본 연구는 중년여성의 생활 스트레스와 우울 간의 관계에서 사회적 지지와 삶의 의미의 매개효과를 확인하여 중년여성의 정신건강 증진을 위한 프로그램 개발의 기초 자료를 제공하는데 목적이 있다. 연구대상자는 D시와 K시 소재 중년여성 중 본 연구에 참여하기를 동의한 201명을 대상으로 자가 보고형 설문지를 이용하였으며, 2018년 3월 4일 부터 30일까지 자료 수집하였다. 수집된 자료는 경로분석을 실시하였으며, 최종 모형의 적합지수는 RMSEA .03, CFI .98, NFI .95로 나타났다. 연구결과는 다음과 같다. 중년여성의 생활 스트레스는 삶의 의미와 사회적 지지에 직접효과가 확인되었고, 생활 스트레스(β=.05, p< .001), 삶의 의미(β=.05, p< .001), 사회적 지지(β=.05, p< .001)는 우울에 직접효과가 유의한 것으로 확인되었다. 또한 생활 스트레스는 삶의 의미와 사회적 지지를 매개하여 우울에 간접효과가 유의한 것으로 확인되어 삶의 의미와 사회적 지지는 부분 매개효과가 확인되었다. 본 연구결과를 바탕으로 중년여성의 정신건강 증진 프로그램 개발의 기초자료로 활용되기를 기대한다.

This study was undertaken to provide basic data for the development of a mental health promotion intervention program, by confirming the mediating effects of social support in relation to the effect on the life stress and meaning of life in middle-aged women. The subjects of this study were 201 middle-aged women from D and K cities. The data were analyzed with descriptive statistics and Pearson's correlations using a statistical program for structural equation modeling (SEM); fitness of the final model was RMSEA 0.03, CFI 0.98, and NFI 0.95. The major learning from this study was that life stress has a direct effect on meaning of life and social support. Life stress (β=0.05, p< 0.001), meaning of life (β=0.05, p< 0.001), and social support (β=0.05, p< 0.001) have a significant and direct effect on depression. The findings also suggest that life stress indirectly affects the mediating effect between meaning of life and social support, and also depression of middle-aged women. We believe that results of this study encompass basic data that will aid in developing a program to promote the mental health of middle-aged women.

본연구대상자는 D시와 K시에 거주하고 있는 만45세~64세의 여성을 편의 표집하였다. 대상자의 선정기준은 본 연구의 목적을 이해하고 설문에 동의한 여성 300명을 대상으로 하였으며, 자궁적출술과 난소절제술을 받은 자, 산부인과 질환이 있는 자는 여성생식기 급성 및 만성질환으로 배제하였다.
자료 수집은 2018년 3월 4일 에서 30일까지 진행하였고, 조사에 소요된 시간은 20분 정도이었다. 수집된 자료 중 응답이 불안전한 것을 제외 한 201부를 분석대상에 포함하였다.

데이터처리

일반적 특성, 측정변인들의 평균, 표준편차 등은 기술통계를 이용하였고, 측정변인 간의 관련성은 Pearson's correlation coefficient로 분석하였다. 본 연구의 경로모형 적합도 검증은 최대 우도법 (maximum likelihood)을 이용하였고, 각 경로계수 및 효과검증은 부트스트랩(bootstrapping)을 이용하였다.

이론/모형

사회적 지지는 Zimet 등[22] 이 개발한 Multiple Dimensional Scale of Perceived Social Support, MSPSS) 도구를 사용하였다. 이 도구는 총 12문항으로 가족지지 4문항, 친구지지 4문항, 특별지지 4문항으로 구성되었다.
삶의 의미는 Steger 등[16]이 개발한 meaning of lfe (MLQ)것을 Won 등[19]이 타당화한 도구를 사용하 였다. 이 도구는 총10문항으로 각 문항은 ‘전혀 그렇지 않다’ 1점에서 ‘매우 그렇다’ 5점까지이며, 점수가 높을수록 삶의 의미가 높음을 의미한다.
생활 스트레스는 Frank와 Zyzanski[20]가 개발한 Brief Encounter Psychosocial Instrument, BEPSI 도구를 Yim 등[21]이 수정·보완한 한국어판 BEPSI-K를 사용하였다. 이 도구는 총 5문항으로 각 문항은 ‘전혀 그렇지 않다’ 1점에서 ‘매우 그렇다’ 5점까지이며, 점수가 높을수록 생활스트레스정도가 높은 것을 의미한다.
우울은 Radloff[23]가 개발한 우울(Center for Epidemiologic Studies Depression Scale, CES-D) 도구를 Chun, Choi와 Yang[24]이 수정 보완한 한국어판 CES-D 를 사용하였다. 이 도구는 총 20문항으로 각 문항은 ‘극히 드물게 그렇다’ 0점에서 ‘거의 대부분 그렇다’ 3점까지이며, 점수가 높을수록 우울정도가 높은 것을 의미한다.

성능/효과

둘째, 대상자의 삶의 의미가 높을수록 우울에 긍정적 효과가 있으며, 생활 스트레스가 우울 간의 관계에서 매개효과가 확인되었다. 이러한 결과는 중년여성을 대상으로 생활스트레스와 우울 등의 심리적 변인간의 매개효 과를 직접적인 관계를 다룬 연구가 거의 없는 실정이므로, 직접적인 비교가 어려운 실정이다.
셋째, 대상자의 사회적 지지가 높을수록 우울에 긍정적인 작용을 하고, 생활 스트레스와 우울 간의 관계에서 부분 매개효과가 확인되었다. 이러한 결과는 많은 선행연구[9,10,13]에서 사회적 지지가 높을수록 스트레스를 예방하거나 경감하여 우울과 같은 정신건강문제를 보호하는 간접효과와 직접효과로 보고되었다.
첫째, 최종모형에서 대상자의 우울에 직접적인 경로를 통해 영향을 미치는 변인은 생활 스트레스, 삶의 의미, 사회적 지지 등으로 확인되었다. 이는 생활 스트레스가 높을수록, 삶의 의미가 낮을수록, 사회적 지지가 낮을수록 대상자의 우울에 부정적으로 영향을 미치는 것으로 볼 수 있다.

후속연구

그러나 본 연구는 일부 중년여성을 대상으로 진행되었기 때문에 중년여성 전체로 일반화하는데 제한이 있다. 따라서 대상자를 확대하여, 추후연구가 진행되 어야 할 것이다. 본 연구의 중년여성의 정신건강 증진을 위한 경로모형은 간명하고 적합한 모형임을 실증적으로 검증하였으므로, 이를 기초로 개인의 사회심리적 변인을 고려한 맞춤형 정신건강증진 프로그램을 개발하여 효과를 검증하는 것이 필요하다.
따라서 대상자를 확대하여, 추후연구가 진행되 어야 할 것이다. 본 연구의 중년여성의 정신건강 증진을 위한 경로모형은 간명하고 적합한 모형임을 실증적으로 검증하였으므로, 이를 기초로 개인의 사회심리적 변인을 고려한 맞춤형 정신건강증진 프로그램을 개발하여 효과를 검증하는 것이 필요하다.
이러한 차이는 본 연구에서는 폐경증상을 단일항목으로 진행하여 차이가 있었다고 보여진다. 중년여성의 생애발달주기의 관점에서 폐경증상을 고려한 생활 스트레스와 우울의 정도가 차이가 있을 수 있으므로, 폐경증상을 고려한 반복연구가 필요하다고 사료된다.
