본 연구는 건강신념모형을 적용하여 임상에서 근무하는 간호사의 손씻기 수행도를 분석하기 위한 서술적 조사연구이다. 본 연구의 대상은 G 광역시에 위치한 일개 병원에 근무하는 임상 간호사 162명이다. 자료수집은 2017년 9월 1일부터 2017년 10월 31일까지였다. 수집된 자료는 SPSS (SPSS Version 20.0, IBM, US)를 이용하여 빈도분석, 백분율, 일원분산분석, t-검정, pearson 상관계수분석 및 다중회귀분석으로 통계처리 하였다. 일반적 특성에 따른 손씻기에 대한 수행도는 모든 영역에서 평균은 3.1점으로 높았으나 통계적으로 유의한 차이는 보이지 않았다. 일반적 특성에 따른 손씻기에 대한 건강신념은 연령에서는 지각된 유익성 영역에서 유의한 차이를 보였으며(p<0.05), 근무연수에서는 지각된 유익성, 지각된 장애성 영역에서 유의한 차이를 보였다(p<0.05). 다섯 개의 하위영역간의 상관관계를 분석한 결과는 지각된 유익성과 지각된 장애성은 부의 상관관계를 나타냈으며, 수행을 위한 동기는 정의 상관관계를 보였다. 본 연구의 결과를 통해 손씻기에 대한 건강신념을 고려한 지속적인 병원에서의 감염관리교육과 올바른 손씻기 교육을 진행한다면 손씻기 수행도가 높아질 것으로 판단된다.

This study is a descriptive study to analyze the hand washing practice of nurses working in clinic applying health belief model. This research involved 162 clinical nurse at G city. The data were collected from Sep 1, 2017 to Oct 31, 2017. The collected data were analyzed using an independent t-test, 1-way ANOVA, pearson's correlation coefficient and multiple regression analysis, where p-values of <0.05 were considered statistically significant by using SPSS 20.0. The overall practice of hand washing by general subjects were high 3.1 in all subjects, but there was no statistically significant difference. Health beliefs about hand washing practice according to general characteristics showed significant difference in perceived benefits (p<0.05), and there was a significant difference in perceived benefits and perceived barriers in age(p<0.05). In regard to the correlation among the subfactor of health beliefs, benefits had a statistically significant negative correlation to barriers and, positively correlated to cues to action. The results of this study suggest that continuing education of infection management in hospitals considering health beliefs about proper hand washing training will enhance hand washing practice.

실질적으로 손씻기 수행도에 영향을 미치는 요인을 이론적으로는 설명하기가 어렵기에 본 연구에서는 병원감염 예방을 위한 체계적이고 과학적 이론 중의 하나인 건강신념모형을 적용하여 임상에서 근무하는 간호사의 손씻기 수행도에 영향을 미치는 요인을 분석하고자 한다.

본 연구에 사용된 조사항목은 연구대상자의 일반적 특성과 손씻기에 대한 건강신념, 손씻기 수행도에 관한 항목이었다. 일반적 특성으로는 성별, 연령, 교육수준, 근무연수이었다.

본 연구는 건강신념모형을 적용하여 일개 병원 임상간호사의 손씻기 수행도에 대하여 알아보고자 시행되었다. 본 연구를 통해 손씻기에 대한건강신념 중 지각된 유익성이 높으면 지각된 장애성은 낮아지고, 수행을 위한 동기는 높아지는 것을 확인하였다.
후속연구
둘째, 손씻기 수행도에 영향을 미칠 수 있는 일반적 특성의 다양한 변수를 규명하는 후속연구가 필요하다.
첫째, 본 연구의 대상이 G 광역시에 위치한 일개 병원 임상간호사만을 대상으로 한정되어서 연구 대상자가 제한적이고 일반화에 어려움이 있다. 따라서 후속 연구를 통하여 대상자의 범위를 확대하여 건강신념모형을 적용한 손씻기 수행도 분석이 추가적으로 필요할 것으로 사료된다.
셋째, 손씻기에 대한 건강신념과 관련된 연구 도구가 제한적이므로 손씻기와 더불어 감염관리와 관련 있는 건강증진행위와 건강신념 등 연구 도구를 추가적으로 적용한 추후 연구가 필요하리라 생각된다.
