A system and method for monitoring a group of patients having a chronic disease or ongoing health condition. The method includes the step of collecting from each patient a corresponding set of measurements of a control parameter of the health condition. Each set of measurements has a collection date. A control value is calculated for each patient from the corresponding set of measurements. The method further includes the steps of generating and displaying a group overview chart having one data point for each patient. Each data point indicates the control value calculated for the corresponding patient and a time period which has elapsed since the collection date of the patient's corresponding set of measurements. In a preferred embodiment, the method includes the additional steps of selecting from the group overview chart at least one of the patients represented thereon and transmitting supervisory instructions to the at least one selected patient.
I claim: 1. A multi-patient data management method for monitoring diabetic patients by a Health Care Professional, via a processor-based system, the processor-based system including a processor and one or more processor-readable media having digital code embedded therein for programming the process
I claim: 1. A multi-patient data management method for monitoring diabetic patients by a Health Care Professional, via a processor-based system, the processor-based system including a processor and one or more processor-readable media having digital code embedded therein for programming the processor, the method comprising: (a) with a plurality of recording devices of a plurality of patients diagnosed with a diabetic condition, recording sets of chronologically-tracked values of diabetes care parameters including measurements of glucose levels of the patients; (b) transmitting each of the stored sets of diabetes care parameter values from the recording devices to a master patient database of the system; (c) collecting and storing at the master patient database the sets of chronologically-tracked diabetes care parameter values including the measurements of glucose levels; (d) processing the stored sets of diabetes care parameter values via the processor-based system, including: comparing certain of the diabetes care parameter values with diabetes care regimens of the patients, said diabetes care regimens being stored in the master patient database; determining a compliance of each of said patients with prescribed measurement regimens included in the stored diabetes care regimens by comparing measurement dates and times provided by the diabetes care parameter values with prescribed measurement dates and times provided by the diabetes care regimens; and determining a completeness of the one or more sets of diabetes care parameter values by comparing measurement frequency values included in the diabetes care parameter values with prescribed measurement frequency values provided by the diabetes care regimens; (e) outputting results of the processing including: indicating in said results via a plurality of compliance indication icons and a plurality of completeness indication icons the determined compliance of the patients with said prescribed measurement regimens and the determined completeness of the patients in following said prescribed measurement regimens to enable a Health Care Professional to assess diabetic conditions of the patients based on the collected sets of diabetes care parameter values, wherein the results are provided as a group overview chart, the group overview chart being displayed via a display of system, the group overview chart allowing for the output results for a first patient included in the plurality of patients and a second patient included in the plurality of patients to be concurrently viewed, a first compliance indication icon included in the plurality of compliance indication icons indicating a determined compliance of the first patient with a first prescribed measurement regimen included in the prescribed measurement regimens, a second compliance indication icon included in the plurality of compliance indication icons indicating a determined compliance of the second patient with a second prescribed measurement regimen included in the prescribed measurement regimens, a first completeness indication icon included in the plurality of completeness indication icons indicating a determined completeness of the first patient in following the first prescribed measurement regimen, a second completeness indication icon included in the plurality of completeness indication icons indicating a determined completeness of the second patient in following the second prescribed measurement regimen; and transmitting supervisory instructions via the system to one or more selected diabetic patients included in the plurality patients based on said assessment by the Health Care Professional of the output results. 2. The method of claim 1, wherein the method further comprises uploading the sets of diabetes care parameter values recorded in the recording devices to one or more personal computing devices and transmitting to the master patient database from the one or more personal computing devices. 3. The method of claim 1, wherein at least one of said recording devices comprises a remote monitoring device. 4. The method of claim 1, wherein at least one of said recording devices comprises an electronic logbook. 5. The method of claim 1, wherein the method includes calculating one or more control values from one or more of the sets of diabetes care parameter values indicating a degree of control over the diabetic condition for one or more of the diabetic patients. 6. The method of claim 1, wherein said supervisory instructions are transmitted by one or more electronic mail messages. 7. The method of claim 1, wherein the chart comprises one or more plots of control blood glucose values versus time period since last collection. 8. The method of claim 1, wherein the chart comprises one or more plots of glucose values versus time. 9. A system for monitoring diabetic patients, comprising a) a plurality of recording devices of a plurality of patients diagnosed with a diabetic condition, the recording devices recording sets of chronologically-tracked values of diabetes care parameters including measurements of glucose levels of the patients; b) a master patient database for storing each of the sets of diabetes care parameter values recorded in the recording devices; c) a communication network for transmitting each of the stored sets of diabetes care parameter values from the recording devices to the master patient database; d) a computing device including a processor and one or more processor-readable media having digital code embedded therein for programming the processor to process the stored diabetes care parameter values and to output results of the processing to enable a Health Care Professional to assess diabetic conditions of the patients based on the collected sets of diabetes care parameter values, wherein the master patient database also has stored therein diabetes care regimens of the multiple patients, and the digital code for programming the processor to compare said diabetes care parameter values with said regimens, wherein comparing the parameter values with said regimens includes: determining, via a component of the processor, a compliance of each of said patients with prescribed measurement regimens included in the stored diabetes care regimens by comparing measurement dates and times provided by the diabetes care parameter values with prescribed measurement dates and times provided by the diabetes care regimens; and determining, via said processor, a completeness of the one or more sets of diabetes care parameter values by comparing measurement frequency values included in the diabetes care parameter values with prescribed measurement frequency values provided by the diabetes care regimens, the output results indicating via a plurality of compliance indication icons and a plurality of completeness indication icons the determined compliance of the patients with said prescribed measurement regimens and the determined completeness of the patients in following said prescribed measurement regimens, the results being provided as a figure, the figure including a chart, the chart being a group overview chart, the group overview chart being displayed via a display of system, the overview chart allowing for the output results for a first patient included in the plurality of patients and a second patient included in the plurality of patients to be concurrently viewed, a first compliance indication icon included in the plurality of compliance indication icons indicating a determined compliance of the first patient with a first prescribed measurement regimen included in the prescribed measurement regimens, a second compliance indication icon included in the plurality of compliance indication icons indicating a determined compliance of the second patient with a second prescribed measurement regimen included in the prescribed measurement regimens, a first completeness indication icon included in the plurality of completeness indication icons indicating a determined completeness of the first patient in following the first prescribed measurement regimen, a second completeness indication icon included in the plurality of completeness indication icons indicating a determined completeness of the second patient in following the second prescribed measurement regimen; and an automated response component for transmitting supervisory instructions to one or more selected diabetic patients. 10. The system of claim 9, further comprising one or more further computing devices for uploading the sets of diabetes care parameters values recorded in the recording devices and transmitting to the master patient database. 11. The system of claim 9, wherein at least one of said recording devices comprises a remote monitoring device. 12. The system of claim 9, wherein at least one of said recording devices comprises an electronic logbook. 13. The system of claim 9, wherein the results include one or more control values calculated from one or more of the sets of diabetes care parameters values indicating a degree of control over the diabetic condition for one or more of the diabetic patients. 14. The system of claim 9, wherein said automated response component comprises an electronic mail component for transmitting said supervisory instructions in an electronic mail message. 15. The system of claim 9, wherein the chart comprises one or more plots of control blood glucose values versus time period since last collection. 16. The system of claim 9, wherein the chart comprises one or more plots of glucose values versus time. 17. A method of multi-patient data management of diabetic patients, comprising: (a) with a plurality of recording devices of a plurality of patients diagnosed with a diabetic condition, recording sets of chronologically-tracked values of diabetes care parameters including measurements of glucose levels of the patients; (b) transmitting each of the stored sets of diabetes care parameter values from the recording devices to a master patient database; (c) collecting and storing at the master patient database the sets of chronologically-tracked diabetes care parameter values including the measurements of glucose levels; (d) processing the stored diabetes care parameter values via the processor-based system, including: comparing certain of the diabetes care parameter values with diabetes care regimens of the patients, said diabetes care regimens being stored in the master patient database; determining a compliance of each of said patients with prescribed measurement regimens included in the stored diabetes care regimens by comparing measurement dates and times provided by the diabetes care parameter values with prescribed measurement dates and times provided by the diabetes care regimens; and determining a completeness of the one or more sets of diabetes care parameter values by comparing measurement frequency values included in the diabetes care parameter values with prescribed measurement frequency values provided by the diabetes care regimens; (e) outputting results of the processing including: indicating in said results via a plurality of compliance indication icons and a plurality of completeness indication icons the determined compliance of the patients with said prescribed measurement regimens and the determined completeness of the patients in following said prescribed measurement regimens to enable a Health Care Professional to assess diabetic conditions of the patients based on the collected sets of diabetes care parameter values, wherein the output results of the plurality of patients are provided via a display as a group overview chart, the group overview chart allowing for the output results for a first patient included in the plurality of patients and a second patient included in the plurality of patients to be concurrently viewed, a first compliance indication icon included in the plurality of compliance indication icons indicating a determined compliance of the first patient with a first prescribed measurement regimen included in the prescribed measurement regimens, a second compliance indication icon included in the plurality of compliance indication icons indicating a determined compliance of the second patient with a second prescribed measurement regimen included in the prescribed measurement regimens, a first completeness indication icon included in the plurality of completeness indication icons indicating a determined completeness of the first patient in following the first prescribed measurement regimen, a second completeness indication icon included in the plurality of completeness indication icons indicating a determined completeness of the second patient in following the second prescribed measurement regimen; and transmitting supervisory instructions to one or more selected diabetic patients. 18. The method of claim 17, further comprising uploading the sets of diabetes care parameter values recorded in the recording devices to one or more personal computing devices and transmitting to the master patient database from the one or more personal computing devices. 19. The method of claim 17, wherein at least one of said recording devices comprises a remote monitoring device. 20. The method of claim 17, wherein at least one of said recording devices comprises an electronic logbook. 21. The method of claim 17, further comprising calculating one or more control values from one or more of the sets of diabetes care parameter values indicating a degree of control over the diabetic condition for one or more of the diabetic patients. 22. The method of claim 17, wherein said supervisory instructions are transmitted by one or more electronic mail messages. 23. The method of claim 17, wherein said results comprise a plurality of displayed icons indicating a completeness of one of the sets of diabetes care parameter values. 24. The method of claim 17, wherein the chart comprises one or more plots of control blood glucose values versus time period since last collection. 25. The method of claim 17, wherein the chart comprises one or more plots of glucose values versus time.
