탈모에 대하여 종합적으로 분석한 연구는 이 등5), 이6), 김 등7), 장 등8)의 탈모에 관한 문헌적 고찰과 김 등9)의 탈모에 관한 임상적 고찰이 있으며, 임 등4)의 탈모의 동서의학적 고찰에 관한 연구가 있었으나 최신의 탈모 치료 경향에 대하여 체계적으로 분석한 논문은 없었다. 이에 저자는 탈모 치료와 관련된 최근의 연구 동향을 살펴보고 이를 토대로 한의학적 탈모 치료의 발전 방향을 모색하고자 연구를 진행하였다.
최근 미용에 대한 관심이 높아지면서 탈모 치료에 대한 요구가 증대됨에 따라 서양의학 및 한의학적으로 다양한 치료방법이 시도되고 있으나 탈모에 관하여 종합적으로 분석한 국내 연구로는 이 등5), 이6), 김 등7), 임 등4), 장 등8)의 탈모에 관한 문헌적 고찰이 있었으며 김 등9)의 탈모에 관한 임상적 고찰이 있을 뿐 탈모 치료의 최신 경향에 대하여 체계적으로 분석한 논문은 없었다. 이에 저자는 탈모 치료와 관련된 최근의 연구 동향을 살펴보고 이를 토대로 한의학적 탈모치료의 발전 방향을 모색하고자 하였다.

3. 전통의학정보 포털(OASIS, Oreintal Medicine Advanced Searching Integrated System) 검색
검색어는 탈모로 3편의 논문을 선별하였다(Table 3).
탈모에 관한 최신 연구 동향을 파악하기 위해 최근 5년간 해외 의학 관련 잡지 및 국내 한의학 관련 잡지에 수록된 탈모에 대한 논문을 검색하여 in vivo 연구와 임상증례보고를 제외한 탈모 단독 치료 논문을 대상으로 조사, 연구하였다.
후속연구
23편의 논문 중 기존의 치료방법으로 알려진 5α-환원효소 억제제(Finasteride, Dutasteride)나 Minoxidil, 스테로이드, 자가면역질환치료제에 관한 연구는 안전성과 다른 요법과의 병행 가능성에 관한 것이었으며, Diclofenac 병행요법, cyclosporine과 psoralen 처리 피부의 자외선 조사, Baricitinib, 레이저요법, 자가혈소판농축혈장치료술과 같은 새로운 치료법의 경우 부작용이 거의 나타나지 않았기 때문에 기존 치료법 대비 안전하다고 주장하고 있으나 광범 위한 연구가 행해지지 않아 안정성에 대해서는 추가적이며 광범위한 연구가 행해져야 할 것으로 보인다.
Minoxidil과 PRP의 경우 환부의 혈류 개선을 통한 발모 촉진 효과가 있는데 이러한 작용은 活血祛瘀와 유사하다고 볼 수 있으므로 기존 치료법 외 活血祛瘀를 새로운 치료법으로 고려해 볼 수 있다. 甘草酸, 養血生髮 캡슐과 같이 중의학에서 다용되는 처방을 응용하거나 피부침, 음악치료, 적외선조사요법과 같은 외치법을 병용하여 치료율을 높일 수 있을 것이다.
정신적인 영역을 치료의 범주에 포함시키는 한의학의 특성을 적극적으로 활용한다면 탈모 치료에 보다 나은 결과를 가져올 수 있을 것이라 생각된다. 또한 탈모에 대한 사상의학적 접근은 새로운 관점의 진단과 치료 방법을 제시할 수 있을 것으로 사료된다.
본 연구에서 언급된 서양의학과 중의학의 유용한 치료법들은 기존 탈모치료에 응용될 수 있을 것이며, 이와 관련하여 추가적인 연구가 필요할 것으로 사료된다.
이와 같이 탈모 치료에 대한 새로운 방법이 다양하게 제시되고 있으며, 이에 대한 한의학적인 관점에서의 접근이 필요하다. 상기 제시된 치료법들은 기존 한의학적 치료방법에 대한 보완 및 실질적인 근거 마련에 도움이 될 것이다. Minoxidil과 PRP의 경우 환부의 혈류 개선을 통한 발모 촉진 효과가 있는데 이러한 작용은 活血祛瘀와 유사하다고 볼 수 있으므로 기존 치료법 외 活血祛瘀를 새로운 치료법으로 고려해 볼 수 있다.
서양의학과 한의학에서 공통적으로 연구되고 있는 분야는 Excimer Laser, 저강도광선요법, 적외선조사 요법과 같은 광선요법과 롤러침, 칠성침과 같은 피부손상을 통한 재생 유도에 관한 것이다. 이와 같이 외치법에 대한 연구가 활발히 이루어지고 있으며 국내에서는 새로운 치료법으로 봉독59), 자하거60), 홍화60,61), 호도61) 등의 약침 시술과 매선치료62)가 제시되고 있으나 증례보고에 그쳐 지속적인 연구가 필요할 것으로 생각된다.
최근 질병의 원인에 대해 정신과적으로 접근하려는 시도가 계속되고 있다. 정신적인 영역을 치료의 범주에 포함시키는 한의학의 특성을 적극적으로 활용한다면 탈모 치료에 보다 나은 결과를 가져올 수 있을 것이라 생각된다. 또한 탈모에 대한 사상의학적 접근은 새로운 관점의 진단과 치료 방법을 제시할 수 있을 것으로 사료된다.
중의학에서 甘草酸과 養血生髮캡슐은 Finasteride나 Minoxidil과 같이 탈모환자에게 흔히 쓰이는 처방으로, 관련된 연구가 활발히 진행되고 있다. 탈모치료 효과를 보인 薏苓祛濕生髮湯, 祛濕健髮湯, 化脂生髮湯, 補髮飮 등의 한약처방이나 外治法에 대해서도 최대 효과를 낼 수 있는 용량 및 빈도와 안정성에 대한 연구가 추가적으로 필요할 것으로 생각된다.
