연구목적 : 심정지 환자에게 자동 심장충격기 적용 시 ‘충격 권고’와 ‘충격 권고되지 않음’ 의 두 가지 경우가 발생하나, 기존 자동 심장충격기 실습은 충격을 권고하는 실습교육만 진행하고 있다. 이러한 교육은 일반인 구조자가 실제로 심정지 환자에게 자동 심장충격기 적용 시 사용 미숙으로 인한 가슴 압박 중지시간이 길게 나타날 수 있다. 본 연구의 목적은 자동 심장충격기 실습 교육방법 차이에 따른 수행능력 비교 및 자동 심장충격기 적용 시 발생 되는 가슴 압박 중지시간을 비교하고자 하였으며, 교육을 통한 교육 전·후 자기효능감 차이를 보고자 하였다. 연구방법 : 심폐소생술교육은 받았지만, 자동 심장충격기 교육을 받지 않은 대학교 1학년 학생들을 대상으로 2015 미국심장협회와 대한심폐소생술협회의 심폐소생술 지침을 토대로 자동 심장충격기 이론교육을 진행하였으며, 실습 교육 시 충격 권고하는 실습만 진행한 그룹과 충격 권고와 충격을 권고하지 않는 실습 교육을 한 두 그룹의 교육 차이를 보고자 하였다. 교육 후 심정지 상황을 구현하여 자동 심장충격기 숙련도, 가슴 압박 중지 시간 자료 수집하였으며, 설문지를 통해 자가 인지도평가, 교육 전·후 자기효능감 자료를 수집 후 통계적으로 분석하였다. 연구결과 : 두 그룹 간의 자동 심장충격기 숙련도는 통계적으로 유의미한 차이가 나타나지 않았다. 충격을 권고하지 않는 경우(Non shockable)의 가슴 압박 중지시간에서 실험군이 대조군보다 통계적으로 유의미하게 짧았다(2.30±1.21sec vs 3.16±1.73sec, p<0.01). 교육 전·후 자기효능감에서 두 그룹 모두 교육 전보다 교육 후 자기 효능감이 높은 것으로 나타났다. 결론 : 충격 권고와 충격 권고하지 않는 실습을 모두 진행한 경우 자동 심장충격기 적용 시 발생 되는 가슴 압박 중지 시간이 짧은 것으로 나타났다.

When a lay person actually applies the AED to a patient with cardiac arrest, such training may result in a longer interrupted chest compression time due to his or her lack of AED proficiency. The purpose of this study was to compare proficiency and the interrupted chest compression time during the use of the AED, depending on different AED practice training methods and to report differences in self-efficacy before and after training. Methods: Based on the 2015 cardiopulmonary resuscitation (CPR) guidelines of the American Heart Association and the Korea Association of Cardiopulmonary Resuscitation, theoretical education about the AED was provided to college freshmen, who received training for CPR, but not AED. Subsequently, this author examined differences between the group which practiced only shockable rhythms in practice training and the group which practiced both shockable and non-shockable rhythms. After training, a cardiac arrest situation was simulated to collect data about AED proficiency and the interrupted chest compression time, while a questionnaire was used to collect data about self-awareness assessment and self-efficacy before and after training; these were then analyzed statistically. Results: AED proficiency between the two groups showed no statistically significant difference. The interrupted chest compression time for non-shockable rhythms in the experimental group was statistically significantly shorter than that in the control group (2.30±1.21 s vs. 3.16±1.73 s; p<0.01). In self-efficacy before and after training, both groups had a higher level of self-efficacy after training than before training. Conclusions: Training that provided practice on both shockable and non-shockable rhythms had a shorter interrupted chest compression time while applying the AED.
