What is claimed is: 1. An apparatus for monitoring a patient's heart condition, comprising: (a) a patient-location based unit configured for patient interaction and a microprocessor to facilitate monitoring a heart condition of the patient; (b) a central server in signal communication with the pati
What is claimed is: 1. An apparatus for monitoring a patient's heart condition, comprising: (a) a patient-location based unit configured for patient interaction and a microprocessor to facilitate monitoring a heart condition of the patient; (b) a central server in signal communication with the patient-location based unit and configured to receive signals from and transmit signals to said patient-location based unit, wherein the transmitted signals include instructions for execution in the patient-location based unit; and (c) a health care professional computer in signal communication with the central server for at least receiving patient health-related information, including at least information about the monitored heart condition, from the central server and to send at least one message from the health care professional computer through the central server to the patient-location based unit, where the patient-location based unit is configured to selectively connect to the central server, and where the central server stores the at least one message at least until there is a connection between the patient-location based unit and the central server. 2. The apparatus of claim 1, wherein the patient-location based unit further includes a receptacle for receiving at least one memory device, the memory device comprising a card or a cartridge. 3. The apparatus of claim 2, wherein the memory device has instructions therein for enabling monitoring the heart condition. 4. The apparatus of claim 2, wherein the memory device has instructions stored therein for modifying the instructions on the patient-location based unit. 5. The apparatus of claim 1, wherein the heart condition is a chronic heart condition. 6. The apparatus of claim 1, wherein the central server includes instructions for transmitting health-related information to the patient-location based unit. 7. The apparatus of claim 1, wherein the interaction with the patient is facilitated through use of a display. 8. The apparatus of claim 1, wherein the interaction with the patient is facilitated by at least one key or button. 9. The apparatus of claim 1, further comprising: a monitoring device configured to monitor a health parameter, the monitoring device comprising a blood glucose monitor, a blood pressure monitor, a peak-flow meter, a pulse monitor, or a body temperature monitor. 10. The apparatus of claim 1, wherein the monitoring occurs over a period of time. 11. The apparatus of claim 1, wherein at least one of the communications uses RF transmissions. 12. The apparatus of claim 1, wherein the transmitted signals include instructions for reconfiguring programs stored in the patient-location based unit. 13. A method for monitoring a patient's health condition, comprising: (a) monitoring a health condition of a patient by using a patient-location based unit configured for patient interaction; (b) communicating information to the patient by utilizing the patient-location based unit, wherein the information can be communicated to the patient-location based unit by sending a message from a health care professional computer through a central server to the patient-location based unit; (c) producing a signal representative of the monitored health condition; (d) communicating the health condition-related signal to the central server for providing health-related information, based on the health condition-related signal communicated to the central server, to the health care professional computer; and (e) receiving instructions from the central server for execution in the patient-location based unit; where the patient-location based unit is configured to selectively connect to the central server, and where the central server stores the at least one message at least until there is a connection between the patient-location based unit and the central server. 14. The method of claim 13, wherein the patient-location based unit further includes a receptacle for receiving at least one memory device; the memory device comprising a card or a cartridge. 15. The method of claim 14, wherein the memory device has instructions therein for enabling monitoring the heart condition. 16. The method of claim 14, wherein the memory device has instructions stored therein for updating the instructions on the patient-location based unit. 17. The method of claim 13, wherein the heart condition is a chronic heart condition. 18. The method of claim 13, wherein the central server includes instructions for transferring health-related information to the patient-location based unit. 19. The method of claim 13, wherein the interaction with the patient is facilitated through use of a display. 20. The method of claim 13, wherein the interaction with the patient is facilitated by a device comprising a key or a button. 21. The method of claim 13, wherein the monitored condition includes a blood glucose, a blood pressure, a pulse, air flow, or a body temperature. 22. The method of claim 13, wherein the monitoring occurs over a period of time. 23. The method of claim 13, wherein at least one of the communications uses RF transmission. 24. The method of claim 13, wherein the instructions in step (f) reconfigure programs stored in the patient-location based unit. 25. A heart condition monitoring system, comprising: (a) at least one central server arranged to receive and transmit data; (b) at least one microprocessor-based subsystem (i) including at least one microprocessor, display and memory (ii) configured to present information on the display and provide a signal representative of a heart condition of a person and (iii) arranged to facilitate communication of heart condition-related data to the central server; and (c) at least one health care professional computer in signal communication with the central server to receive health-related information based on the heart condition-related data received from the microprocessor-based subsystem and to send at least one message from the health care professional computer through the central server to the patient-location based unit, where the patient-location based unit is configured to selectively connect to the central server, and where the central server stores the at least one message at least until there is a connection between the patient-location based unit and the central server. 26. The system of claim 25, configured to realize systems for at least one of (a) self-care monitoring; and (b) control of afflictions. 27. The system of claim 25, wherein the microprocessor-based subsystem further includes (i) a data management unit; and (ii) a monitoring device that can transmit data to the data management unit or that can be connected to the data management unit. 28. The system of claim 27, wherein at least one monitoring device includes one or more of (a) a blood glucose monitor, (b) a blood pressure monitor, (c) a pulse monitor; (d) peak-flow meter; or (e) a body temperature monitor. 29. The system of claim 25, wherein at least one microprocessor, display and memory is in a handheld device. 30. The system of claim 25, wherein the microprocessor-based subsystem is capable of displaying pictorial information. 31. The system of claim 25, wherein the microprocessor-based subsystem further includes at least one of an insertable program card or cartridge, and wherein instructions on the program card or cartridge are configured to facilitate the monitoring. 32. The system of claim 25, wherein the system is configured to use the heart condition-related data to process a report. 33. The system of claim 32, wherein the report includes information for a period of time. 34. The system of claim 25, wherein the information presented includes at least one message. 35. The system of claim 25, wherein the information presented includes step-by-step instructions. 36. The system of claim 25, wherein the information presented is educational or motivational. 37. The system of claim 25, wherein the presentation of information can be controlled using at least one menu. 38. The system of claim 25, wherein the system is configured to enable at least one program to be provided for storage in a memory and execution by the micro-processor-based subsystem. 39. The system of claim 38, further comprising at least one of an insertable program card or cartridge and wherein at least part of the program is stored on the card or cartridge. 40. The system of claim 25, wherein at least one of the communications uses RF transmission. 41. A heart condition monitoring method, comprising: monitoring a heart condition of a person by using at least one microprocessor-based subsystem including at least one microprocessor, display and memory; presenting information on the display to the person; producing a digital signal representative of the monitored heart condition; establishing a connection between the microprocessor-based subsystem and a central server, communicating heart condition-related data to the central server for providing health-related information, based on the heart condition-related data communicated to the central server when the connection has been formed, and further communicating the health condition-related signal to a health care professional computer; disconnecting the connection between the microprocessor-based subsystem and a central server, storing at least one message from the health care professional computer in the central server at least until the next connection and sending the message from the central server to the microprocessor-base subsystem for the person being monitored, when the next connection is established. 42. The method of claim 41, further comprising at least one of self-care monitoring by the person; and controlling an affliction of the person. 43. The method of claim 41, wherein the microprocessor-based subsystem further includes (i) a data management unit; and (ii) a monitoring device that can be received directly by the data management unit or that can be connected to the data management unit. 44. The method of claim 41, wherein the monitored condition includes one or more of (a) blood glucose; (b) blood pressure; (c) pulse; (d) air flow; or (e) body temperature. 45. The method of claim 41, wherein at least one microprocessor, display and memory is in a handheld device. 46. The method of claim 41, further comprising displaying pictorial information on the display. 47. The method of claim 41, wherein the microprocessor-based subsystem further comprises at least one of an insertable program card or cartridge having instructions to facilitate the monitoring. 48. The method of claim 41, further comprising using the heart condition-related data to process a report. 49. The method of claim 48, wherein the report includes information for a period of time. 50. The method of claim 41, wherein the presented information includes at least one message. 51. The method of claim 41, wherein the presented information includes step-by-step instructions. 52. The method of claim 41, wherein the presented information is educational or motivational. 53. The method of claim 41, further comprising controlling the presentation of information by using at least one menu. 54. The method of claim 41, further comprising: providing at least one program; storing the program; and executing the stored program with the microprocessor-based subsystem. 55. The method of claim 54, wherein at least part of the provided program is on an insertable card or cartridge. 56. The method of claim 41, wherein the step of communicating is done using RF transmission. 57. A heart condition monitoring system, comprising: means for monitoring a heart condition of a person by using at least one microprocessor-based subsystem means comprising at least one microprocessor, display and memory; means for presenting information on the display to the person; means for producing a digital signal representative of the monitored heart condition; means for opening and closing a line of communication between the means for monitoring and a central server; means for communicating heart condition-related data to the central server; means for providing health-related information, based on the heart condition-related data communicated to the central server, to at least one health care professional; and means for providing a message from the at least one health care professional through the central server to the person, where the means for providing stores the message in the central server until a new connection is opened between the means for monitoring and the central server. 58. The system of claim 57, wherein the means for communicating is adapted to communicate using RF transmission.
