Purpose: To determine effects of irrational parenthood cognition, family support, and resilience on depression in infertile women. Methods: Subjects were 118 infertile women who agreed to participate in this study. Data were collected from April 16 to July 31, 2018. Collected data were analyzed using descriptive statistics, t-test, analysis of variance (ANOVA), Pearson's correlation and multiple regression with SPSS WIN 23.0 program. Results: Depression significantly differed according to the burden of treatment cost and presence of people giving stress. Depression showed significantly positive correlation with irrational parenthood cognition and significantly negative correlations with family support and resilience. Factors affecting depression were irrational parenthood cognition, family support, and resilience. Irrational parenthood cognition had the greatest effect on depression. These three variables explained 35.8% of total variance. Conclusion: Irrational parenthood cognition, family support, and resilience affected depression of infertile women, with irrational parenthood cognition having the greatest effect. Therefore, it is important to develop and implement programs that can reduce irrational parenthood cognition and increase family support and resilience in order to lower depression of infertile women. 본 연구 대상자는 B 광역시에 소재하는 1개 대학병원 난임 센터, 1개 여성병원 난임 클리닉, 1개 난임 전문 여성의원에서 난임 진료 및 치료를 받는 여성을 대상으로 편의 추출하였다. 연구대상자의 선정기준은 산부인과 전문의에 의해 일차성 난임 혹은 이차성 난임을 진단받은 출산 경험이 없는 여성, 현재 난임 진료 혹은 치료(배란유도, 인공수정, 체외수정 등)를 받고 있거나 난임 치료를 시행하여 실패를 경험한 여성, 정신과적 질환이 없는 여성 등이다.

이론/모형

Beck depression inventory (BDI)[24]는 가장 널리 사용되고 있는 우울 척도 중에 하나로 미국 정신과학회가 Diagnostic and Statistical Manual of Mental Disorders, 4th Edition (DSMIV)의 진단기준을 반영하여 개정(1996)한 BDI-II를 Kim 등[25]이 한국어로 번안한 한국판 벡 우울척도 2판(Korean Beck Depression Inventory, K-BDI-II) 도구를 저작권을 구입한 뒤 사용하였다. 자기보고식 질문지로 총 21문항, 4점 Likert 척도로 구성되며, 점수의 범위는 최저 0점에서 최고 63점으로 BDI 총점에 따라 0–13점은 최경도 우울상태, 14–19점은 경도 우울상태, 20–28점은 중등도 우울상태, 29–63점은 고도 우울상태로 분류되어, 점수가 높을수록 우울 수준이 심하다는 것을 의미한다.
난임 부부를 대상으로 개발한 14문항의 비합리적 부모 신념(Questionnaire for irrational parenthood cognition, QIPC) 도구[7] 중 윤리적 문제로 1문항을 제외시켜 13문항으로 수정[8], 번안한 도구[20]를 번안자의 승인을 받고 그대로 사용하였다. 이 도구는 총 13문항으로 구성되어 있고, 5점 Likert 척도로 구성되며 ‘전혀 그렇지 않다’에 0점, ‘그렇지 않다’에 1점, ‘보통이다’에 2점, ‘그렇다’에 3점, ‘매우 그렇다’에 4점으로 이루어졌다.
대학생을 대상으로 개발한 대인관계 지지척도(Interpersonal support evaluation list, ISEL) 도구[21]를 수정한 사회적 지지도구[22] 중 가족지지 문항만을 사용한 도구[11]를 사용하였다. E-mail을 통해 도구 개발자의 승인을 얻은 뒤 사용하였다.
코너-데이비슨 회복력 척도(Korean Connor-Davidson resilience scale, CD-RISC) 도구[23]를 개발자에게 승인 요청 후 요구한 사용료를 지불한 다음, 한글판 도구를 첨부파일로 받아 사용하였다. 총 25문항으로 구성되며, 5점 Likert 척도로 구성되며 ‘전혀 그렇지 않다’에 0점, ‘거의 그렇지 않다’에 1점, ‘때때로 그렇다’에 2점, ‘흔히 그렇다’에 3점, ‘거의 언제나 그렇다’ 에 4점으로 이루어졌다.

성능/효과

난임 여성의 비합리적 부모 신념, 가족지지, 회복탄력성 및 우울과의 상관관계를 검증한 결과, 난임 여성의 우울은 비합리적 부모 신념과 유의한 양의 상관관계가 있고, 가족지지와 회복탄력성과는 유의한 음의 상관관계가 있는 것으로 나타났다. 이는 난임 여성을 대상으로 한 연구[27]에서 난임 여성의 비합리적인 부모 신념은 주위의 사회적 압박과 상관관계가 있었으며, Kim과 Shin [11]의 연구에서는 난임 여성의 가족지지와 회복탄력성이, Jung [15]의 연구에서는 난임 여성의 회복탄력성과 우울이 상관관계를 나타내 본 연구결과와 일치하였다.
본 연구에서 난임 여성의 우울에 영향을 미치는 요인은 비합리적 부모 신념, 가족지지, 회복탄력성으로, 이 중 비합리적 부모 신념의 영향력이 가장 큰 요인으로 확인되었다. 따라서 난임 여성의 우울을 감소시키기 위해서 비합리적 부모 신념과 같은 부정적 인지를 변화시킬 필요가 있다.
본 연구에서 난임 여성의 우울은 일반적 특성과 난임 관련 특성 중 치료비 부담, 스트레스 주는 사람의 유무에서 유의한 차이가 있었다. 이는 같은 도구를 사용한 연구[15]에서 교육수준, 스트레스 주는 사람, 남편과의 관계, 비용부담감에서 유의한 차이를 나타낸 결과와 부분적으로 일치하는 것이다.
