Purpose: The purpose of this study was to examine the effect of preoperative education using multimedia on level of pain, uncertainty, anxiety and depression in hysterectomy patients. Methods: A non-equivalent control group, with a pretest-posttest design was used in this study. The level of post operation pain, pre and post operation uncertainty, anxiety and depression of both the groups was measured. The experimental group was provided with preoperative education using multimedia on the preoperative day at the hospital. The control group was only given usual care. Data were analyzed using SPSS/WIN version 22.0 program. Results: Experimental group showed lower level of post-operation pain in 1 hour (t=-5.08, p<.001) and 24 hours (t=-5.20, p<.001) but not 48 hours (t=-0.91, p=.368). Uncertainty showed significant interaction effect of Group by Time (F=4.16, p=.018). Conclusion: Preoperative education using multimedia for patients with hysterectomy would be effective in lowering patients' level of pain, uncertainty, anxiety and depression.

제안 방법

교육 내용은 여성 생식기에 대한 이해, 자궁절제술의 적응증, 자궁절제술의 종류, 수술 후 기대되는 결과, 병동 안내, 수술 전 준비, 수술실 이동, 수술 후 간호 및 통증관리, 수술 후 합병증 및 예방법, 퇴원 후 주의 사항, 퇴원 후 신체 회복을 돕는 운동 및 식이섭취, 폐경증상과 관리, 성생활 복귀와 적응방안으로 구성하였고 수술 후 호흡기, 순환기 합병증 예방법 및 PCA 사용법은 직접 시범을 보이고 실습할 수 있도록 하였다. 교육내용은 의료계에 종사하고 있지 않은 일반인 2명에게 확인 후 어려운 의학용어나 이해되지 않는 부분을 수정하여 완성된 내용을 녹음하였고 교육내용을 바탕으로 사진, 동영상 촬영 및 파워포인트를 이용하여 프리젠테이션 자료를 제작하였다.
수술 후 1일째의 불안은 입원시보다 낮았다. 따라서 사전 조사는 교육 전인 입원일에 실시하였고 사후 조사는 교육 후 2차에 걸쳐 실시하였다. 1차 사후 조사 측정시기는 수술 전인 수술당일 아침 7AM으로 하였고 2차 사후 조사는 수술 후 2일째에 시행하였다.
본 연구에서 개발된 교육 동영상은 개복 및 복강경하 자궁절제술 환자를 위해 개발된 교육 자료이고 두 가지 수술 각각의 치료 및 관리로 내용을 구성하였다. 연구결과, 자궁절제술 전 동영상 교육이 수술 후 1시간, 24시간의 통증을 경감시키고 조사시점별 불확실성 및 불안과 우울을 감소시키는데 효과적이므로 자궁절제술 환자에게 적극적으로 활용할 수 있다는 것에 의의가 있다.

대상 데이터

표본수의 크기에 필요한 효과크기 추정은 선행연구[19]를 토대로 하였다. Power analysis법에 의해 유의수준(⍺) .05, 검정력(1-β) .80, 효과의 크기(d) .50을 기준으로 G*Power Analysis로 계산하여 실험군 26명, 대조군 26명이 산출되었으며 실험군, 대조군 각각 탈락률 20%를 고려한 30명의 자료를 수집하였다. 본 연구는 연구참여자의 윤리적 보호를 위해 생명윤리위원회의 심의를 거쳐 승인 후(IRB 승인번호: BD 2013-154) 시행하였다.
연구대상은 경기도 소재 C종합병원 산부인과 병동에 양성 자궁질환으로 자궁절제술을 위해 입원한 환자이다. 대상자 선정기준은 폐경 전 양성자궁질환으로 난소적출을 제외한 단순 자궁절제술을 목적으로 병원에 입원하여 수술 후 PCA를 사용하기로 동의한 자로 하였고, 수술 후 합병증이 발생하여 입원 기간이 연장되었거나 치료계획이 변경된 된 자, 악성자궁질환으로 판명된 자는 제외하였다.

데이터처리

0 프로그램을 이용하여 분석하였다. 대상자의 일반적 특성은 빈도분석 및 기술통계로 분석하였고 실험군과 대조군의 동실성 검증은 x² test, Fisher's exact test, Independent t-test으로 분석하였다. 불확실성, 불안-우울의 효과 분석 및 통증 정도는 Paired t-test, repeated measures ANOVA로 분석하였다.

이론/모형

본 연구에서는 Mishel[8]이 개발한 불확실성척도를 Chung 등[24]이 번안한 도구를 사용하였다. 본 도구는 총 33문항, 5점 Likert 척도로 점수 계산 시 15번 문항은 제외하므로 총점은 최저 32점, 최고 160점의 범위를 가진다.
본 연구에서는 Zigmond와 Snaith[25]가 개발한 병원 불안-우울척도를 Oh 등[26]이 번안한 도구를 사용하였다. 본 도구는 총 14개 문항, 4점 Likert 척도로 총점은 최저 0점, 최고 21점의 범위를 가진다.

성능/효과

본 연구는 동영상을 이용한 수술 전 교육을 자궁절제술 환자에게 적용하여 수술 후 통증 및 수술 전, 후 불확실성, 불안과 우울에 미치는 효과를 알아보기 위하여 시행된 연구이다. 자궁절제술 전 동영상을 이용한 교육을 받은 대상자는 동영상 교육을 받지 않은 대상자보다 수술 후 1시간, 24시간의 통증 정도가 낮았고 조사시점에 따른 불확실성, 불안과 우울의 감소에 효과가 있었으므로 실무에서 자궁절제술 전 교육자료로 활용 할 수 있을 것으로 생각된다.

후속연구

본 연구의 결과를 근거로 환자의 요구와 상황에 적절한 다양한 교육 매체를 개발, 활용할 것을 제언한다. 또한, 자궁절제술교육 동영상의 효과를 일반화하기 위해서는 수술 방법 별 대상자 수를 확대한 반복 연구가 필요할 것이며 자궁절제술 환자의 불확실성이 가장 높은 시기에 대한 연구 및 수술 후 회복기간인 6주 후의 불확실성 및 불안과 우울에 효과적이 교육에 대한 추가 연구가 필요할 것이다.
