The HBV DNA was not detected in all subjects with isolated anti-HBc. Conclusion: None of the subjects with isolated anti-HBc had a false positive result (primary response); therefore, they should be excluded from vaccination programs in Korea. To differentiate between immunity and occult infections, a single dose of vaccine, with a follow-up anti-HBs test, is preferable for subjects with isolated anti-HBc. An amnestic response indicates late immunity, and no response a suspect occult infection.

제안 방법

대상 데이터
2002년 8월부터 2003년 12월까지 일개 대학병원 종합검진센터를 방문한 20세 이상 60세 미만의 수진자에서 6개월 이상의간격울 두고 시행한 검사에서 HBsAgQ, anti-HBs(-)JgG 竊-HBc(뉘, anti-HCV(-), 및 간기 능(aspartate aminotransferase, alanine aminotransferase, r-glutamyltranspeptidase)0 ] 정상이고, B형 간염 예방접종과수혈의 과거력이 없는 경우를 실험군으로 하였다. 과거력에서 항체 생성에 영향을 줄 수 있는 만성간질환자나 면역 관련 질환, 스테로이드제재를 포함한 면역 억제 치료를 받고 있는 경우, 임신 또는 수유중이거나, 현재 중한 질환이 있는 경우는 모두 제외하였다.

그러나우리 나라에서 1983년부터 B형 간염 예방접종이 시행된점울 감안한다면, 본인이 접종울기억하기 어려운 20세 미만은 연구 대상에서 제외하였기 때문에 많지는 않았으리라고 본다. 둘째, 일개 검진센터방문자를 대상으로 하여 이들이 우리 나라 전체 인구를 대표한다고 보기는 어 렵기 때문에 본 연구결과를 일반화하기에는 무리가 있다. 검진센터 방문자는 지역 사회 인구와 비교하여 교육과 경제 수준이 높고보다 긍정적인 건강행위(흡연, 음주, 운동) 를 가지고 있다.
본 연구의 제한점은 첫째, 예방접종력울연구 대상자의 기 억 에 의 존하여 recall bias 가작용하여 과거에 예방 접종 울 받았던 사람도 연구 대상자에 포함되었울 수 있다. 그러나우리 나라에서 1983년부터 B형 간염 예방접종이 시행된점울 감안한다면, 본인이 접종울기억하기 어려운 20세 미만은 연구 대상에서 제외하였기 때문에 많지는 않았으리라고 본다.
