광범위 내성결핵의 문제점은 무엇인가? | 5% 정도가 초기치료에 실패하고 결핵균의 핵심 치료약제에 모두 내성이 생겨 다제 내성 결핵으로 이환되고 있는데 이는 약물효과가 감소한 상태에서 치료기간은 장기화되면서 치료순응도가 떨어져 강력한 내성의 슈퍼결핵균을 가진 광범위 내성결핵이 되는 것으로 이들 중 50% 이상이 3~7년 내에 사망할 수 있다[6]. 현재 광범위 내성결핵은 완치률이 현저히 낮아 세계적으로도 심각한 문제가 되고 있으며, 우리나라에서도 지속적으로 증가하는 추세이다[6]. 따라서 결핵의 완치를 위해서는 초기치료가 중요하지만 대부분의 경우 결핵균에 감염되어도 초기에는 증상이 두드러지게 나타나지 않아서 적절한 치료시기를 놓치는 경우가 많으며, 유병 기간이 긴 전염성 질환으로 발병 후 최소 6~9개월에서 2년 이상 장기간의 화학적 약물복용이 필수적임에도 완치에 이르지 못하고 중간에 실패하는 경우가 많다. | |
