Various methods and apparatus are implemented for modifying cardiac operation, for treating a condition of a patient's heart, and for monitoring cardiac operation. An electrode arrangement is used to capture the myocardium for re-synchronization of the left and right ventricles by providing first and second signal components having opposite polarity on respective electrodes. Use of a defibrillation lead may also be particularly useful. Other aspects involve use of such cardiac modulation technique to facilitate mechanically and/or electrically synchronous contractions for resynchronization, and a technique for pacing and/or mapping by generating pulses for delivery to a cardiac site useful for improving heart function as measured, e.g., by QRS width, fractionation, late LV activation timing, mechanical synchronicity of free wall and septal wall, effective throughput/pressure, or a combination thereof.

For use in a myocardium capture by delivering pacing signals via electrodes to a target region in the right ventricle, at or near the septum of a heart and at or near a bundle of His and at or near the right ventricular outflow tract and distal from an apex of the right ventricle, a circuit arrangement comprising:first and second conductors, configured to be respectively coupled to the electrodes, for providing pacing signals of opposing polarity to the target region at or near the septum of the heart and at or near the bundle of His, and a third conductor configured for providing a defibrillation signal to the right ventricle apex; anda control module configured for generating the pacing signals, with two pulses occurring sufficiently proximate one another in at least one of time or distance for the myocardium capture near the target region at or near the septum of the heart and at or near the bundle of His, on the first and second conductors and for generating the defibrillation signal.
