연구배경: 흡연이 갑상선 기능에 상당한 영향을 미치지만 아직 정확한 관계는 밝혀지지 않았다. 본 연구에서는 요오드 섭취가 충분한 우리나라에서 흡연이 갑상선 기능에 미치는 영향에 대해 알아보고자 하였다. 방법: 2001년 1월 1일부터 2003년 12월 31일까지 서울아산병원 건강증진센터를 방문한 20세 이상 79세 이하의 성인들 중에서 공복의 정맥 혈청으로 TSH 와 유리 T4를 측정하였던 90,970명 중 문진표 기록상 갑상선 질환의 기왕력 및 가족력이나 갑상선 기능에 영향을 주는 약을 복용한 기왕력이 있는 대상을 제외한 47,577명을 대상으로 하였다. 문진표에 근거하여 흡연력은 비흡연자, 과거 흡연자, 현재 흡연자로 구분하였고 현재 흡연자에서 일일 평균 흡연량은 20개피 1갑 기준으로 1갑 미만, 1~2갑 미만, 2갑 이상 흡연의 세 군으로 분류하여 모든 자료를 후향적으로 분석 하였다. 결과: 남자에서 TSH의 기하 평균 농도는 비흡연자(1.65 mIU/L; 95% CI, 1.63-1.68)와 비교하여 현재 흡연자(1.40 mIU/L; 95% CI, 1.38-1.41) 및 과거 흡연자(1.59 mIU/L; 95% CI 1.57-1.61)에서 낮았다. 유리 T4의 평균 농도는 비흡연자(1.234 ng/dL; 95% CI, 1.230-1.238)와 비교하여 현재 흡연자(1.236 ng/dL; 95% CI, 1.234-1.239)에서 높았으나 과거 흡연자(1.241 ng/dL; 95% CI, 1.238-1.244)와는 차이는 없었다. 여자에서도 TSH의 기하 평균 농도는 비흡연자(1.85 mIU/L; 95% CI, 1.83-1.87)와 비교하여 현재 흡연자(1.75 mIU/L; 95% CI, 1.67-1.87)에서 낮았지만 과거 흡연자(1.80 mIU/L; 95% CI, 1.70-1.91)와 차이는 없었다. 유리 T4의 평균 농도는 비흡연자(1.138 ng/dL 95% CI, 1.135-1.140)와 비교하여 현재 흡연자(1.149 ng/dL 95% CI, 1.139-1.159)에서 높았으나 과거흡연자(1.149 ng/dL 95% CI, 1.136-1.162)와 차이가 없었다. 남자의 현재 흡연자 중에서 하루 평균 흡연량이 1갑 미만 흡연군(1.42 mIU/L; 95% CI, 1.39-1.46)과 비교하여 2갑 이상 흡연군(1.35 mIU/L; 95% CI, 1.31-1.39)에서 TSH의 기하 평균 농도가 낮았다. 남자에서 비흡연자와 비교하여 현재 흡연자에서 무증상갑상선기능저하증의 유병률은 낮았다(odds ratio, 0.53 95% CI, 0.43-0.66). 결론: 이전에 갑상선 질환이 없었던 우리나라 성인에서 흡연은 낮은 TSH 농도 및 높은 유리 T4농도와 연관성을 보였고 남자에서 무증상 갑상선기능저하증의 낮은 유병률과 연관이 있었다. 따라서 향후 갑상선 기능 이상을 해석하는데 있어서 흡연의 영향도 고려해야 할 것으로 생각된다.

Methods: We conducted a cross-sectional study that included 90,970 adults (age range: 20 to 79) who had visited the health promotion center at Asan Medical Center between January 1, 2001, and December 31, 2003. Those subjects with previous known thyroid disease, a history of thyroid operation, a history of thyroid function altering medication (herb, estrogen or digestive) or a family history of thyroid disease were excluded. Finally, 47,577 subjects (males: 30,726, females: 16,851) were included in this study. We calculated the age-adjusted geometric mean of the serum TSH and the age-adjusted mean of the serum free T4 among the current, former and never smokers. We also analyzed the age-adjusted prevalence of hypothyroidism and hyperthyroidism among each group. Results: Among men, the geometric mean TSH level was significantly low in the current (1.40 mIU/L, 95% confidence interval [CI]: 1.38-1.41) and former smokers (1.59 mIU/L, 95% CI: 1.57-1.61) compared with the never smokers (1.65 mIU/L, 95% CI: 1.63-1.68). The mean free T4 level was high in the current smokers (1.236 ng/dL, 95% CI: 1.234-1.239) compared with the never smokers (1.234 ng/dL, 95% CI: 1.230-1.238). Similarly, among women, the geometric mean TSH level was low in the current smokers (1.75 mIU/L, 95% CI: 1.67-1.87) compared with the never smokers (1.85 mIU/L, 95% CI: 1.83-1.87).
