Objectives: This article aims to review clinical studies related to traditional Korean medicine (TKM) for cancer-related cognitive impairment (CRCI) up to date. Methods: EMBASE, Pubmed and eight Korean databases were searched for clinical studies about TKM for CRCI up to July 2016. We selected articles about subjects with cancer, using TKM interventions including acupuncture, moxibustion or herbal medicine, containing assessment about cognitive function. Results: Two randomized controlled trials (RCTs) and one randomized controlled feasibility study were included. Selected studies were containing assessments about cognitive function but not as primary outcomes. The three studies included one acupuncture study and two herbal-medicine studies. Only one herbal-medicine study showed improvement in cognitive function. Conclusions: Few clinical studies mainly focusing CRCI treated with TKM have been performed and have shown contradictory results until now. Future studies should be performed considering findings from pre-clinical and clinical studies altogether, and they should be controlled to minimize risk of bias thorough out overall courses of designing, conducting and reporting them.

대상 데이터

국내 논문의 검색은 NDSL (National Discovery for Science Leaders), OASIS (Oriental medicine Advanced Searching Integrated System), RISS (Research Information Service System), KoreanTK (Korean traditional knowledge portal), KISTI (Korean institute of science and technology information)를 이용하여 시행하였다. 국외 논문은 EMBASE와 Pubmed를 이용하여 검색하였다. 검색은 2016년 7월에 시행되었으며, 검색어는 “암”, “항암치료”, “인지 장애”, “인지 기능”, “침”, “뜸”, “한약”을 각 검색엔진에 적합하도록 조합하여 사용하였다.

3건의 연구는 각각 유방암³⁵⁾, 난소암³⁴⁾, 위암³³⁾의 단일 암종을 모집 대상으로 하였다. 암의 치료 내용을 살펴보면, 1건의 논문에서는 ‘6개월간 CT를 받은 이력이 없는 향후 CT가 예정되어 있는 환자’를 대상으로 하였고³⁴⁾, 2건의 논문은 암 치료의 종류를 제한하지 않았는데, 그 중 1건의 논문에서는 CT를 받은 환자가 66.

이론/모형

1건의 논문에서는 The Functional Assessment of Cancer Therapy-Cognitive Function (FACT-Cog)를³⁵⁾ 2차 평가 변수로 사용하였고, 2건의 논문에서는 삶의 질 평가를 위해 European Organization for Research and Treatment of Cancer Quality of Life Core Questionnaire C30 (EORTC-QLQ-C30)33,34)을사용하고, 인지 기능 영역(cognitive function subscale)을 포함한 세부 영역별 점수를 보고하였다(Table 1).

성능/효과

. 이 연구에서는 EORTC-QLQ-C30의 인지 영역에서 시험군의 유의한 호전을 보였다. 다른 2건의 연구는 각각 FACT-Cog와 EORTC-QLQ-C30의 인지 영역 상에 유의한 호전을 보이지 않았다(Table 1).
본 연구에서 역시 4기 위암의 진행을 억제하는 것을 목적으로 하고, 그 평가 지표를 삶의 질로 설정하였다. 비뚤림 위험이 있어 해석에 주의가 필요하나, 전체적인 삶의 질 및 인지 세부 항목에서 유의한 차이를 보였다. 구성 약재 중 천남성(Arisaemaamurense)과 반하(rhizome pinelliae preparatum)는 허혈성 손상에 대한 뇌세포 보호 효과가 알려져 있으며^40,41), 산자고(cremastrae pseudobulbus)는 항암 효과와 함께 조혈기능장애 및 신독성 등의 부작용이 주목 받고 있으므로^42,43), 향후 추가 연구를 통해 효과 및 안전성에 대해 상세히 살펴볼 필요가 있을 것으로 사료된다.

후속연구

먼저 연구 대상자를 살펴보면, 유방암과 난소암을 대상으로 한 2건의 연구에서는 폐경 여부, 호르몬치료 여부 등 인지 능력에 영향을 미치는 요인들에 대한 보고가 이루어지지 않아 인지 장애의 요인 및 치료 효과에 대한 포괄적인 해석에 한계가 있었다. 암 치료의 종류 및 그와 관련된 임상 연구 중재의 시점 역시 중요한 변수인데, 이에 대한 제한이 3건의 연구에서 모두 달랐다.
1건의 연구는 암 치료의 종료 후 기력회복을 목표로 시행하였고³⁵⁾, 2건의 연구는 치료 과정 중에 임상연구용 중재를 병행하며 암의 진행을 막거나³³⁾ CT의 독성을 최소화하는 것을³⁴⁾ 목표로 하였다. 따라서 향후 연구에서는 목표하는 치료 기전에 따라 암 치료의 내용 및 그 시점을 상세히 고려하는 것이 필요하다. 나아가 모든 암 치료가 종료된 환자를 대상으로 하더라도 진단 및 치료의 시점으로부터 임상연구 시작까지의 기간, 치료의 종류 등은 CRCI에 영향을 미치는 주요한 요인이므로, 관련 정보를 함께 보고하는 것이 필요할 것이다.
따라서 향후 연구에서는 목표하는 치료 기전에 따라 암 치료의 내용 및 그 시점을 상세히 고려하는 것이 필요하다. 나아가 모든 암 치료가 종료된 환자를 대상으로 하더라도 진단 및 치료의 시점으로부터 임상연구 시작까지의 기간, 치료의 종류 등은 CRCI에 영향을 미치는 주요한 요인이므로, 관련 정보를 함께 보고하는 것이 필요할 것이다.
Chan등³⁴⁾은 CT의 독성을 최소화 하기 위해 化痰, 制濕, 消散하는 약물을 위주로 사용하여 면역 기능의 향상 효과를 보고 하였다³⁸⁾. 그러나 삶의 질 설문 상에는 호전을 보이지 않았기 때문에 향후 치료 목표에 따른 적합한 평가 도구에 대한 고려가 필요할 것으로 보인다. 또한 해당 연구에서 CT 중에 대조군에서 전반적인 삶의 질이 증가하였고, 임상 연구용 한약의 용량은 연구자가 임의로 결정하였다는 언급이 있어, 위약 및 치료약의 적절성에 대해 확신하기 어려우며 향후 보완된 연구가 필요한 상태이다.

살펴본 바와 같이 침과 한약치료의 인지 향상 효과를 근거로 CRCI에 적용해 볼 수 있으나, 다른 인지 장애에 비해 안전성이 중요하게 다루어져야 할 것이다. 특히 CT와 한약 투여가 병행된 연구에서 치료의 중간 시점에 오히려 인지 기능의 유의한 악화를 보였고³⁴⁾, 이에 대해 해당 저자들은 시험약에 포함된 복령(Poria cocos)에 의해 IL-6가 증가되어 이러한 결과를 야기한 것으로 해석하였다^34,48,49).
Avisar 등⁴⁶⁾은 침치료는 항암화학요법을 비롯한 일반 치료용 약물과 상호작용을 일으키지 않아 치료의 안정성을 해치지 않을 것으로 보이지만 한약의 경우에는 약물 상호작용의 가능성이 있어 이상 반응과도 관련이 있을 것으로 판단하였다. 향후 안전하고 효과적인 한방치료의 제공을 위해서는 CRCI에 관련된 안전성, 신경학적 기전⁴⁷⁾ 및 임상적으로 관찰된 효과³⁷⁾를 함께 고려할 필요가 있다.
또한 최근에 연구되기 시작한 분야로서 안전성에 대한 정보가 포함된 연구가 부족하여, 본 논문들만으로 안전성을 단정하기 어려운 상태이다. 그러나 본 연구는 암 이후의 삶에 대한 관심이 증가하며 적절한 의학적 관리가 요구되는 상황에서, CRCI에 대한 한방 치료 연구의 현 상태를 확인하고 향후 연구 계획을 수립하기 위한 근거가 될 수 있으리라 사료된다.
그러나 암 환자의 인지 상태가 독립적인 영역으로 주목 받기 시작하였으며, 한약 치료를 통해 인지 기능의 향상을 보인 연구와 일시적 저하를 보인 연구 등 상반된 결과가 보고되어 있는 상태이다. 한의학적 치료의 가능성과 그 기전에 대한 연구가 이루어져 있으므로, 암과 관련되지 않은 인지 장애의 치료법 및 암과 관련된 기전으로 인한 뇌세포 손상을 보호하는 방법에 근거한 전임상 및 임상 연구가 수행되어야 할 것이다. 현재는 연구의 양과 질이 부족한 상태이므로 향후 적절한 표본수를 가진 체계적인 임상연구의 수행 및 투명한 보고를 통해 안전성과 효과에 대한 근거 구축이 필요하다.
한의학적 치료의 가능성과 그 기전에 대한 연구가 이루어져 있으므로, 암과 관련되지 않은 인지 장애의 치료법 및 암과 관련된 기전으로 인한 뇌세포 손상을 보호하는 방법에 근거한 전임상 및 임상 연구가 수행되어야 할 것이다. 현재는 연구의 양과 질이 부족한 상태이므로 향후 적절한 표본수를 가진 체계적인 임상연구의 수행 및 투명한 보고를 통해 안전성과 효과에 대한 근거 구축이 필요하다. 새롭게 주목 받고 있는 분야인 암 환자의 인지 장애 증상에 대한 한방 치료의 향후 연구 및 임상적 활용에 본 문헌 고찰이 기초 자료로서 활용되길 기대한다.
