materials and protocols for nurses who take care of patients with heart transplantation.
