Purpose: The objective of this study was to identify the factors influencing maternal identity in pregnant women. Methods: Using a descriptive research design, a cross-sectional survey was conducted. In total, 127 pregnant women were recruited from a tertiary hospital in Korea from January to April 2019. Measurements included maternal identity, stress, depression, spousal and familial support, and demographic and obstetric characteristics. Data were analyzed by descriptive statistics, the independent t-test, one-way ANOVA, Pearson correlation coefficients, and stepwise multiple regression using SPSS version 25.0. Results: The mean score for maternal identity was 131.15 out of 160, and the mean scores for stress, depression, and spousal and familial support were 14.59 (out of 40), 6.82 (out of 30), and 109.04 (out of 132), respectively. Stress (r=-.38, p<.001), depression (r=-.37, p<.001), and spousal and familial support (r=.37, p<.001) were significantly correlated with maternal identity. In multiple regression analysis, stress (β=-0.27, p=.005) and spousal and familial support (β=0.23, p=.014) were found to be significant factors influencing maternal identity in pregnant women (F=14.19, p<.001). Conclusion: It is necessary to develop effective strategies to mitigate stress and to encourage spousal and familial support in pregnant women. Such strategies could further enable pregnant women to enhance their maternal identity.

1 프리웨어를 이용해 확인한 결과, 최소 표본의 수는 109명이었다. 선행연구[9]에서의 탈락률 약 25%를 고려하여 136명의 대상자에게 설문지를 배부하였고, 배부된 설문지 중 131부가 회수되었으며, 불충분한 응답의 설문지 4부를 제외하여 최종 127명 대상자의 자료를 연구의 분석에 이용하였다.

2) 임부의 일반적 및 임신 관련 특성에 따른 모성 정체성의 차이는 independent t-test, one-way ANOVA를 이용해 분석하였고, 사후검증으로 Scheffé test를 이용하여 분석하였다.
4) 임부의 모성 정체성에 영향을 미치는 요인은 단계적 다중회귀분석(stepwise multiple regression)을 이용하여 분석하였다.
임부의 모성 정체성에 영향을 미치는 요인을 탐색하기 위해, 본 연구의 주요한 독립변수인 스트레스, 우울 및 배우자와 가족의 지지와 함께 단변량 분석에서 모성 정체성과의 관련성이 있는 것으로 설명된 특성인 습관성 유산 여부와 난임 시술 방법을 독립변수로 투입하여 다중회귀분석을 실시하였다. 독립변수에 대한 회귀분석의 가정을 검정하기 위해 다중공선성, 잔차, 특이값 등을 확인하였 다.

이론/모형

배우자와 가족의 지지는 Prenatal Psychosocial Profile [17] 도구를 Kim [18]이 한국어로 번안한 도구를 사용하여 측정하였다. 본 도구 는 총 22개 문항으로 배우자의 지지 영역 11문항과 가족 및 친지의 지지 영역 11문항으로 구성되어 있다.
본 연구에서는 Kim과 Hong [10]이 개발한 임부의 모성 정체성 도구를 사용하였으며, 이는 모성 정체성의 행위적 요인 20문항과 정서적 요인 20문항의 총 40문항으로 구성되어 있다. 모든 문항은 Likert 4점 척도(‘아니다’ 1점, ‘매우 그렇다’ 4점)로 측정되고, 점수 가능 범위는 40–160점으로, 점수가 높을수록 예비 어머니로서 모성 정체성이 높은 상태임을 의미한다.
우울은 Han 등[14]이 Edinburgh Postnatal Depression Scale [15]을 한국어로 번안한 도구를 사용하여 측정하였다. 도구 개발 당시 본 도구는 산후우울증에 대한 선별검사 목적으로 개발되었으나, 이후 임신 중 우울에 대한 측정도구로서 국내외 연구에서 타당도를 확보하여 이용된 바 있다[16].
지각된 스트레스는 Lee 등[11]이 Perceived Stress Scale [12]을 한국어로 번안한 도구를 사용하여 측정하였다. 본 도구는 임부 및 산모를 대상으로 타당도가 검증된 바 있다[13].

성능/효과

본 연구는 임부의 모성 정체성 정도를 파악하고 모성 정체성에 유의한 영향을 미치는 요인들을 확인하고자 시행되었다. 본 연구에서 임부의 모성 정체성 정도는 중등도 이상의 수준을 보였고, 스트레스, 우울 및 배우자와 가족의 지지와 유의한 상관관계를 보였으며, 특히 모성 정체성은 스트레스가 낮을수록, 배우자와 가족의 지지가 높을수록 유의하게 증가하는 것으로 나타났다. 본 연구결과를 토대로 임부의 모성 정체성을 증진시키기 위해 이들 변수를 고려한 간호중재 프로그램을 개발하고 적용하여, 향후 임부의 모성 정체성과 나아가 여성의 모성 역할 증진 및 태아 애착을 향상시킬 수 있을 것으로 기대된다.

후속연구

첫째, 본 연구는 1개 지역 대학병원에 내원하는 임부를 대상으로 조사하였으나, 추후 국내 다양한 지역의 대상자 수집을 통해 임신의 초기 과정부터 추후 출산 이후에 이르기까지 각 시기에 따른 모성 정체성의 영향요인을 파악하고자 하는 종단적 연구가 필요할 것이다. 둘째, 본 연구에서 차이 검증 및 상관관계 분석 시 유의하였으나 회귀분석에서는 제외되었던 습관성 유산 여부 및 난임 시술 방법과 우울 변수 등을 포함하여 다양한 관련 요인들을 고려한 모성 정체성 영향 요인에 대한 반복 연구가 필요할 것이다. 셋째, 본 연구에서 모성 정체성의 유의한 영향 요인으로 나타난 스트레스 및 배우자와 가족의 지지를 고려한 임부의 모성 정체성 증진 프로그램의 개발 및 효과 평가 연구가 필요할 것으로 생각된다.
