목적: 호스피스 완화의료 병동에 입원한 말기 암환자가 통증을 호소할 때 진통제만 투여한 경우와 손 마사지를 병행했을 경우 통증감소의 효과를 비교한다. 방법: 단일군 시계열 설계 유사실험 연구로서 호스피스 완화의료병동에 입원한 25명의 환자를 대상으로 통증을 호소할 때 진통제를 투여하고 5분, 10분, 20분, 2시간 후에 각각 통증 점수를 측정하였고, 동일한 대상자가 다시 통증을 호소할 때, 진통제를 투여한 후 손 마사지를 병행하여 동일한 방식으로 통증점수를 측정하였다. 결과: 진통제만 투여한 군과 진통제를 투여한 후 손 마사지를 병행한 군간의 통증정도는 유의한 차이가 없었다. 그러나 두군 모두 시간이 흐름에 따라 통증정도가 유의하게 감소하였다. 결론: 손 마사지를 시행한 실험군이 대조군보다 통계적으로 유의하지는 않으나 통증정도가 낮은 경향이 있으므로 말기 암환자에게 손 마사지의 보완적 활용가치를 완전히 배제할 수는 없으며, 진통제 사용량에 따른 통증의 차이도 확인 되었으므로 간호사들은 말기 암환자를 위한 진통제 약물요법에 대하여 더 많은 교육과 연구가 요구된다.

Purpose: This study examined the effectiveness of a hand massage combined with analgesics on pain control in hospice patients with terminal cancer. Methods: This study is a quasi-experimental study with a single group time series design. The study included 25 terminal cancer patients who were admitted to a hospice ward. Each patient's pain level was measured after analgesics use only (control group). When patients complained of pain again, the pain level was assessed after administering a combination of hand massage and analgesics (experimental group). As for the experimental treatment, the participants were provided with oil hand massage on each hand for 5 minutes. Results: The experimental group and the control group showed no significant differences in the changes of pain score (F=0.74, P=0.3939). Conclusion: Although the pain level of the experimental group did not significantly improve compared with the control group, their pain levels tended to be low to begin with. Thus, a complementary utility value of hand massage cannot be completely excluded in terminal cancer patients. Since the pain level significantly changed according to the dosage of analgesic, nurses need more education and research on analgesic drug therapy for terminal cancer patients.

이에 본 연구는 말기 암환자가 통증을 호소할 때 진통제만 투여한 경우와 손 마사지를 병행하는 경우 통증 감소의 효과를 비교하고, 말기 암환자의 통증완화에 효과적인 간호중재 방안을 모색하고자 하였다.
가설 설정
본 연구의 가설은 통증을 호소하는 대상자에게 진통제와 손 마사지를 병행한 실험군은 진통제만 사용한 대조군보다 통증점수가 낮을 것이다.
제안 방법
대상자는 연구목적을 이해하고 연구 참여를 수락한 자이며, 입원 3일 후 급성기 통증이 어느 정도 조절된 환자로서 오일에 알레르기가 없는 자, 손에 병변이 없는 자, 질문지의 내용을 이해하고 언어적 소통이 가능한 자로 선정하였다. 본 연구의 중재는 호스피스 완화 병동에 입원한지 3일 이후 통증 호소 시 진통제만 투여한 군을 대조군으로 하고, 동일 환자가 다시 통증을 호소했을 때 진통제와 오일을 이용한 손 마사지를 5∼10분간 시행한 경우 실험군으로 구분하였다. 대상자 수는 G-power 3.
대상 데이터
본 연구의 중재는 호스피스 완화 병동에 입원한지 3일 이후 통증 호소 시 진통제만 투여한 군을 대조군으로 하고, 동일 환자가 다시 통증을 호소했을 때 진통제와 오일을 이용한 손 마사지를 5∼10분간 시행한 경우 실험군으로 구분하였다. 대상자 수는 G-power 3.1.2 program을 이용하여 t-test에서 유의수준 α=0.05, 그룹의 수=2, 효과크기 0.90 (large effect), 검정력 0.75로 산출한 결과 40명이었으나, 탈락률 20% (8명) 를 고려하여 실험군 25명, 대조군 25명을 선정하였다.
대상자는 K시 I종합병원 완화의료병동에 입원한 환자를 대상으로 2013년 8월부터 2014년 8월까지 자료를 수집하였다. 자료를 수집하기 전, 연구의 방법과 내용에 대하여 병원임상시험심사위원회(IRB)의 승인을 받았다.
데이터처리
자료 분석은 SPSS 12.0 통계프로그램을 이용하였으며, 일반적 특성은 빈도수와 백분율로 산출하였고, 실험군과 대조군의 사전 통증정도에 대한 동질성은 paired t-test로 검증하였으며, 가설 검정은 Linear Mixed Model을 적용하여 파악하였다.
성능/효과
본 연구의 결과 손 마사지가 통증감소에 유의한 효과는 없었지만 손 마사지는 아무런 도구 없이 단순히 수기로만 이루어질 수 있고, 통증 이외의 주관적 반응에도 긍정적인 효과가 있으므로 말기 암환자를 위한 보완 요법으로 활용할 수 있는 가치가 있다고 생각한다. 향후 급성기 종합병원 완화의료병동 임상현장에서 통증에 영향을 줄 수 있는 다른 사회적 변수까지 고려하여 대상자를 늘리고, 진통제 사용에 손 마사지를 병행함으로써 통증감소 효과뿐 아니라 불안이나 우울 등을 감소시키는지 반복 연구를 하는 것이 필요하다고 생각한다.
후속연구
본 연구는 연구설계와 대상자 선정과 같은 제한점은 있었으나 암성 통증을 완화시키기 위해는 진통제 투여가 기본적인 치료가 되고, 중요함을 확인할 수 있었다. 또한 암환자의 통증완화에 약물요법이 주로 사용되지만 실제 약물요법의 효과를 증명하는 연구가 없는 실정이므로 이에 대한 추후 연구의 필요성을 제언(9,20)한 바와 같이 호스피스 완화의료 병동에서 약물요법을 시행하는 간호사들에게 통증조절을 위한 다양한 약물요법에 대한 교육과 연구에 더욱 관심을 기울일 것을 제언한다.
암환자의 통증조절을 위한 중재연구를 메타분석한 Min과 Oh(21)는 암환자의 통증조절을 위하여 적용한 손 마사지의 효과가 있다고 보고하였으나, Jo 등(22)은 손 마사지를 포함한 통합적 완화돌봄 중재가 말기 암환자의 통증 완화에는 효과가 없는 것으로 나타났다고 보고하였으며, 이는 통증을 측정하는 도구의 다양성과 중재 기간 및 중재 후 통증을 측정하는 시기 등에 따라 차이가 있기 때문일 것이라고 하였다. 본 연구에서 실험군이 대조군보다 통증정도가 낮은 경향이 있었으므로 진통제와 손 마사지를 병용하는 것이 진통제만 투여하는 것 보다 통증 감소에 효과적인지를 확인하는 추후 연구가 필요하다고 생각한다.
