Purpose: This study attempts to identify the level of awareness, ethical awareness, and decision-making among nursing students, in response to pandemic infectious diseases. Methods: Subjects were 210 nursing college students attending colleges in 5 cities and provinces nationwide. Data were collected from November 2017 to April 2018 using a self-administrated questionnaire, and analyzed utilizing descriptive statistics. Results: Considering the awareness of pandemic infectious diseases, the nursing students expressed high concern about the possibility of future outbreaks of pandemic infectious diseases and seriousness of the situation. There were numerous negative views on the response and stockpiling of drugs by the government. Ethical awareness of pandemic infectious diseases was high, with demands for accurate information and proper protective equipment. Overall, ethical decision-making when responding to pandemic infectious diseases represented a high score. Also, higher awareness levels of pandemic infectious diseases would result in increased ethical and ethical decision-making. Conclusion: In conclusion, we propose the introduction of an ethical education program for medical personnel and nursing university students, to enable the handling of future pandemic outbreaks of new infectious diseases.

대상 데이터

본 연구의 대상은 4개 시도(서울, 강원, 충남, 경북) 에 위치한 5개 학교 간호대학생 210명으로 자료수집 시점에 정규 교육과정 중 임상 실습을 경험한 학생을 대상으로 하였다. 연구 참여자의 구체적 선정 기준과 제외 기준은 다음과 같다.
95로 210명이 산출되었으며, 탈락률을 고려하여 총 230부를 배포하여 225부가 회수되었다. 이중 응답이 불충분한 5부와 감염 교육을 받지 않은 10명을 제외한 210부를 최종 분석에 사용하였다.

데이터처리

대상자의 일반적 특성은 백분율, 평균, 표준편차와 같은 기술통계로 분석하였다. 간호대 학생의 신종 감염병에 대한 인식, 신종 감염병 상황에서의 윤리 인식 및 윤리적 의사결정 정도는 각 문항별 점수와 전체문항 점수의 평균과 표준편차를 구하였다. 측정 변수들 간의 상관관계는 Pearson's Correlation Coefficient를 이용하여 확인하였다.

이론/모형

신종 감염병에 대한 인식은 Park (2016)이 개발한 신종 감염병에 대한 인식 도구를 사용하였다. 도구는 각 질문에 대한 개인의 생각을 나타내는 것으로, 신종 감염병에 대한 명확한 설명이 가능한지, 신종 감염병 진행 과정에 대해 관심이 있는지, 신종 감염병 발생할 가능성이 있는지, 신종 감염병에 대한 WHO와 정부의 대응이 적절했는지, 정부가 충분한 약제를 비축하고 있는지, 신종 감염병 발생 시 치료제가 부족할 것이지, 전문가들이 잘 예측할 수 있는지, 감염병의 심각도가 높은지, 국가 감염병 위기 단계 분류 및 판단기준이 적합한지에 대한 질문들로 구성되어 있다.

성능/효과

본 연구는 전국 5개 간호대학생 220명을 대상으로 신종 감염병에 대한 인식도, 신종 감염병 상황에서의 윤리 인식 및 윤리적 의사결정의 수준을 파악하고자 시도되었다. 본 연구 결과 간호대학생의 신종 감염병에 대한 인식은 미래 신종 감염병 발생 가능성 및 제반 상황의 심각성에 대해 높은 우려를 나타냈다. 반면 감염병 유행 시 정부의 대응이나 정부의 약제 비축 능력에 대해서는 적절하지 못하다는 의견을 보였다.
상관관계 분석 결과 간호대학생의 신종 감염병에 대한 인식 정도가 높을수록 신종 감염병 발생 상황에서 윤리 인식이 향상되고 올바른 윤리적 의사결정을 할 수 있는 것으로 나타났다. 선행연구(Park & Lee, 2019)에서는 직접 환자를 돌본 대상자 그룹이 그렇지 않은 대상자 그룹에 비해 윤리 인식과 윤리적 의사결정 수준이 높았던 점을 참고하면, 감염병에 대해 정확하게 인지하고 있는 것이 올바른 윤리 인식과 윤리적 의사결정에 도움을 줄 수 있음을 알 수 있다.
반면 감염병 유행 시 정부의 대응이나 정부의 약제 비축 능력에 대해서는 적절하지 못하다는 의견을 보였다. 신종 감염병 상황에서의 윤리 인식은 정확한 정보와 적절한 보호 장비를 제공 받는 것에 대한 요구도가 높았으며, 신종 감염병 상황에서의 윤리적 의사결정에서는 전반적인 영역에서 높은 윤리적 의사결정 수준을 나타냈다. 또한, 신종 감염병에 대한 인식 수준이 높을수록 신종 감염병 상황에서 높은 윤리 인식을 가지고 올바른 윤리적 의사 결정을 할 수 있었다.

후속연구

신종 감염병 발생의 국가적 재난 상황은 대처보다는 대비하는 것이 더욱 중요하며, 정부와 의료 분야 그리고 온 국민이 함께 극복해야 할 상황이다. 국가 정책기관에서는 재난 상황에 대처하는 의료인을 위한 윤리적 대응방안을 제시해야 할 것이며, 교육적 측면에서는 다양한 사례 연구를 통해 윤리적 인식 정립에 도움을 주는 것이 필요할 것이다. 실무에서는 임상 환경별 대응 프로토콜 마련과 교육, 훈련을 통한 숙지가 필요할 것이다.
