목적 : 소화성 궤양 출혈은 내시경 지혈술의 도입 이후 응급 수술 및 사망률이 감소하였으나 아직도 약 10~30% 의 환자에서 재출혈이 발생한다. 프로톤 펌프 억제제(PPI) 추가 사용으로 재출혈 감소가 인정되고 있으나 그 투여방법에 대해서는 확립되지 않았다. 이에 이번 연구는 소화성 궤양 출혈 환자에서 재출혈의 위험인자를 알아보고 PPI 투여 방법에 따른 재출혈률에 대해 알아보고자 하였다. 방법 : 2003년 3월부터2006년 2월까지 토혈, 흑색변, 혈변으로 응급실을 방문하여 내시경검사에서 소화성궤양 출혈로 진단된 191명을 대상으로, 재출혈 유무에 따른 임상적 특징, 검사실 소견, 내시경검사 소견과 치료방법에 대하여 후향적으로 분석하였다. 내시경검사 후 혈변 또는 토혈의 재발, 쇼크의 재발생, 24시간 이내 2 g/dL 이상의 혈색소치 감소를 재출혈로 판정하였다. PPI 투여 방법으로는 내시경 검사 전후 적어도 3일 이상 간헐적 정맥 투여군(iv bolus group), 지속적 정맥 투여군(continuous infusion group), 기타군(others group)으로 구분하였다. 결과 : 소화성 궤양 출혈로 진단된 191명 중 9명(4.7%)에서 재출혈이 발생하였는데 9명의 재출혈군과 182명의 지혈군으로 나누어, 성별, 연령, 동반질환의 유무, 소화성 궤양 및 궤양 출혈의 과거력, 비스테로이드성 소염제, 항응고제 또는 아스피린 복용력, 흡연, 음주, 토혈 유무, 내원 시 혈색소치, 출혈의 위치, 출혈 양상(활동성 출혈, 노출 혈관, 출혈반점), H. pylori, 감염여부에 차이가 없었다. 다만 5 unit 초 과의 수혈이 필요한 경우(p=0.016)와 내원 시 쇼크 상태 (p=0.013)가 통계적으로 유의하였다. 또한 PPI 투여 방법(지속적, 간헐적 주사) 등에서 통계적 차이는 없었다. 결론 : 이번 연구에서의 소화성 궤양 재출혈률은 4.7%로 기존의 내시경 지혈치료 후 재출혈에 관한 보고 10.3~20.1%보다 낮았는데 이는 아마도 24시간 이내의 적극적인 내시경 치료와 함께 PPI조기 투여 때문이라고 생각한다. 따라서 쇼크 상태와 같이 대량 출혈이 일어난 경우나 내원 당시 5 unit 초과의 수혈이 필요한 재출혈 위험성이 높은 군에서는 곧바로 PPI를 투여하고 활력증후가 안정되자 마자 내시경 치료에 들어가는 접근방법이 필요하다고 생각한다.

Recently, treatment with proton pump inhibitor (PPI) after hemostasis has decreased the rate of rebleeding. In this study, we analyzed risk factors of peptic ulcer rebleeding and we evaluated whether the rebleeding rate could be different depending on the PPI treatment method. Methods : From March 2003 through February 2006, 639 patients visited the emergency room (ER) due to gastrointestinal hemorrhage. Among them, 191 patients were diagnosed to have peptic ulcer by endoscopy, and they were retrospectively analyzed for their clinical, laboratory and endoscopic findings. The PPI treatment method was categorized into the IV bolus group, the 8 mg/hr continuous infusion group and the others group. Results : Emergency endoscopy was performed within 24 hours in 86.9% of the patients with bleeding peptic ulcer. Rebleeding occurred in 9 cases (4.7%) within 7 days after hemostasis. On the basis of univariate analysis, shock on ER arrival (p=0.013) and over 5 units of packed red cells (PRCs) transfusion (p=0.016) were significant risk factors for rebleeding, yet the PPI treatment method did not affect the rate of rebleeding. Conclusions : The rebleeding rate of peptic ulcer in our study was 4.7%, and this was lower than the previous reports, and the rebleeding rate in our report may have been caused by the early hemostatic therapy together with intravenous infusion of PPI. These approaches are necessary as soon as the patients are stabilized, and especially for the group of patients who are at a high risk for rebleeding, such as those who experience shock at the ER and those who need over 5 units of PRC transfusion.
