Intravenous alfaxalone provided smooth induction for inhalation anesthesia in almost cases, but transient apnea and twitching/paddling were observed after induction and during recovery, respectively. 2차 진료기관인 서울대학교 동물병원에서는 복합질환이 있는 환자가 내원하게 되며, 높은 마취위험도를 가진 환자의 시술이 이루어진다. 이번 연구의 목적은 높은 마취위험도를 가진 환자에서 alfaxalone의 품종, ASA 단계, 병용 진정제 및 진통제의 조합에 따른 마취유도제로서의 임상적 유용성을 평가하는 것이었다.
제안 방법
Alfaxalone을 개에서 2 mg/kg, 고양이에서 5 mg/kg의 정맥투여 후 환자의 안검반사, 눈동자 위치 및 턱근긴장도를 통하여 기관내 튜브의 장착을 시도하였다. 환자의 연하반사, 턱근긴장도의 유지로 삽관이 불가능한 경우 alfaxalone 1 mg/kg를 추가 투여하였다.
환자의 ASA단계(4)는 환자의 병력, 기저 질환, 내원 시 실시한 신체검사 결과와 수술 전 검사 결과를 고려하여 3명의 마취의가 평가하였다(Table 1). 각 환자는 개별적으로 평가하여 수술 전 안정화를 위하여 수액 및 투약이 필요 시 처치하였다.
마취 중 환자 감시는 환자감시장치(Datex-Ohmeda S/5®, GE healthcare, Helsinki, Finland)를 이용하여 실시하였으며, 마취제의 최소폐포농도, 심전도, 직접 또는 간접동맥혈압, 산소포화도, 호기말 이산화탄소분압을 감시하였다.
마취유도 전 5분 동안 4 L/min의 산소를 공급하였으며, 마취의 유도는 개에서 alfaxalone (Alfaxan®, Jurox, Rutherford, Australia) 2-3 mg/kg, 고양이에서 5 mg/kg을 천천히 (60초 이상) 정맥 내로 주사하여 실시하였다.
각 환자는 마취 12시간 전부터 음식 섭취를 제한하였다. 마취유도에 앞서 환자에 결정질 용액, 진정제 및 진통제를 환자의 상태 및 예상되는 통증 정도에 따라 투여하였다. 진정제로는 acepromazine (세다젝트®, 삼우메디안, 예산, 대한민국), diazepam (디아제팜®, 삼진제약, 서울, 대한민국), midazolam (미다졸람®, 부광약품, 서울, 대한민국) 중의 선택된 약제를 정맥 투여하였으며, 진통제로는 hydromorphone (딜리드®, 하나제약, 화성, 대한민국), fentanyl (펜타닐®, 하나제약, 화성, 대한민국), butorphanol (부토판주사®, 명문제약, 서울, 대한민국), tramadol (트라마돌염산염®, 한올바이오파마, 대전, 대한민국)중의 선택된 약제를 투여하였다.
환자의 연하반사, 턱근긴장도의 유지로 삽관이 불가능한 경우 alfaxalone 1 mg/kg를 추가 투여하였다. 마취유도의 평가는 삽관의 용이성과마취제의 주입 시 통증 반응의 유무를 기준으로 평가하였다(Table 2) (10,11).

Alfaxalone의 마취유도 평가 시 정맥 내 투여한 대부분의 환자(97.3%)에서 부드러운 마취유도를 보였으나, 4증례(2.7%)에서 기침, 부분적인 근육 단일수축 및 사지강직 등의 흥분 증상을 관찰할 수 있었다(Table 2). 모든 증례에서 alfaxalone의 정맥 내 주입 시 통증 반응은 없었다(Table 2).
환자의 시술법과 기대되는 통증에 따라 다양한 진정제와 진통제가 병용투여 되었다(Table 6). Midazolam과 hydromorphone의 조합이 59례(39.3%)로 가장 높은 빈도를 보였으며, acepromazine과 hydromorphone이 22례(14.7%), midazolam과 fentanyl이 14례(9.3%)의 빈도를 보였다.

환자의 평균연령은 8.4 ± 4.0 년, 평균 체중은 7.1 ± 5.8 kg 였으며, 환자의 ASA단계는 ASA 3단계가 86두(57.3%)로 가장 높은 빈도를 보였으며, ASA 2단계는 52두(34.7%), ASA 4단계는 9두(6.0%)의 빈도를 보였다(Table 4).
