목적: 식도 및 위의 점막하 종양은 대부분 특별한 증상이 없어서 상부 위장관 내시경에서 우연히 발견되는 경우가 많다. 이들 대부분은 양성 경과를 보이지만 위장관 간질종양은 악성 변화를 할 수 있어서 종양의 크기에 따라 절제가 필요한 경우도 있으나 고유근층에 있는 경우 고식적인 방법의 내시경적 절제술로는 절제 가 힘들다. 이에 본 교실에서는 점막하 종양의 절제에 내시경 종양하 박리법을 시행하여 이의 유용성에 대해 알아보고자 하였다. 대상 및 방법: 점막하 종양으로 진단된 15예(남자 10예, 여자 5예, 위 13예, 식도 2예)에서 일괄 절제를 위해 다양한 종류의 절개도로 종양의 변연부 점막하층과 기저부를 박리하였다. 시술 전 모든 환자에서 내시경 초음파를 시행하여 종양의 크기와 기원 그리고 종양의 성장 방향을 알아보았으며 절제 후 최종 조직형과 크기, 시술 시간과 시술 방법, 일괄 절제 여부와 합병증에 대해 후향적으로 분석하였다. 결과: 병리학적 진단으로 고유근층의 위장관 간질종양이 6예, 고유근층의 평활근종이 2예, 점막근층의 평활근종이 2예, 점막하층의 이소성 췌장이 3예, 점막하층의 지방종이 1예, 점막하층의 혈관종이 1예였으며 13예(86.7%)에서 일괄 절제가 가능하였다. 절제된 조직의 평균 크기는 29.5×21.1 mm이었고, 평균 시술 시간은 49.4분이었다. 고유근층 기원 종양을 가진 2예의 환자에서 천공이 발생되었으나 클립을 이용한 내시경적 봉합술로 외과적 수술 없이 치료되었다. 결론: 식도 및 위 점막하 종양에서 내시경 종양하 박리법은 비교적 큰 크기의 종양도 일괄 절제가 가능하므로 유용한 치료 방법이 될 수 있다. 그러나 고유근층과 같이 깊은 곳의 종양은 천공 등의 합병증 발생에 유의해야 한다.

Background/Aims: The removal of esophageal and gastric submucosal tumors is difficult using conventional endoscopic mucosal resection methods. This study examined the usefulness of an endoscopic subtumoral dissection for an en-bloc resection of submucosal tumors. Methods: An endoscopic subtumoral dissection was attempted for an en-bloc resection in 15 submucosal tumors (M:F=10:5, 13 stomach, 2 esophagus). Before the procedures, endoscopic ultrasonography was performed in all cases. The procedure was carried out using various electrosurgical knives, such as an endoscopic submucosal dissection. Results: Pathological and immunohistochemical studies confirmed a gastrointestinal stromal tumor in 6 cases. Other pathological diagnoses were made in 9 patients with submucosal lesions: leiomyoma (4), ectopic pancreas (3), lipoma (1), and hemangioma (1). An en-bloc resection was performed in 13 of the 15 tumors (86.7%). The mean specimen size was 29.5×21.1 mm. The mean procedure time was 49.4 minutes (range: 8∼103 minutes). Gastric perforation was a complication in 2 cases with GIST. However, the two perforated cases were treated with endoscopic closure using endoclips and recovered without the need for surgery. Conclusions: An endoscopic subtumoral dissection technique is useful for an en-bloc resection of esophageal and gastric submucosal tumors. However, sufficient attention should be paid to the detection of perforations in the case of tumors with a proper muscle origin.
