Purpose: This study aimed to identify variables influencing the health-related quality of life (HRQoL) of adults with epilepsy in order to establish a structural model and design an intervention strategy to improve patients' HRQoL. Methods: The selected subjects were 212 patients with epilepsy aged between 18 and 70 years who were currently receiving treatment from hospital, general hospital, and clinic. They were surveyed using a structured questionnaire. Results: The goodness of fit measures of the final hypothetical model were as follows: ${\chi}^2/df=2.51$, GFI=.91, AGFI=.90, CFI=.96, SRMR=.04, NFI=.93, and RMSEA=.08. The major variables influencing the HRQoL of adults with epilepsy were epilepsy self-efficacy, depression, social support, and side effects of anti-epileptic drugs (AEDs), which were significant in the mentioned order, whereas the duration of AEDs use and perceived stigma did not show any effects. Six variables accounted for 75.6% of HRQoL. Variables having a direct and total effect on the HRQoL of adults with epilepsy were the side effects of AEDs, social support, epilepsy self-efficacy, and depression, and those with an indirect effect were the side effects of AEDs and social support. Conclusion: It is necessary to accurately identify the side effects of AEDs in adults with epilepsy and accurately observe the physical changes caused by depression. In addition, it is imperative to establish an active and effective nursing intervention program to strengthen the self-efficacy of the patients and to improve their quality of life through social support provided by family members and medical professionals.

제안 방법
본 연구는 뇌전증 환자의 삶의 질에 영향을 미치는 각 요인들 간의 상호 인과적 관계와 상대적 영향 정도를 규명하기 위해 선행 문헌의 고찰과 관련 이론을 기반으로 개념적 모형을 구축하였다. 뇌전증 환자는 발작조절을 위해 평생 한 두 개 이상의 약물을 꾸준히 복용하며 정상적인 삶을 유지하는 것이 치료의 최대 목표이지만, 장기간의 약물 복용기간과 복합요법에 따르는 약물 관련요인은 주의력이나 집중력 결핍, 정신서행(mental slowing), 조화운동불능, 우울감, 어지럼증, 졸림, 복시 등의 여러 부작용을 노출시키고, 정서적 안녕감이나 인지적 기능 및 사회적 기능 저하 등을 초래하여 전반적인 삶의 질 저하를 가져온다[3].
대상 데이터
뇌전증 이외에 신경외과, 신경과, 정신건강의학과적 질환으로 다른 약을 복용하는 경우에는 약물의 영향 및 삶의 질과 관련된 일상생활의 제한에 영향을 미칠 수 있다는 뇌전증 전문가의 자문에 따라 제외기준에 포함시켰다. 구조방정식 모델검증에 필요한 표본의 크기는 200여개를 임계치로 제시하고 있어[22] 본 연구에서는 212명을 편의표집 하였다.
본 연구는 J시에 소재하는 일개 상급 종합병원과 종합병원, 정신건강의학과 의원에서 각각 약물치료를 받고 있는 뇌전증 환자를 대상으로 하였으며, 선정기준은 만 18세 이상 70세 이하의 성인 환자, 연구 참여 최소 1년 전에 신경외과 및 신경과 정신건강의학과 전문의로부터 뇌전증 진단을 받고 항뇌전증 약물을 복용 중인 자, 의사소통이 가능하며 연구의 목적을 이해하고 연구 참여를 허락한 자로 하였다. 대상자의 제외기준은 뇌전증 이외의 신경외과, 신경과, 정신건강의학과적 질환으로 약물을 복용 중인 자, 뇌전증 관련 수술을 받은 지 5년이 경과되지 않은 자, 삶의 질에 영향을 미칠 수 있는 심부전, 신부전, 암 등의 만성질환이 동반된 자로 하였다.

본 연구에서 뇌전증 자기효능감은 Bandura [20]의 사회인지이론과 Robinson 등[16]이 수정한 뇌전증의 스트레스 관리 이론을 토대로 그 연관성을 개념화하였다. 사회인지이론의 구성요소에서 상호결정주의는 개인적 요인과 환경적 요인, 행동적 요인 간에 서로 긴밀한 영향을 주고 받으며 역동적인 상호 작용을 하는데, 환경과 상황은 개인의 행태에 영향을 주는 지지체계의 일부로서 사회 환경을 포함하고 있다[20].
사회적 지지는 Cohen과 Hoberman [19]이 개발한 대인관계 지지 평가 척도(Interpersonal support evaluation list [ISEL])를 Kim과 Lee [27]가 번역한 도구로서, 저자로부터 승인을 받은 후 사용하였다. 본 도구는 전체 12문항으로 가족지지 7문항, 의료인 지지 5문항이며 2개의 하위요인으로 구성되어 있다.

본 연구는 뇌전증 환자의 삶의 질에 영향을 미치는 변인들 간의 경로와 영향력을 파악하여 삶의 질 향상을 위한 중재전략을 마련하고자 시도되었다. 뇌전증 환자의 약물 부작용은 뇌전증 자기 효능감을 통해 삶의 질에 미치는 부정적 영향을 상쇄하였으며, 뇌전증 환자의 우울은 직접적으로 삶의 질을 저하시키면서 항뇌전증 약물 부작용의 정도에 따라 삶의 질을 더욱 악화시킬 수 있는 요인으로 작용하였다. 사회적 지지는 뇌전증 자기효능감을 통하여 삶의 질이 더욱 증진될 수 있는 촉진적 자원이 되고, 우울을 감소시켜 삶의 질에 긍정적인 영향을 미치는 간접효과가 있음을 확인하였다.
본 연구에서 삶의 질에 대한 항뇌전증 약물 복용기간, 항뇌전증 약물 부작용, 사회적 지지, 뇌전증 자기효능감, 우울, 낙인인지의 총 설명력은 75.6%로 비교적 높게 나타났다. 이는 본 연구의 가설적 모형에서 선정된 6개의 변수들이 삶의 질을 설명하기에 적절한 것으로 판단되며, 뇌전증 환자의 삶의 질을 증진시키기 위한 방안과 중재 프로그램 등을 모색하고자 할 때 이와 같은 변수들이 먼저 고려되어야 할 것으로 사료된다.
