The script program is executable by a remote apparatus that displays information and/or a set of queries to the individual through a user interface. Responses to the queries that are entered through the user interface together with individual identification information are sent from the remote apparatus to the server system across a communication network. The server system also includes an automated answering service for providing a series of questions from a stored set of questions for an individual at the remote apparatus to respond to, storing responses to each provided question in the series of questions and providing a service based on the individual's response to the questions.

The invention claimed is: 1. A monitoring system configured to allow at least one individual to communicate with a computer system using an apparatus remotely situated from the computer system, wherein the apparatus comprises: a network interface for establishing a communication link between the apparatus and the computer system through a communication network; a user interface for displaying queries and response choices provided by a script program received from the computer system and for receiving responses to the queries from the at least one individual; a memory for storing the script program and the responses; and a processor connected to the network interface, the user interface and the memory, wherein the processor is configured to execute the script program to display the queries and the response choices, to receive the responses, to establish the communication link, and to transmit the responses to the computer system, and wherein the script program further includes a connection command to establish the communication link and a transmit command to transmit the responses from the apparatus to the computer system. 2. The monitoring system according to claim 1, wherein the user interface comprises a display for displaying the queries and response choices provided by the script program. 3. The monitoring system according to claim 2, wherein the user interface comprises a plurality of input buttons located adjacent to the display, and wherein each of the response choices for a corresponding query are displayed proximate a respective one of the input buttons. 4. The monitoring system according to claim 1, wherein the user interface comprises a touch-sensitive display screen. 5. The monitoring system according to claim 4, wherein the responses are received through the touch-sensitive display screen. 6. The monitoring system according to claim 1, wherein the connection command specifies a prescribed connection time for establishing the communication link. 7. The monitoring system according to claim 1, wherein the apparatus further includes a notification device connected to the processor and configured for notifying the individual when unanswered queries are stored in the apparatus. 8. The monitoring system according to claim 7, wherein the notification device comprises a visual indicator for visually notifying the individual. 9. The monitoring system according to claim 7, wherein the notification device comprises a sound transducer for audibly notifying the individual. 10. The monitoring system according to claim 7, wherein the notification device comprises a speech synthesizer for audibly notifying the individual. 11. The monitoring system according to claim 1, wherein: the apparatus further comprises a device interface connected to the processor and configured for receiving measurements from at least one monitoring device for producing measurements of a physiological condition of the individual and for transmitting the measurements to the apparatus; the script program further includes a collect command to collect the measurements from the at least one monitoring device; the memory is further configured to store the measurements; the network interface is further configured to transmit the measurements to the computer system for storage in a database. 12. The monitoring system according to claim 11, wherein the device interface is configured to interface with a plurality of monitoring devices, and wherein the collect command specifies a selected monitoring device from which to collect the measurements. 13. The monitoring system according to claim 12, wherein the device interface comprises a plurality of jacks. 14. The monitoring system according to claim 1, wherein the apparatus further includes a housing sufficiently compact to enable the apparatus to be hand-held and carried by the individual. 15. The monitoring system according to claim 1, wherein the apparatus further comprises a speech recognizer. 16. The monitoring system according to claim 1, wherein the apparatus is further configured to collect data for an identification check. 17. A method for communicating with a computer using an apparatus remotely situated from the computer comprising the steps of: A) establishing a communication link between the apparatus and the computer through a communication network; B) receiving a script program containing queries and corresponding response choices from the computer; and C) storing the script program containing the queries and corresponding response choices, wherein execution of the script program causes the apparatus to communicate the queries and the response choices to an individual, to receive responses to the queries from the individual, and to transmit the responses to the computer. 18. The method according to claim 17, further comprising the step of notifying the individual when unanswered queries are stored in the apparatus. 19. The method according to claim 18, wherein the step of notifying the individual comprises lighting a visual indicator. 20. The method according to claim 18, wherein the step of notifying the individual comprises emitting tones from a sound transducer. 21. The method according to claim 17, wherein the step of notifying the individual comprises speech synthesis to synthesize the queries, response choices and prompts through a speaker. 22. The method according to claim 17, wherein communicating the queries and the response choices to the individual comprises displaying the queries and the response choices on a display. 23. The method according to claim 17, wherein receiving responses to the queries from the individual comprises determining which of a plurality of user input buttons is pressed. 24. The method according to claim 17, wherein receiving responses to the queries from the individual comprises determining where a touch-sensitive display screen is touched. 25. The method according to claim 17, wherein receiving responses to the queries from the individual comprises performing speech recognition of responses spoken into a microphone. 26. The method according to claim 17, further comprising the steps of: receiving measurements of a physiological condition of the individual from a monitoring device in response to a collect command of the script program; storing the measurements; and transmitting the measurements from the apparatus to the computer system through the communication link. 27. The method according to claim 26, wherein the collect command specifies a selected monitoring device of a plurality of monitoring devices from which to collect the measurements, and the method further comprises the step of prompting the individual to connect the selected monitoring device to the apparatus. 28. The method according to claim 26, wherein the monitoring device comprises a device selected from the group consisting of blood glucose meter, peak flow meter, electrocardiograph (ECG or EKG), blood pressure cuff, weight scale and pulse rate monitor. 29. The method according to claim 17, wherein the step of receiving the script program containing the queries and corresponding response choices from the computer further comprises the step of sending an identification code to the computer, and wherein the script program received is based upon the identification code. 30. An apparatus for communicating with a computer using an apparatus remotely situated from the apparatus, the apparatus comprising: a communication interface for receiving a script program containing a set of queries and response choices from the computer and for transmitting responses to the set of queries to the computer; a user interface for communicating the set of queries and response choices to an individual and for receiving the responses to the set of queries from the individual; a memory configured to store the script program and the responses to the set of queries; and a processor connected to the communication interface, the user interface, and the memory for executing the stored script program, wherein the stored script program when executed causes the apparatus to communicate the set of queries to the individual, to receive the responses to the set of queries from the individual, and to transmit the responses to the computer. 31. The apparatus of claim 30, wherein the user interface comprises a display for displaying the queries and user input buttons for entering the responses. 32. The apparatus of claim 30, wherein the user interface includes a speech synthesis component for audibly communicating the set of queries to the individual and a speech recognition component for receiving spoken responses to the set of queries. 33. The apparatus of claim 30, further comprising: at least one jack configured to connect the apparatus to at least one monitoring device for obtaining measurements of a physiological condition of the individual; and a device interface connected between the at least one jack and the processor, wherein the processor is configured to receive the measurements from the monitoring device, store the measurements in the memory, and transmit the measurements to the computer via the communication interface. 34. The apparatus of claim 33, wherein the device interface is configured to connect to a plurality of monitoring devices, and the stored script program specifies a selected one of the plurality of monitoring devices from which to collect the measurements. 35. The apparatus of claim 30, wherein the stored script program specifies a connection time at which to establish a communication link with the computer. 36. The apparatus of claim 30, further comprising one or more indicators selected from the group consisting of a visual indicator and an audible indicator for notifying the individual that unanswered queries are stored in the remotely programmable apparatus. 37. The apparatus of claim 36, wherein the visible indicator comprises displaying a prompt on a display. 38. The apparatus of claim 30, wherein the apparatus is adapted to allow the individual to select a time at which to respond to the set of queries. 39. A method for communicating with a computer using an apparatus remotely situated from the computer, the method comprising the steps of: exchanging data with the computer through a communication network, wherein the data includes a custom script program executable by the apparatus to communicate queries and response choices to an individual, to receive responses to the queries from the individual, and to transmit the responses to the computer; storing the custom script program and the responses to the queries in the apparatus; and executing the custom script program in the apparatus to communicate the queries, to receive the responses, and to transmit the responses to the computer. 40. The method of claim 39, wherein the custom script program contains information customized to the individual. 41. The method of claim 39, further comprising the steps of: collecting measurements of a physiological condition of the individual from a monitoring device through a device interface in response to a collect command of the custom script program; storing the measurements in the apparatus; and transmitting the measurements from the apparatus to the computer. 42. The method according to claim 41, wherein the monitoring device comprises a device selected from the group consisting of blood glucose meter, peak flow meter, electrocardiograph (ECG or EKG), blood pressure cuff, weight scale and pulse rate monitor. 43. The method of claim 39, further comprising the step of notifying the individual that unanswered queries are stored in the apparatus using one or more of indications selected from the group consisting of lighting a visual indicator, emitting one or more tones from a sound transducer, speech synthesis and displaying a prompt on a display. 44. The method of claim 39, wherein the queries are communicated to the individual through a speech synthesizer and the responses are received from the individual through a speech recognizer.
