Purpose: The purpose of this study was to examine the effects of hand massage on depression, self-esteem and vitality in elderly patients in convalescent hospitals. Methods: This study employed a nonequivalent control group pretest-posttest design. The subjects were 24 elderly patients in the experimental group and 25 in a control group in a convalescent hospital located in G city. Data were collected using a questionnaire that relied on the geriatric depression scale (short form) by Sheikh and Yesavage; the self-esteem scale by Rosenberg; and the vitality scale used by Johnson and revised by Oh. The experimental group were treated with a general hand massage using only carrier oil on both hands for 10 minutes a day for seven days. The control group was given no treatment. The collected data were analyzed using t-test, $x^2$-test, and Fisher's exact test with SPSS Win Version 18.0. Results: The results are as follows: The experimental group showed a significant improvement in depression, self-esteem and vitality than the control group. Conclusion: The results showed that hand massage was an effective intervention program for depression, self-esteem and vitality of elderly patients in convalescent hospitals.

대상 데이터

본 연구의 대상자는 충청남도 G군에 소재하는 P요양병원에 입원중인 만 65세 이상의 노인으로 한국형 간이정신상태검사(Mini Mental State Examination-Korea, MMSE-K) 점수가 18점 이상인 자, 항우울제를 복용하지 않는 자, 수면제를 복용하지 않는 자, 손에 병변이 없는 자로서 설문지 응답을 위해 일상적인 의사소통이 가능한 자로 하였다.

본 연구에서는 Rosenberg (1973)가 개발한 자아존중감 척도를 Jeon (1974)이 번역한 도구를 사용하였다. 총 10문항 4점 척도로 긍정문항 5문항과 부정문항 5문항으로 부정문항은 역환산하였으며 ‘전혀 아니다’ 1점에서 ‘항상 그렇다’ 4점으로 점수의 범위는 10-40점이며 점수가 높을수록 자아존중감 정도가 높은 것을 의미한다.
본 연구에서는 Sheilkh와 Yesavage (1986)가 개발한 노인우울척도(Geriatric Depression Scale-Short Form, GDS-Short Form)를 Kee (1996)가 번역한 한국형 15문항 노인우울척도(Short Form Geriatric Depression Scale, S-GDS)를 사용하였다. 총 15문항으로 구성되어 있으며, 부정적으로 기술된 문항은 역 환산하였다.

성능/효과

마지막으로, 요양병원 입원 노인의 생기 정도는 점수의 범위가 20-60점인 도구에서 평균 32.6점으로 나타나 Oh (2002)의 연구보다 낮은 수준인 중간 이하의 점수를 보여주었고, 손마사지를 받은 실험군은 받지 않은 대조군에 비하여 생기가 의미 있게 증가되어 손 마사지가 요양병원 입원노인의 생기 증진에 효과적인 간호중재임을 확인할 수 있었다. 이러한 결과는 손 마사지를 통한 신체 접촉이 피부와 근육의 혈액순환과 신진대사를 증가시켜 신체조직 중 특히 근육이나 신경계에 새로운 활력을 가해준 결과로, 손 마사지를 통한 새로운 자극이 주위의 집중을 일으켜 각성이 나타나고 이 각성이 정서적 쾌감을 유발시킨 결과로 해석할 수 있다(Oh, 2002).

후속연구

본 연구결과를 토대로, 손 마사지는 요양병원 입원노인의 우울을 감소시키고 자아존중감 및 생기를 개선하는데 효과가 있는 간호중재법임을 확인하였으므로, 간호사는 요양병원 입원노인의 부정적 심리상태를 향상시킬 수 있는 손 마사지 기술을 습득하여 활용하는 능력과 태도가 필요하다. 또한, 노인은 폭 넓은 간호중재법을 적용하는데 있어서 신체능력 저하와 감각기계 저하 등으로 적지 않은 위험을 가진 대상자이며, 그 중 요양병원 입원노인은 질병으로 인해 더 많은 적용의 한계를 가지고 있다.
