This case report was conducted to introduce the use of miniscalpel-acupuncture for the treatment of hand osteoarthritis. A-76-year old female patient who had pain on both hands due to osteoarthritis was treated with miniscalpel-acupuncture for 7 weeks. The main outcome were the Numeral Rating Scale (NRS) and the grip strength by dynamometer. After miniscalpel-acupuncture treatment, NRS was decreased and the strength of grasp was increased. The result of this study suggests that the treatment of miniscalpel-acupuncture may be effectively used to decrease pain and increase grip strength of patients who suffer from hand osteoarthritis. Further research is necessary by the reason of the limited sample size of this study.

대상 데이터

대상 환자는 시골에서 농사를 짓는 76세 여성이다. 본 증례는 치료 시행 시 연구 출판 및 개인정보 보호에 관해 고지하였고, 환자의 동의를 받아 진행되었다.

데이터처리

매 치료 전에 각 손가락 관절의 통증을 0~10까지 numeral rating scale (NRS)로 측정하였으며, 근력 검사를 위해 악력을 측정하였다. 악력은 Jamar 유압식 악력계(Jamar dynamometer, Lafayette Instrument Co.

이론/모형

발침 후 압박지혈과 함께 지혈밴드를 붙여주며, 환부가 오염되지 않도록 교육하였다. 본 침 치료법 서술은 Standards for Reporting Interventions in Clinical Trials of Acupuncture(STRICTA) 기준에 맞추어 작성되었다.

성능/효과

, Lafayette, IN, USA)를 사용하여 측정하였고, 정확성을 높이기 위해 3번 측정하여 측정값의 평균값을 기록하였다. 7주간의 치료 후 세 관절의 NRS는 각각 6~8점에서 모두 2점으로 감소하였고, 악력의 경우 우측 손은 15.3 kg에서 22.3 kg, 좌측 손은 20 kg에서 24 kg으로 각각 증가하였다(Table Ⅰ). ;7주간 치료 후 각 관절의 굴곡 제한이 호전되어 손을 꽉 쥘 수 있었다.
상기 증례를 통해 변형을 동반한 Hand OA에 도침 치료가 통증 완화와 기능 회복에 도움이 될 수 있다는 것을 확인하였다. 단일치료만 시행하여 도침 치료만의 효과를 평가하였으며, 기타 치료에 반응하지 않던 환자의 증상이 호전되었다는 점에 의의가 있다.

후속연구

본 증례에서 치료 후 증상이 호전되었다가 오히려 통증과 악력이 악화되는 경우가 있었는데, 이러한 반응은 치료 후 환자가 손가락 관절을 많이 사용한 경우 나타났다. Hand OA는 반복적으로 손을 과사용하는 경우 증상이 악화되는 것이 기존에 보고되었으므로¹⁹⁾, 추후 연구에서는 결과에 영향을 줄 수 있는 노동과 같은 인자는 체계적인 평가와 기록이 필요할 것으로 사료된다.
본 증례에서 도침 치료를 통해 신전 건 부착부를 자극해 재생을 유도하고, 자락을 통해 염증물질을 배출시켜 통증이 완화된 것으로 추정된다. 그러나 기존 생리적 연구는 무릎관절염에 국한된 연구로, Hand OA에 도침 치료가 미치는 영향에 대해서는 독립적인 연구가 필요할 것으로 보인다.
단일 증례 보고라는 한계점과 손의 반복적인 사용과 같이 결과에 영향을 미치는 교란인자를 차단하지 못했다는 한계점이 있지만, 본 증례는 수술 적응증에 해당하는 Hand OA에 대해 도침의 단일 중재로 증상과 악력을 호전시켰다는 점에서 의미가 있다. 또한 Hand OA는 높은 유병률에 비해 한의학 연구가 부족한 질환으로 추후 지속적 임상 연구가 필요할 것으로 사료된다.
