Xenograft group persistant inflammatory reaction until 4 weeks, but in HA treated xenograft group inflammatory reaction was decreased at 2 weeks. 대상 데이터
30마리의 백서를 희생시킨 후 두정부에 정중부 절개를 3 cm 길이로 시행한 뒤 근육 및 골막을 박리하여 두개골을 노출시킨 후 치과용 high speed handpiece와 Round bur를 이용하여 전두골, 측두골, 후두골을 포함한 두개골을 채취하였다.
실험동물을 일정기간 동안 동일 조건에서 사육된 체중 250g 내외의 생후 3개월된 Spague-Dawly계 백서 30마리를 대조군 및 실험군 15마리씩 2개군으로 분류하였다.
성능/효과
1. 모든 이식재료에서 이식 초기에는 염증반응을 발견할 수 있었는데, 자가골과 동종골인 백서의 탈회동결건조골에서는 1주에서만, 이종골인 사람의 탈회동결건조골이식군에서는 4주에서도 염증반응을 보였다. 그러나 히알루론산을 처리한 사람의 탈회동결건조골군에서는 2주까지에서만 염증반응을 발견할 수 있었다.
2. 조골세포 활성도는 대조군에서는 모두 2주에서부터 보였지만 사람의 탈회동결건조골을 처리한 실험군에서는 6주부터 관찰되었고, 히알루론산을 처리한 사람의 탈회동결건조골군에서는 4주에서부터 관찰되었다.
3. 파골세포 활성은 2주에 이종골 이식시 히알루론산 투여유무에 상관없이 경도로 나타났으며 4주이후에는 군간에는 차이가 없었다.

대조군인 자가골 이식군과 동종골 이식군은 동종골이식군이 실험 1주에서 자가골 이식군보다 약간 많은 육안적 염증소견 및 이식편의 이동등을 발견할 수 있었으나 2주에서 염증반응이 적어지면서 주위골과 약한 결합을 보여주었고(Fig. 1A), 4주후에는 두 이식군 모두 주위골과 점차 융화되는 양상을 발견할 수 있었으며 두 군간에는 차이가 없는 유사한 치유 양상을 보였다. 6주군에서는 두 군 모두 주위골과 구별하기 힘들 정도로 골 형성이 잘되었고(Fig.
대조군인 자가골과 백서의 동종골 이식군 두 군은 골 치유과정이 유사하게 나타났는데 골결손부가 성숙골로 대체되어 6주째에서는 거의 숙주골과 융합되는 양상을 발견할 수 있었다 (Fig. 5A, 5B).
대조군인 자가골과 백서의 동종골 이식군 모두 골이식 부위에는 중등도의 염증세포 침윤을 볼 수 있는(Fig. 2A, 2B) 반면, 이종골군에서는 가장 심한 염증세포침윤을 관찰할 수 있었는데 이식골 주위에 출혈과 많은 염증세포 침윤을 동반하고 있었다(Fig. 2C).
대조군인 자가골과 백서의 동종골 이식군의 4주에서 두 군 모두 증가된 조골세포 활성도와 숙주골주위로 신생골이 활발히 형성되는 모습이 관찰되었다(Fig. 4A, 4B).
본 실험에서도 단순히 탈회 및 동결건조를 시킨 이종골의 이식군에서는 이식 초기부터 심한 염증반응 및 일부에서는 이식재료가 이동하는 양상을 보여주어 항원성이 아직도 유지되고 있으며 이식재료의 고정도 용의치 않음을 알 수 있었다. 이러한 염증반응은 약 4주까지도 계속되었고 염증반응이 가라앉은 6주에서야 골형성 능력이 나타남을 발견할 수 있었다.
본 연구에서는 실험실상 골 형성 능력이 있다고 보고된37) 히알루론산을 생체내 이종골에 투여시 이종골만을 이식한 군에서 4주까지 계속 남아있던 염증반응이 2주에서부터 약해지면서 빨리 감소하며 4주부터 조골세포의 활성도가 조금씩 나타나는 양상을 발견할 수 있었다. 따라서 히알루론산의 처리가 항원성이 높은 이종골의 초기 염증반응을 빨리 감소시키며 조골세포를 활성화 시키는데 영향을 미친다고 생각된다.
이종골 이식군에서는 조골세포 활성도, 신생골 형성, 성숙된 골로의 대치 등이 6주와 큰 변화가 없었으며 히알루론산을 처리한 이종골군에서는 6주보다는 증가된 중등도의 조골세포 활성도 양상을 발견하였으나 신생골의 생성 및 골의 성숙은 아직도 미약한 양상을 보여주고 있었다.
히알루론산을 처리한 이종골군에서도 약간의 차이는 있었지만 많은 염증세포침윤을 발견할 수 있었으며 히알루론산 처리유무에 따른 염증세포 침윤, 신생골 형성 차이는 없었다. 파골세포 출현은 실험군과 대조군간의 차이가 없이 미약하였으며 신생골 형성은 전군에서 관찰되지 않았고 모세혈관 형성 또는 충혈은 대조군보다는 이종골군에서 가장 많았는데 히루론산 처리 이종골군에서는 이보다 감소된 중등도로 나타났다(Fig. 2D).

또한 다른 실험등에서도 이종골의 이식시 골 생성 등이 늦거나 미약한 경우가 많았고 12주 이상 장기적으로 관찰한 경우가 많아, 조골세포 활성도가 4주에서 처음 나타나고 8주에서 증가된 점들을 고려해볼 때 12주이상의 장기적인 관찰에서는 보다 활발한 조골세포의 활성 및 신생골의 생성등이 관찰될 수 있을 것으로 사료된다. 따라서 앞으로 히알루론산의 골에 미치는 영향에 대한 임상적 응용과 투여 방법, 농도별 안정성 여부등에 관한 더욱 장기적이고, 체계적인 연구가 필요할 것으로 생각된다.
히알루론산 처리 이종골 이식군이 단순 이종골 이식군보다 빠른 시기인 4주부터 조골세포의 활성도가 보인다고 하지만 다른 동종골 등에서의 히알루론산이나 다른 성장인자의 처리시 보다는 상당히 늦게 활성도를 보이는 것으로 사료된다. 또한 골의 성숙되는 양상이나 신생골의 생성은 미약하거나 거의 약하게 나타나는 양상을 볼 때, 히알루론산이 이종골에서 골을 유도시키는지에 대한 보다 보완된 연구가 필요하다고 사료된다.
이상과 같이 백서의 골결손부에 이식된 이종골에 히알루론산을 처리한 경우는 이종골에서 발생된 염증이 보다 빨리 감소하며 조골 세포의 활성도 보다 조기에 발견되었으나 골의 성숙되는 양상이나 신생골의 생성은 미약하게 나타나 이에 대한 계속적인 연구가 필요할 것으로 사료된다.
