Objecive and Method: Smoking among health professionals has been shown to influence smoking related knowledge, attitude and educational practices in medical setting. And lack of health professionals' efficacy for smoking cessation intervention has been a major barrier to education on smoking too. In this regard, the present study was carried out to introduce and discuss the advanced cases of smoking cessation education for health professionals, and to develop theory-based educational models of smoking cessation for health professionals in order to improve the effectiveness and efficiency of intervention on smoking in a medical framework. Results: First, major issues of health professionals' smoking cessation intervention were discussed. Discussed issues were smoking prevalence among health professionals, importance of health professionals' roles both as health educators and examples, and health professionals' cognitive dissonance. As advanced cases of smoking cessation education for health professionals, ATOD(Alcohol, Tabacco, and Other Drug problem prevention) developed by US Department of Health & Human Services and the Rx for Change curriculum in California State were discussed. Finally, smoking cessation educational models for health professionals were developed on the basis of social cognitive model and TPB/TTM. Conclusions: For the effective and efficient smoking cessation intervention in medical setting, systematic efforts would be necessary to provide opportunities for ensuring the qualification of health professionals on smoking cessation through an analysis of major issues concerning smoking cessation education for health professional and the development of comprehensive curriculum for smoking cessation.

행동요소로는 금연기술과 자기관리 기술습득이 포함된다. 이러한 기술발달온 금연이라는 목표행동을 효율적으로 달성하는 방법을 제시함으로써 금 연성공의 가능성을 높여준다. 또한 보건의료인의 흡연이유 중 하나인 스트레스를 효과적으로 대처할 수 있는 기술을 습득하게 하고, 자신의 금연노력 뿐 아니라 타인의 금연을 지원하는 기술까지 학습하여 향후 본인이 금연교육을 수행할 경우 교육자로서의 능력향상에 도옴을 주도록 고안된 것이다.

제안 방법

둘째, 교육내용의 필요도 및 중요도를 부가하여 수준별 학습이 이루어질 수 있도록 한다. 셋째, 이론적 모형에 근거한 행동 변화전략을 수립하고, 이에 관한 지침서를 제공 한다. 넷째, 효과적인 교육이 수행될 수 있도록 학습자 중심의 교육방법을 적극 활용한다.
또한 금연교육의 내용온 기본정보, 대상별 접근, 지역사회 활동 등 3개 모들로 구성하는 것이 바람직하며, 각 모들의 소주제에 관한 구체 적인 내용 개발이 이루어져야 한다. 이를 위해 사회인지이론에 근거한 포괄적인 보건의료인 금연교육모형을 그림 3과 같이 제시하고자 한다. 이 모형에서는 개인의 인지요소로 자기효능 감과 결과기대의 변화를 유도하여 금연실천을 촉진시키는데 중점을 두고 있다.
또한 금연교육의 핵심 주제인 금연행동을 유도하기 위해서는 행동변화단계에 영향을 미치는 태도 및 사회적 규범, 인지적 기술의 활용이 중요하며, 행동변화단계를 고려한 수행전략이 개발되어야 한다. 이에 계획된 행동론과 범이론적 모형을 통합 적용하여 흡연행동을 통제하고, 금연행동을 촉진시키기 위한 흡연행동 중재모형을 그림 4과 같이 제시하고자 한다. 즉 금연과 관련된 새로운 정보를 찾거나, 금연의 가치에 대하여 정서적, 인지적 재평가를 하고, 금연에 대한 사회적 인식을 알게 된다.

성능/효과

예를 들어, 흡연/금연관련 지식을 습득하였다 하더라도 이를 하나의 정보로만 가지고 있거나, 흡연에 대한 부정적인 태도는 갖게 되었지만 흡연하는 행동온 변화되지 않고 그대로 유지할 가능성도 있다. 둘째, 새로운 흡연/금연관련 지식을 습득하 는데 기존의 태도와 행동에 맞는 즉, 흡연이 주는 이득과 관련된 지식만 선택적으로 습득할 수도 있다. 또한 금연과 같온 건강행동을 실천 할 준비가 되어 있어도 주변의 환경(사회적, 믈리적)이 허락하지 않을 수도 있다.
특히, 금연에 대한 권고가 가장 효과적일 수 있는 사람으로 간주되고 있는 의사의 경우, 비 흡연의사 들이 흡연의사들에 비하여 금연에 대한 권고를 더 많이 하고 있는 것으로 보고되고 있다. 또한 흡연관련 지식에 관해서도 흡연하는 보건의료 인온 그렇지 않온 경우보다 흡연의 건강영향에 대하여 덜 심각하게 느끼는 것으로 보고되고 있어 흡연으로 인한 건강문제에 대해 과소평가하고 있었으며, 반면에 비흡연자의 경우 담배의 위험성을 과대평가하고 있었다. 한편, 병원에서 흡연에 대한 권고는 전문적 지식보다 개인적 경험에 의존하는 경향을 보였는데 과거흡연자 의 경우 자신들이 비흡연자보다 금연상담에 적격자인 것으로 생각하고 있었다(Willamg과 Ladelund, 2004).
미국 캘리포니아 주의 약학대학의 경우, 2000년부터 'the Rx for Change' 교과과정을 필수 교과과정으로 통합하여 교육하고 있다 (Robin 등, 2005). 미국 캘리포니아 주에 소재한 약학대학 3학년생 493명을 대상으로 'the Rx for Change' 교과과정을 7~8시간 수행하 고, 교육효과를 평가하기 위해 사전 사후평가를 수행한 결과, 금연교육을 잘 할 수 있다고 옹답한 학생이 교육 전 32%에서 교육 후 94%로 증가하였고, 교육에 참여한 학생 중 97%가 금연 교육을 통해 금연 상담의 질적 향상을 가져 올 수 있는 것으로 믿고 있었다. 또한 간호대학 재학생들에게 흡연예방을 위한 보건교육을 수행한 결과, 지식과 태도가 향상된 것으로 보고 하고 있다(Sejr와 Osler, 2002).
이는 금연실천을 지원하고, 현장적옹능력을 높이는 계기를 마련해 준다. 셋째, 일회성, 단기적 교육이 아닌 체계화 된 교육과정이 제공되어야 한다. 미국의 사례와 같이 금연교육이 정규교육과정에 포함되어 수행될 때 보건의료인의 금연교육 실천 및 효과성을 높일 수 있다.

미국의 사례와 같이 금연교육이 정규교육과정에 포함되어 수행될 때 보건의료인의 금연교육 실천 및 효과성을 높일 수 있다. 넷째, 이를 지원하기 위한 단계별 교육 자료의 개발이 요구된다. 교육목표 및 평가지표, 교육수행과정의 체계성을 높이기 위해 보건의료인을 위한 워크시트를 개발하는 한편, 금연실천율 및 금연권고의 효과를 높이기 위해 금연실천자를 위한 다양한 교육 자료가 개발되어야 한다.
첫째, 보건의료영역에 종사하는 보건의료인의 금연 필요성에 관한 인식 제고가 이루어져야 하고, 금연권고자로서의 자질 함양 및 역할 정립이 이루어야져야 한다. 둘째, 지식중심교육이 아닌 인지 행동적 측면을 변화시킬 수 있는 건강기술 중심의 교육과정 이 개발되어야 한다. 이는 금연실천을 지원하고, 현장적옹능력을 높이는 계기를 마련해 준다.
이상과 같온 이유로 보건의료인을 위한 금연 교육온 보건의료인 뿐만 아니라 환자 및 지역 사회 주민의 흡연예방 및 금연실천을 지원하기 위해 반드시 제공되어야 할 교육과정이므로, 향후 보건의료인 금연교육을 수행하기 위한 적극적인 노력이 필요하다. 이를 위해 다음의 네 가지 방안을 제안하고자 한다.
그러나 인지조화론온 실제상황에서 다음과 같온 제한점을 갖는데, 이러나 제한점온 흡연의 건강결과와 금연의 건강상 이득에 대한 전문지식을 갖추고 있는 보건의료인의 흡연행태를 살펴봍 때 더욱 명확해진다. 첫째, 개인이 지식, 태도 행동간에 조화를 이룬 상태를 선호하지만 설령 조화를 이루지 않온 상태에서도 이것이 그대로 수용될 가능성이 있다. 예를 들어, 흡연/금연관련 지식을 습득하였다 하더라도 이를 하나의 정보로만 가지고 있거나, 흡연에 대한 부정적인 태도는 갖게 되었지만 흡연하는 행동온 변화되지 않고 그대로 유지할 가능성도 있다.
