Purpose: The aim of this study was to develop and measure the effect of a movie-based-nursing intervention program designed to enhance motivation for rehabilitation and reduce depression levels in stroke patients. Methods: The study used a quasi-experimental, nonequivalent control group and a pretest-posttest design. The 60 research subjects were assigned to the experimental (n=30) or control group (n=30). The movie-based nursing intervention program was provided for the experimental group during 60-minute sessions held once per week for 10 weeks. The program consisted of patient education to strengthen motivation for rehabilitation and reduce depression, watching movies to identify role models, and group discussion to facilitate therapeutic interaction. Results: After 10 weeks of participation in the movie-based nursing intervention program, the experimental group's rehabilitation motivation score was significantly higher, F=1161.54 (within groups df=49, between groups df=1), p<.001, relative to that observed in the control group. In addition, the experimental group's depression score was significantly lower relative to that observed in the control group, F=258.97 (within groups df=49, between groups df=1), p<.001. Conclusion: The movie-based nursing intervention program could be used for stroke patients experiencing psychological difficulties including reduced motivation for rehabilitation and increased depression during the rehabilitation process.

제안 방법

영화기반 간호중재 프로그램의 개발은 2013년 7월부터 2014년 6월 첫째 주까지 이루어졌으며 환자관찰 및 문헌고찰, 의료진 면담, 추가 문헌고찰, 연구자의 영화치료 전문강사 자격취득, 프로그램의 초안 작성, 프로그램의 적용가능성 확인, 전문가 자문 및 프로그램 수정-보완 순으로 진행되었다.
대상자의 일반적 특성은 인구사회학적 특성과 질병관련 특성으로 구분하고 질문지를 통해 조사하였다. 인구사회학적 특성은 연령, 성별, 결혼상태, 학력 등의 내용으로 구성되었고, 질병관련 특성은 뇌졸중 발병시기, 유형, 발병횟수, 마비 부위, 일상생활동작 수행능력, 주 돌봄 제공자, 치료비 부담을 묻는 내용으로 구성하였다.

대상 데이터

80을 기준으로 [20,21] 두 개의 평균을 비교시 필요한 표본수를 산출한 결과 각 그룹 당 최소 21명이 요구되었다. 따라서 탈락율을 고려하여 실험군과 대조군을 각각 30명씩 모집하였고 이 중 실험군 6명, 대조군 3명이 중도 탈락하여 최종 분석에 포함된 연구대상자는 10주간 영화기반간호중재 프로그램에 모두 참석한 실험군 24명, 대조군 27명이었다.
본 연구는 경기도에 위치한 A병원과 B병원에 재활치료를 위해 입원중인 뇌졸중 환자를 대상으로 시행되었다. A병원과 B병원은 같은 재단에서 운영하는 노인전문병원으로 재활시설의 규모와 질, 재활치료 운영체계, 의료진의 수 등 재활과 관련된 치료적 환경이 유사한 곳이다.

이론/모형

뇌졸중 환자의 우울을 측정하기 위한 도구는 Yesavage 등[24]이 개발하고 Cho 등[25]이 번안한 한국판 노인우울척도 단축형 도구 (Geriatric Depression Scale Short Form Korea version [GDSSFK])를 개발자와 번안자의 승인을 받은 후 사용하였다. 본 도구는 15개 문항으로 ‘예’ 1점, ‘아니오’ 0점으로 채점하며 역문항은 역환산하여 채점한다.
뇌졸중 환자의 재활동기를 측정하기 위한 도구는 Han과 Lim [9]이 개발한 장애인의 재활동기 측정도구를 개발자의 승인을 받은 후 사용하였다. 본 도구는 자기결정 정도에 따라 과제지향적 동기, 변화 지향적 동기, 의무적 동기, 외부적 동기, 무동기 등 5개 하위영역으로 구분되고 총 27문항으로 구성되어 있다.

성능/효과

본 연구는 뇌졸중 환자의 재활동기 강화 및 우울 감소를 위한 영화기반 간호중재 프로그램을 개발하고 그 효과를 검정하기 위해 시행되었으며 연구결과, 영화기반 간호중재 프로그램은 뇌졸중 환자의 재활동기 강화와 우울 감소에 효과가 있는 간호중재로 확인되었다.
장기간의 재활치료가 요구되는 뇌졸중 환자에게 있어서 재활동기는 재활의 예후에 영향을 미치는 매우 중요한 심리적 요인이다[5,7-9]. 본 연구에 참여한 뇌졸중 환자의 재활동기 평균점수는 실험군 55.33±5.67점, 대조군 57.78±5.99점으로 도구의 점수범위 27~108점을 기준으로 볼 때 비교적 낮은 수준이었으며 동일한 도구를 사용하여 65세 이상 재가 편마비 뇌졸중 노인의 재활동기 점수를 측정한 Jeon [23]의 연구의 재활동기 점수와 비교했을 때 더 낮은 것으로 나타났다.
본 연구에서 뇌졸중 환자에게 주 1회, 60분씩, 10주간 환자교육, 관련영화감상, 집단토의로 구성된 영화기반 간호중재 프로그램을 적용한 결과 재활동기 증진에 유의한 효과가 있었고, 뇌졸중 환자의 재활동기에 대한 중재의 효과크기(effect size)는 3.40으로 Cohen의 기준[20]으로 볼 때 0.80이상이므로 큰 효과(large effect)가 있는 것으로 나타났다. 이는 재활병원에 입원하여 재활치료중인 뇌졸중 편마비 환자에게 주 5회, 10분씩 4주간 집단체조 프로그램을 적용한 Yeo와 Kim [26]의 연구의 효과크기 1.

후속연구

그리고본 연구에서는 선행연구[12]에서 밝혀진 뇌졸중 환자의 우울방어요인인 장애수용과 우울위험요인인 스트레스 및 재발에 대한 불안을 프로그램 구성에 반영한 결과 뇌졸중 환자의 우울을 감소시키는 효과가 있음을 확인하였으나 각각의 요인을 직접 측정하지 않아 개별 요인이 우울 감소에 어떠한 영향을 미치는지에 대해서는 확인하지 못하였다. 따라서 추후연구에서는 장애수용, 스트레스, 불안을 종속 변수로 포함시켜 각 요인이 뇌졸중 환자의 우울 감소에 어떠한 영향을 미치는지 구체적으로 확인할 것을 제언한다.
