Chest radiographs can be performed efficiently at low cost and are readily available at most institutions. Various studies have been conducted therein, and a large volume of chest radiographs that need to interpret present practical challenges for radiologists in maintaining diagnostic quality. It is also widely used in lung cancer prevention and screening. Reporting of the radiologist is desired in clinical diagnosis, but not always possible due to heavy workload. In other words, an automatic system of abnormality classification would be advantageous, allowing reporting works to focus more on analysis of abnormal identification in chest radiographs. Recently, deep learning techniques are considered to be state of the art for classification, detection, and segmentation of natural images, which arises from the recent success. According to the advance of deep learning in computer vision, deep learning can assist radiologists to diagnose multiple abnormalities in chest radiographs. In this thesis, we have conducted various studies based on deep learning with a large number of chest radiographs. We described datasets queried between 2011 and 2016 in our institutions and from 2017 to 2018 in others. These datasets consist of normal and abnormal, some of which were labeled in seven disease patterns on the basis of the report. These datasets were actually confirmed and delineated by attending thoracic radiologists with in-house software considering corresponding CT images scanned within one week. Afterward, deep learning approaches were proposed as the solution to filter non-chest radiographs being applicable for our development in the extracted datasets. After cleansing and de-identification, chest radiographs were preprocessed to apply a wide variety of developments based on deep learning because these datasets could have various kinds of natural and disease-caused variations. At first, all these datasets were cropped to fit images spatially with the coordinates of the lung boundary segmented using deep learning algorithms. In addition, the various kinds of preprocessing are applied to whole images and cropped images and their effects were evaluated. Second, for the classification of normal and abnormal in chest radiographs, performances of state-of-art network architectures were assessed by using standard binary classification metrics. For localization, visualization of CNN activations was also performed. Thirdly, we performed the detection of multiple abnormalities based on deep learning for chest radiographs. In addition, we revised the state-of-art network in You Only Look Once (YOLO) v2 and each model was used to infer multiple abnormalities with ensemble methods. Next, bone-suppression chest radiographs were specially developed with deep learning algorithms because of its importance in the clinical fields. Both bone suppression and original images were assessed for comparative analysis with the CAD based on deep learning. Further, due to the importance to evaluate paired chest radiographs within a short interval, the reproducibility of CAD was evaluated. Lastly, we integrated applicable captioning algorithms from these CAD for application to clinical workflows. In conclusion, we found that deep learning developments could assist radiologists to improve the quality of their diagnoses of a range of pulmonary lesions in clinical practice. Specifically, the preprocessing is necessary and crucial for the CAD developments on multi-center trials. The ability of deep learning such as classification and detection in chest radiographs could provide practical utility, which needs further study. For their utilization for imaging biomarkers in various clinical settings, the reproducibility of CAD with paired chest radiographs within a short interval was evaluated in this study. These algorithms could be integrated into an actual clinical setting, specifically in an emergency situation where trained radiologists are either overburdened or unavailable. Our studies suggest that deep learning algorithms showed empirical performance in various fields on chest radiographs and had the potential to be used in clinical settings for the diagnosis of various lung diseases via chest radiographs.
