A customized dental tray comprising ULDPE is formed using a person's own teeth as a template, or using a stone model of the person's teeth. A preformed dental tray or sheet comprising ULDPE is heated to become plastically deformable and placed over at least a portion of the person's teeth in order to form an impression of the person's teeth and yield an intermediate customized dental tray. Upon cooling sufficiently, the intermediate tray is removed from the persons' mouth and trimmed as needed to yield a finished customized tray corresponding to the person's teeth. The performed tray or sheet preferably comprises ULDPE, which yields a highly flexible and resilient final tray, although they may also contain other polymers or blends. The customized dental tray is useful for teeth bleaching and other dental treatments known in the art.

A customized dental tray as defined in claim 1, the dental tray further comprising at least one of ethylene vinyl acetate, polycaprolactone, polypropylene or another form of polyethylene.3. A customized dental tray as defined in claim 1, the upper periphery of the dental tray overlapping at least a portion of the person's gingiva when in use.4. A customized dental tray as defined in claim 1, the upper periphery of the dental tray terminating at the person's gingival margin when in use.5. A customized dental tray as defined in claim 1, the upper periphery of the dental tray terminating below the person's gingival margin when in use.6. A customized dental tray as defined in claim 14, the customized dental tray having an average wall thickness in a range of about 0.3 mm to about 1.8 mm.17. A customized dental tray as defined in claim 14, the customized dental tray having an average wall thickness in a range of about 0.4 mm to about 1.4 mm.18. A customized dental tray as defined in claim 14, the customized dental tray having an average wall thickness in a range of about 0.7 mm to about 1.2 mm.19. A method of manufacturing the customized dental tray of claim 1, the method comprising:providing a stone model representative of the person's teeth; providing a preformed sheet or tray comprising ultra low density polyethylene; placing the preformed sheet or tray over the stone model; and vacuum forming a customized dental tray from the preformed sheet or tray. 22. A method as defined in claim 21, further comprising applying a blockout material to at least a portion of the stone model in order for the customized dental tray to have one or more reservoirs formed therein.23. A method as defined in claim 21, further comprising trimming excess tray material from the customized dental tray.24. A method as defined in claim 23, the customized dental tray being trimmed so as to overlap the person's gingiva.25. A method as defined in claim 23, the customized dental tray being trimmed so as to terminate at the person's gingival margin.26. A method as defined in claim 23, the customized dental tray being trimmed so as to terminate below the person's gingival margin.27. A method as defined in claim 23, the customized dental tray further being scalloped up and around the person's interdental papilla.28. A method as defined in claim 21, the customized dental tray having a wall thickness in a range of about 0.4 to about 1.4 mm.29. A method for treating a person's teeth comprising: providing the customized dental tray of claim 1;introducing a quantity of a dental composition into the dental tray; placing the dental tray over the person's teeth and leaving it in place for a desired time in order to provide a desired treatment to at least one of the person's teeth or gingiva; and removing the dental tray from the person's teeth. 30. A method as defined in claim 29, wherein the dental composition comprises a dental bleaching composition.31. A method as defined in claim 29, wherein the dental composition comprises at least one of an anticariogenic or an antimicrobial composition.32. A method as defined in claim 29, wherein the dental tray comprises:a main body portion comprising a customized impression of at least a portion the person's teeth and a hollow interior into which the dental composition is placed; and an upper periphery that is designed to terminate shy of the person's gingival margin when the dental tray is placed over the person's teeth. 33. A method as defined in claim 29, wherein the dental tray comprises:a main body portion comprising a customized impression of at least a portion the person's teeth and a hollow interior into which the dental composition is placed; and an upper periphery that is designed to terminate at the person's gingival margin when the dental tray is placed over the person's teeth. 34. A method as defined in claim 29, wherein the dental tray comprises:a main body portion comprising a customized impression of at least a portion the person's teeth and a hollow interior into which the dental composition is placed; and an upper periphery that is designed to overlap the person's gingival margin when the dental tray is placed over the person's teeth. 35. A method as defined in claim 29, the customized tray being substantially transparent, the method further comprising examining the dental tray when placed over the person's teeth to determine whether the dental composition has a desired distribution over the person's teeth.
